Abstract

A method of performing an orthopedic procedure in the sacroiliac region. At least one aperture is formed that at least partially extends through at least one of an ilium and a sacrum. An undercutting system is at least partially inserted into the aperture. The undercutting system includes an insertion apparatus, a probe assembly and a cutting assembly. The probe assembly is moved with respect to the insertion apparatus from a retracted position to an extended position. The probe assembly is manipulated within a joint between the ilium and the sacrum while the probe assembly is in the extended position. The cutting assembly is moved with respect to the insertion apparatus from a retracted position to an extended position. The cutting assembly is sharper than the probe assembly. The cutting assembly is manipulated within the joint between the ilium and the sacrum while the cutting assembly is in the extended position to form a fusion region. The undercutting system is removed from the aperture.

Claims (29)

1. A method of performing a sacroiliac fusion procedure, wherein the method comprises:

providing a cutting assembly having a distal end;

forming an aperture in an ilium;

positioning the cutting assembly in a retracted configuration;

extending the distal end of the cutting assembly through the aperture in the ilium;

moving the cutting assembly to an extended configuration where the distal end pivots with respect to a non-pivoted portion of the cutting assembly;

rotating the cutting assembly to cut tissue between the ilium and the sacrum;

withdrawing the cutting assembly from the aperture in the ilium; and

inserting a fastening device through the aperture in the ilium and into the sacrum.

2. The method of claim 1 , wherein rotating the cutting assembly to cut tissue includes cutting tissue located transversely beyond an outer extent of the aperture in the ilium.

3. The method of claim 1 , wherein rotating the cutting assembly includes rotating the cutting assembly about an axis.

4. The method of claim 1 , wherein rotating the cutting assembly includes rotating the cutting assembly in at least two different directions.

5. The method of claim 1 , further comprising moving the cutting assembly to the retracted configuration before the cutting assembly is withdrawn from the aperture in the ilium.

6. A method of performing a sacroiliac fusion procedure, wherein the method comprises:

forming an aperture in an ilium;

extending a cutter through the aperture in the ilium;

positioning the cutter between the ilium and a sacrum;

rotating the cutter to cut tissue of at least one of the ilium and the sacrum or between the ilium and the sacrum; and

withdrawing the cutter from the aperture in the ilium.

7. The method of claim 6 , further comprising cutting tissue of the at least one of the ilium and the sacrum or between the ilium and the sacrum transversely beyond an outer extent of the aperture formed in the ilium.

8. The method of claim 6 , further comprising moving the cutter to an extended configuration, wherein the cutter extends transversely beyond an outer extent of the aperture formed in the ilium.

9. The method of claim 8 , wherein moving the cutter to the extended configuration includes shifting the cutter from within an insertion apparatus to a position between the sacrum and the ilium.

10. The method of claim 8 , further comprising shifting the cutter to the extended configuration prior to rotating the cutter.

11. The method of claim 6 , wherein rotating the cutter includes rotating the cutter about an axis extending through the aperture.

12. The method of claim 11 , further comprising rotating the cutter in first and second directions about the axis.

13. The method of claim 6 , further comprising moving the cutter to a retracted configuration wherein the cutter is positioned within an insertion apparatus, before the cutter is withdrawn from the aperture in the ilium.

14. The method of claim 6 , further comprising inserting a fastening device through the aperture in the ilium and into the sacrum.

15. The method of claim 6 , wherein the aperture formed in the ilium has a circular cross-section.

16. The method of claim 6 , wherein the cutter has a non-circular cross-section.
