Abstract

Methods and surgical tools for treating back pain use a spinal facet debridement tool with cautery and denuding action and minimally invasive protocol that can denude and cauterize soft tissue associated with a synovial capsule of the spinal facet joint whereby the synovial capsule between the spinal facet joint is removed by rotating the head of the combination debrider tool at a low rotation speed to scrape and cauterize the end plate receptor region so as to denude and denervate the joint preventing regeneration, and thereby treating back pain.

Claims (19)

1. A set of surgical tools for spinal facet surgical procedures for alleviating spinal pain, comprising: a debrider tool having a distal end with a denuding and cauterization head comprising an electrically conductive cautery element, wherein the debrider tool comprises or is in communication with a motor that drives the denuding and cauterization head, wherein the denuding and cauterization head comprises a plurality of electrically non-conductive and radially extending projections defining denuding elements, wherein the radially extending projections defining the denuding elements include a first denuding element and a second denuding element that are linear and diametrically opposed along a single axis; a cannula sized and configured with a longitudinally extending open primary channel that is sized and configured to slidably receive, in a longitudinal direction, at least the distal end of the debrider tool; and an external stabilizer configured with a bottom that is adapted to reside against skin of a patient and that has a maximal width, wherein the bottom merges into or is coupled to a tubular segment with an open through channel that extends a distance above the bottom, wherein the tubular segment has a width that is smaller than the maximal width of the bottom of the external stabilizer, and wherein the tubular segment releasably holds the cannula, with the debrider tool inside the cannula and with the distal end of the debrider tool extending out of the bottom of the external stabilizer, wherein the cannula couples to the tubular segment of the external stabilizer in a height-adjustable longitudinal position, and wherein, the debrider tool cooperates with a proximal end of the cannula so that, in position, the distal end of the debrider tool extends out a distal end of the cannula only a distance in a range of about 2 mm to about 7 mm.

2. The tools of claim 1 , wherein the maximal width of the bottom of the external stabilizer is between about 2-6 inches.

3. The tools of claim 1 , wherein the maximal width of the bottom of the external stabilizer is between about 3-5 inches.

The tools of claim 1 , wherein the tubular segment has a height that is between about 2-10 inches, and wherein the debrider tool comprises a drive shaft that has non-conductive and longitudinally extending flutes with a distal end portion that holds the cautery element and that provides the non-conductive, radially extending denuding elements that together define the denuding and cauterization head.

The tools of claim 1 , wherein the debrider tool comprises a barrel that forms at least part of a drive shaft of the denuding and cauterization head or encases a drive shaft of the denuding and cauterization head, and wherein the barrel has a length that is greater than a height of the tubular segment.

6. The tools of claim 1 , wherein the tubular segment has a height that is between about 3-6 inches.

7. The tools of claim 1 , wherein the bottom of the external stabilizer has a thickness of between about 1-10 mm, wherein the maximal width is between 2-6 inches, and wherein the tubular segment has a height that is between 2-10 inches.

8. The tools of claim 1 , wherein the bottom of the external stabilizer has a thickness that is between about 2-4 mm.

The tools of claim 1 , wherein the bottom of the stabilizer is semi-rigid, wherein the bottom of the external stabilizer has a planar bottom surface that is configured to contact skin of the patient with a width that is between 2-6 inches and wherein the bottom surface is configured to conformably reside against the skin of the patient.

10. The tools of claim 1 , wherein the debrider tool comprises a handle that is axially offset from the denuding and cauterization head, wherein the cannula is sized and configured to cooperate with the debrider tool to define a longitudinal stop for the debrider tool.

11. The tools of claim 10 , wherein the cannula comprises an electrically insulating material, and wherein the cannula comprises visible indicia of depth position.

12. The tools of claim 11 , wherein the visible indicia of depth comprises a graduated scale on an outer wall.

13. The tools of claim 1 , wherein the maximal width of the bottom of the external stabilizer is between 2-10 times greater than a width of the cannula.

14. The tools of claim 1 , wherein a gap space extends between inner ends of the first and second denuding elements, and wherein the cautery element resides in the gap space.

15. The tools of claim 12 , wherein the cautery element is a straight linear cautery element that resides in the gap space and that is orthogonal to the first and second denuding elements.

16. The tools of claim 1 , wherein the denuding and cauterization head of the debrider tool has a maximum rotation speed of between about 10 rpm to about 5000 rpm.

17. The tools of claim 1 , wherein the debrider tool and the cannula each comprise visual indicia of a depth position.

A set of surgical tools for spinal facet surgical procedures for alleviating spinal pain, comprising: a debrider tool having a distal end with a denuding and cauterization head comprising an electrically conductive cautery element, wherein the debrider tool comprises or is in communication with a motor that drives the denuding and cauterization head, wherein the denuding and cauterization head comprises a first denuding element and a second denuding element that are (i) electrically non-conductive, (ii) linear and (iii) diametrically opposed along a single axis; a cannula sized and configured with a longitudinally extending open primary channel that is sized and configured to slidably receive, in a longitudinal direction, at least the distal end of the debrider tool; and an external stabilizer configured with a bottom that is adapted to reside against skin of a patient and that has a maximal width, wherein the bottom merges into or is coupled to a tubular segment with an open through channel that extends a distance above the bottom, wherein the tubular segment has a width that is smaller than the maximal width of the bottom of the external stabilizer, wherein the tubular segment releasably holds the debrider tool with the distal end of the debrider tool extending out of the bottom of the external stabilizer, wherein the bottom of the stabilizer is semi-rigid, wherein the bottom of the external stabilizer has a planar bottom surface that is configured to contact the skin of the patient with a width that is between 2-6 inches, and wherein the bottom surface is configured to conformably reside against the skin of the patient.

The tools of claim 18 , wherein the first and second denuding elements have a gap space therebetween, wherein the gap space resides in a medial location of a distal end of the denuding and cauterization head, wherein the first and second denuding elements project distally in the longitudinal direction, and wherein a cautery element resides in the gap space between the first and second denuding elements.
