Abstract

Provided herein are methods and systems of molecular profiling of diseases, such as cancer. In some embodiments, the molecular profiling can be used to identify treatments for the disease, such as treatments that provide likely benefit or likely lack of benefit for the disease. The molecular profiling can include analysis of immune modulators such as PD-1 and/or its ligand PD-L1.

Claims (21)

78 . A method comprising:

obtaining a biological sample from an individual diagnosed with cancer;

determining the levels of (1) PD-L1 and/or PD-1 expression; and (2) tumor-infiltrating lymphocytes (TILs);

comparing, to reference levels, each of the levels of (1) PD-L1 and/or PD-1 expression;

and (2) tumor-infiltrating lymphocytes (TILs);

generating a recommendation for treating the individual who has PD-L1 and/or PD-1 expression levels and/or TILs levels that are higher than the reference levels, the recommendation comprising treating the individual using one or more immune checkpoint inhibitor therapy; and

transmitting the recommendation electronically or on a tangible media to a researcher, a physician, a genetic counselor, a client, a customer, a consumer, an end user, a hospital, a business, or the individual.

79 . The method of claim 78 , comprising

additionally determining the level of genomic mutations and/or microsatellite instability in the biological sample of the individual; and wherein the individual who has PD-L1 and/or PD-1 expression levels and/or TILs levels that are higher than the reference levels also has genomic mutations and/or microsatellite instability levels that are higher than the reference levels.

80 . The method of claim 78 , wherein the immune checkpoint inhibitor therapy comprises treating the individual with a PD-1 inhibitor or a PD-L1 inhibitor.

81 . The method of claim 80 , wherein the PD-1 inhibitor comprises nivolumab, pidilizumab, pembrolizumab, or AMP-224.

82 . The method of claim 81 , wherein the PD-1 inhibitor comprises nivolumab.

83 . The method of claim 78 , wherein the TILs comprise T-cells.

84 . The method of claim 83 , wherein the T cells comprise CD8+ T-cells.

85 . The method of claim 83 , wherein the T cells comprise cytotoxic T-cells.

86 . The method of claim 78 , wherein the biological sample comprises cells from a solid tumor.

87 . The method of claim 78 , wherein the biological sample comprises fixed tissue.

88 . The method of claim 78 , wherein the expression of PD-L1 is determined in at least one of tumor cells, T cells, natural killer (NK) cells, macrophages, dendritic cells (DCs), B cells, epithelial cells, and vascular endothelial cells.

89 . The method of claim 78 , wherein the expression of PD-1 is determined in tumor infiltrating lymphocytes (TILs).

90 . The method of claim 78 , wherein the expression of PD-L1 and/or PD-1 comprises gene expression, protein expression, or both.
