Abstract

A device includes a handle, an expandable structure including a plurality of splines extending from a proximal hub to a distal hub, a first electrode on a first spline of the plurality of splines, an outer tube extending from the handle to the proximal hub, and a shaft extending through the outer tube from the handle to the distal hub. The expandable structure has a collapsed state and a self-expanded state. The handle is configured to retract the shaft. Retracting the shaft may expand the expandable structure outward of the self-expanded state.

Claims (48)

1. A method of reducing a duty cycle during a neurostimulation treatment period while maintaining an efficacious therapeutic effect, the method comprising:

during a first duration, alternatingly:

applying a neurostimulation to a nerve in a chest cavity; and

stopping application of the neurostimulation;

during a second duration, wherein said second duration is after the first duration and wherein no neurostimulation is applied during the second duration;

monitoring a physiologic signal comprising at least one of cardiac contractility, arterial blood pressure, central venous pressure, capillary pressure, systolic pressure variation, blood gases, cardiac output, systemic vascular resistance, pulmonary artery wedge pressure, gas composition of exhaled breath, or mixed venous oxygen saturation; and

storing a post-stimulation level of the physiologic signal;

after the second duration, monitoring the physiologic signal for deviation from the post-stimulation level by a threshold value;

detecting the deviation greater than the threshold value; and

after detecting the deviation, for a third duration, alternatingly applying the neurostimulation and stopping applying the neurostimulation until the deviation is less than the threshold value, thereby reducing the duty cycle during the neurostimulation treatment period while maintaining the efficacious therapeutic effect, the efficacious therapeutic effect being increasing cardiac contractility.

2. The method of claim 1 , wherein applying the neurostimulation during the first duration is until achieving a steady state of the physiologic signal and comprises alternatingly turning the neurostimulation ON and OFF.

3. The method of claim 2 , wherein a parameter of the neurostimulation ON increases from an initial value to a final value during the first duration.

4. The method of claim 3 , wherein the parameter comprises at least one of ON duration, amplitude, pulse width, or frequency.

5. The method of claim 2 , wherein turning the neurostimulation OFF comprises allowing decay of the physiologic signal.

6. The method of claim 5 , comprising reducing the decay over time.

7. The method of claim 5 , wherein periods of turning the neurostimulation OFF are shorter than periods of turning the neurostimulation ON.

8. The method of claim 1 , wherein applying the neurostimulation until the physiologic signal is at the post-stimulation level during the third duration comprises alternatingly turning the neurostimulation ON and OFF.

9. A method of reducing a duty cycle during a neurostimulation treatment period while maintaining an efficacious therapeutic effect, the method comprising:

during a first duration, alternatingly:

applying a neurostimulation in a chest cavity; and

stopping applying the neuro stimulation;

after the first duration, for a second duration:

not applying the neuro stimulation;

monitoring a physiologic signal; and

storing a post-stimulation level of the physiologic signal;

after the second duration, monitoring the physiologic signal for deviation from the post-stimulation level by a threshold value;

detecting the deviation greater than the threshold value; and

after detecting the deviation, for a third duration, alternatingly applying the neurostimulation and stopping applying the stimulation until the deviation is less than the threshold value, the efficacious therapeutic effect being increasing cardiac contractility.

10. The method of claim 9 , comprising, during the first duration, increasing a parameter of applying the neurostimulation from an initial value to a final value.

11. The method of claim 10 , wherein the parameter comprises at least one of ON duration, amplitude, pulse width, or frequency.

12. The method of claim 9 , comprising, during the first duration, allowing decay of the physiologic signal during stopping applying the neurostimulation.

13. The method of claim 12 , comprising reducing the decay over time.

14. A method of reducing a duty cycle during a neurostimulation treatment period while maintaining an efficacious therapeutic effect, the method comprising:

during a first duration:

not applying a neuro stimulation;

monitoring a physiologic signal; and

storing a post-stimulation level of the physiologic signal;

after the first duration, monitoring the physiologic signal for deviation from the post-stimulation level by a threshold value;

detecting the deviation greater than the threshold value; and

after detecting the deviation, for a second duration, alternatingly applying the neurostimulation and stopping applying the stimulation in a chest cavity until the deviation is less than the threshold value, the efficacious therapeutic effect being increasing cardiac contractility.

15. The method of claim 14 , comprising, before the first duration:

monitoring the physiologic signal; and

applying the neurostimulation until achieving a steady state of the physiologic signal.

16. The method of claim 15 , wherein applying the neurostimulation until achieving the steady state of the physiologic signal comprises alternatingly turning the neurostimulation ON and OFF.

17. The method of claim 16 , wherein a parameter of applying the neurostimulation ON increases from an initial value to a final value during the first duration, and wherein the parameter comprises at least one of ON duration, amplitude, pulse width, or frequency.

18. The method of claim 16 , wherein turning the neurostimulation OFF comprises allowing decay of the physiologic signal, and wherein the method comprises reducing the decay over time.

19. The method of claim 16 , wherein periods of turning the neurostimulation OFF are shorter than periods of turning the neurostimulation ON.

20. The method of claim 14 , wherein periods of applying the neurostimulation are shorter than periods of stopping applying the neurostimulation during the second duration.
