Abstract

An assembly for restoring a gastroesophageal flap valve includes a restoration device substantially free for rotation. The assembly comprises an elongated member configured to be fed through a throat, down an adjoining esophagus and into an associated stomach. The elongated member has a distal end. The assembly further comprises a gastroesophageal flap valve restoration device carried on the distal end of the elongated member for placement in the stomach, and an invaginator carried by the elongated member. The invaginator is configured to grip the esophagus and the elongated member and invaginator are coupled for restricted relative axial movement and substantially free relative rotational movement.

Claims (41)

1. A method of restoring a flap of a gastroesophageal flap valve associated with a stomach and esophagus, comprising:

inflating the stomach;

gripping a tissue wall of the esophagus using an invaginator having a plurality of orifices to pull a vacuum;

gripping stomach tissue from within the stomach at a point displaced from an esophageal opening of the stomach;

pulling the gripped stomach tissue into a mold to form a first fold of stomach tissue;

pulling the stomach downwardly to align a fold line in the first fold of stomach tissue with the esophageal opening of the stomach;

deploying a first fastener through the first fold of stomach tissue; and

rotating the invaginator while maintaining an axial position of the invaginator relative to the esophageal wall in order to form a sequential, second fold of stomach tissue.

2. The method of claim 1 , wherein the stomach is adjacent to the esophagus and wherein the step of gripping the tissue wall of the esophagus occurs prior to gripping the stomach tissue.

3. The method of claim 1 , further comprising deflating the stomach after deploying the first fastener.

4. The method of claim 1 , further comprising releasing the grip on the stomach tissue after the first fastener is deployed.

The method of claim 1 , further comprising maintaining the gripping of the tissue wall of the esophagus and repeating the inflating, gripping of stomach tissue, pulling, deploying, and rotating steps for forming the second fold of stomach tissue and deploying a second fastener displaced from the first fastener.

The method of claim 1 , wherein the inflating step includes inflating the stomach to a first pressure, wherein the method further comprises visualizing the stomach from within the stomach after inflating the stomach to the first pressure, and increasing inflation pressure within the stomach to a second pressure after visualizing the stomach before gripping stomach tissue.

7. A method of deploying a fastener through stomach tissue a predetermined distance from a gastroesophageal junction associated with a stomach and esophagus, comprising:

inflating the stomach;

providing a fastener deployment apparatus and a fastener deployer that ejects a fastener for deployment into the stomach tissue;

advancing the apparatus through the esophagus toward the stomach;

aligning a location marker with respect to the gastroesophageal junction to cause the fastener deployer to be aboral of the gastroesophageal junction;

gripping a tissue wall of the esophagus using an invaginator having a plurality of orifices to pull a vacuum;

ejecting a first fastener from the fastener deployer into the stomach tissue with the location marker aligned with respect to the gastroesophageal junction; and

rotating the invaginator while maintaining an axial position of the invaginator relative to the esophageal wall in order to eject a second fastener from the fastener deployer into the stomach tissue.

8. The method of claim 7 , wherein a chassis and a bail are hingedly coupled so that as the bail is pivoted on the hinge toward the chassis a fold of stomach tissue is formed therebetween.

9. The method of claim 8 , further comprising inflating the stomach before deploying the first fastener.

10. The method of claim 9 , wherein the gripping step occurs prior to forming the fold of stomach tissue.

11. The method of claim 10 , wherein the step of gripping the wall of the esophagus includes gripping the wall of the esophagus with the vacuum.

12. The method of claim 11 , further comprising releasing the grip on the esophageal wall after the second fastener is deployed.

13. The method of claim 12 , further comprising, before the gripping step, inflating the stomach to a first pressure, visualizing the stomach from within the stomach, and inflating the stomach to a second pressure, the second pressure being greater than the first pressure.

14. The method of claim 7 , wherein after ejecting the second fastener, repeating the inflating, ejecting, and rotating steps for deploying a third fastener into the stomach tissue.

15. A method of forming a gastroesophageal flap valve associated with a stomach and esophagus, comprising:

providing a device having a gripping element and a fastening element;

advancing the device into the esophagus;

inflating the stomach;

aligning an invaginator with an esophageal wall and drawing a vacuum through a plurality of orifices in the invaginator to fix the axial position of the device relative to the esophageal wall;

viewing the stomach while the stomach is inflated to select an area to grip stomach tissue with the gripping element;

gripping the stomach tissue with the gripping element after the viewing step;

moving the stomach tissue with the gripping element;

forming a fold of tissue using a first tissue layer and a second tissue layer displaced during the moving step;

deploying the fastening element through the first tissue layer and the second tissue layer;

the inflating step being carried out by inflating the stomach to a first pressure during the viewing step and increasing inflation pressure within the stomach to a second pressure after the viewing step and before the gripping step; and

rotating the invaginator while maintaining an axial position of the invaginator relative to the esophageal wall in order to form a sequential, second fold of stomach tissue.

16. The method of claim 15 , wherein the deploying step is carried out so that the fold of tissue formed by the gripped stomach tissue forms the gastroesophageal flap valve between the stomach and the esophagus.
