Abstract

The present disclosure provides methods for evaluating daily ammonia exposure based on a single fasting ammonia blood level measurement, as well as methods that utilize this technique to adjust the dosage of a nitrogen scavenging drug, determine whether to administer a nitrogen scavenging drug, and treat nitrogen retention disorders.

Claims (17)

1. A method of treating a subject with hepatic encephalopathy who has previously been administered an initial dosage of glyceryl tri-[4-phenylbutyrate], said method comprising:

a) measuring a fasting blood ammonia level for the subject;

b) comparing the fasting blood ammonia level to the upper limit of normal for blood ammonia level and

c) administering an adjusted dosage of the glyceryl tri-[4-phenylbutyrate] that is greater than the initial dosage if the fasting blood ammonia level is greater than half the upper limit of normal for blood ammonia level.

2. The method of claim 1 , wherein prior to said administration, the subject exhibits an altered mental status.

3. The method of claim 1 , wherein the blood sample used for measuring fasting blood ammonia levels is a venous blood sample.

4. The method of claim 1 , wherein the blood sample used for measuring fasting blood ammonia levels is a plasma blood sample.

5. The method of claim 1 , wherein the fasting blood ammonia level is measured from a fasting morning blood draw.

6. The method of claim 1 , wherein the fasting period for obtaining a fasting blood ammonia level is 4 hours or more, 5 hours or more, 6 hours or more, 7 hours or more, 8 hours or more, 9 hours or more, 10 hours or more, 11 hours or more, or 12 hours or more.

7. The method of claim 1 , wherein the fasting period for obtaining a fasting blood ammonia level is 4-8 hours, 6-8 hours, or 8-12 hours.

8. The method of claim 1 , wherein the fasting period for obtaining a fasting blood ammonia level is overnight.

9. The method of claim 1 , wherein the upper limit of normal for blood ammonia level is 35 μmol/L.

10. The method of claim 1 , further comprising measuring urinary PAGN excretion and determining an effective dosage of the glyceryl tri-[4-phenylbutyrate] based on a mean conversion of PAA prodrug to urinary PAGN of 60-75%.

11. The method of claim 1 , wherein administering an adjusted dosage of the glyceryl tri-[4-phenylbutyrate] that is greater than the initial dosage comprises increasing the amount of glyceryl tri-[4-phenylbutyrate] per administration.

12. The method of claim 1 , wherein administering an adjusted dosage of the glyceryl tri-[4-phenylbutyrate] that is greater than the initial dosage comprises increasing the number of administrations of the glyceryl tri[4-phenylbutyrate].

The method of claim 1 , wherein administering an adjusted dosage of the glyceryl tri-[4-phenylbutyrate] that is greater than the initial dosage comprises increasing the amount of glyceryl tri-[4-phenylbutyrate] per administration and increasing the number of administrations of the glyceryl tri-[4-phenylbutyrate].

14. The method of claim 1 , further comprising repeating steps (a) to (c) until the subject exhibits a fasting blood ammonia level at or below half the upper limit of normal for blood ammonia level.
