Abstract

Systems, devices, and methods suitable for use with procedures performed at least partially percutaneously are provided. In some procedures, two or more access devices for providing access to adjacent surgical locations within a patient are used. Certain embodiments of the access device comprise an elongate body having a distal end with one or more cutouts. The cutouts on adjacent access devices are generally aligned with each other to permit passage of a portion of a fixation element from one access device to the other access device. A fastener with an elongated removable head may be delivered to the surgical site through the access device. After a distal end of the fastener is secured to the surgical site, a portion of the elongated housing is detached from the remainder of the fastener and removed from the patient.

Claims (29)

1. A pedicle screw assembly comprising:

a threaded shaft;

a head coupled to the threaded shaft and including a housing forming a U-shaped recess configured to receive a connecting rod;

an elongate body coupled to a proximal portion of the head at a break-off portion and extending, proximally from the proximal portion of the head, a length sufficient such that the elongated body extends above a patient's skin when the threaded shaft is secured within a patient's vertebra, the elongate body comprising a first slot extending from a first side of the U-shaped recess the length of the elongate body and a second slot extending proximally from a second side of the U-shaped recess a portion of the length of the elongate body, wherein a cross-section of the elongate body proximal the second slot forms a C-shape, wherein the elongated body is adapted to be removed from the housing at the break-off portion; and

a removal device including an outer sleeve, an inner sleeve receivable within the outer sleeve, a first handle connected to the inner sleeve, and a second handle connected to the outer sleeve.

2. The pedicle screw assembly of claim 1 , wherein the second slot is directly opposite the first slot, and the first slot and the second slot operate to extend the U-shaped recess of the housing.

3. The pedicle screw assembly of claim 2 , wherein the elongate body includes a first sidewall and a second sidewall extending from the housing between the first slot and the second slot.

4. The pedicle screw assembly of claim 3 , wherein the U-shaped recess is formed by threaded sidewalls for receiving a set screw; and

wherein the first sidewall and the second sidewall each include a threaded portion extending from the threaded sidewalk of the U-shaped recess.

5. The pedicle screw assembly of claim 1 , wherein the threaded shaft and the head are cannulated.

6. The pedicle screw assembly of claim 1 , wherein the housing includes one or more facets arranged in a hexagonal pattern on an outer surface adapted to receive a removal device.

7. The pedicle screw assembly of claim 1 , wherein the removal device is adapted to exert a differential torque or a shearing force on the break-off portion.

8. The pedicle screw assembly of claim 1 , wherein the inner sleeve is configured to extend distally into the elongate body with a distal end adjacent the break-off portion when fully inserted.

9. The pedicle screw assembly of claim 8 , wherein the outer sleeve is configured to extend distally over the elongate body and engage the housing when the removal device is fully engaged with the pedicle screw assembly.

10. The pedicle screw assembly of claim 9 , wherein the outer sleeve includes a distal hex shaped portion configured to engage hex-shaped facets on an outer surface of the housing.

11. The pedicle screw assembly of claim 8 , wherein rotation of the first handle with respect to the second handle imparts a shear stress on the distal end of the elongate body to detach the elongate body from the housing at the break-off portion.

12. An extended length reduction pedicle screw system, the pedicle screw system comprising:

a threaded shaft; and

a break-off head, the break-off head comprising:

a housing including two arms extending proximally to form a U-shaped recess adapted to receive a connecting rod;

an elongate body attached to the two arms of the housing at a neck portion, the elongate body including a first slot extending from the neck to a proximal end and a second slot extending from the neck part way to the proximal end, wherein the cross-section of the elongate body proximal to the second slot forms a C-shape, and wherein the neck portion has a reduced cross-section to facilitate removal of the elongate body in one piece; and

a removal device including an outer sleeve configured to extend distally over the elongate body and engage the break-off head when the removal device is fully engaged with the pedicle screw, an inner sleeve receivable within the outer sleeve and configured to extend distally into the elongate body with a distal end adjacent the neck portion when fully inserted, a first handle connected to the inner sleeve, and a second handle connected to the outer sleeve.

13. The pedicle screw system of claim 12 , wherein the second slot is directly opposite the first slot, and the first slot and the second slot operate to extend the U-shape recess of the housing.

14. The pedicle screw system of claim 13 , wherein the elongate body, includes a first sidewall and a second side wall extending from the housing between the first slot and the second slot.

15. The pedicle screw system of claim 14 , wherein the U-shaped recess is formed by threaded sidewalls for receiving a set screw; and

wherein the first sidewall and the second sidewall each include a threaded portion extending threads from the threaded sidewalls of the U-shaped recess and enable reduction of a connecting rod into the housing.

16. The pedicle screw system of claim 12 , wherein the threaded shaft and the break-off head are cannulated, and wherein the reduced cross-section of the neck is thinner in cross-section than the two arms of the housing as well as the elongate body proximal the neck.

17. The pedicle screw system of claim 12 , wherein the break-off head includes one or more facets on an outer surface adapted to receive a removal device.

18. The pedicle screw system of claim 12 , wherein the removal device adapted to exert a differential torque or a shearing force on the neck portion.
