Abstract

An implantable device for providing electrical stimulation of cervical vagus nerves for treatment of chronic cardiac dysfunction is provided. A stimulation therapy lead includes helical electrodes configured to conform to an outer diameter of a cervical vagus nerve sheath, and a set of connector pins electrically connected to the helical electrodes. A neurostimulator includes an electrical receptacle into which the connector pins are securely and electrically coupled. The neurostimulator also includes a pulse generator configured to therapeutically stimulate the vagus nerve through the helical electrodes in alternating cycles of stimuli application and stimuli inhibition that are tuned to both efferently activate the heart's intrinsic nervous system and afferently activate the patient's central reflexes by triggering bi-directional action potentials.

Claims (34)

1. A vagus nerve stimulator for treating chronic cardiac dysfunction, comprising:

an implantable neurostimulator comprising:

a pair of helical electrodes;

a nerve stimulation therapy lead electrically coupled to the pair of helical electrodes;

a pulse generator configured to deliver electrical therapeutic stimulation comprising a stimulation waveform applied to both of the helical electrodes via the nerve stimulation therapy lead; and

a recordable memory configured to store stimulation parameters comprising a programmable ON-time during which the pulse generator delivers the electrical therapeutic stimulation, a programmable OFF-time during which the electrical therapeutic stimulation is inhibited, and a periodic duty cycle comprising a percentage calculated from a ratio of the ON-time to the OFF-time;

wherein the periodic duty cycle is between a lower predefined percentage and an upper predefined percentage; and

wherein the pulse generator is further configured to operate under the stimulation parameters stored in the recordable memory.

2. The neurostimulator of claim 1 , wherein the lower predefined percentage is 5% and the upper predefined percentage is 30%.

3. The neurostimulator of claim 1 , wherein the stimulation parameters further comprise a programmable output current, a programmable signal frequency, and a programmable pulse width.

4. The neurostimulator of claim 3 , wherein the pulse width is within a range of 100 to 250 μsec, the output current is within a range of 0.02 to 50 mA, and the signal frequency is at least 10 Hz.

The neurostimulator of claim 3 , wherein the pulse generator is further configured to provide at least one of a ramp-up time during which the output current is progressively increased preceding therapeutic stimulation at a full output current or a ramp-down time during which the output current is progressively decreased following the therapeutic stimulation at the full output current.

6. The neurostimulator of claim 1 , wherein the stimulation waveform comprises a form that causes propagation of action potentials in both an afferent direction and an efferent direction within neuronal fibers of a cervical vagus nerve.

7. The neurostimulator of claim 1 , wherein the recordable memory is further configured to receive modified stimulation parameters via wireless telemetry from a programming wand.

8. A method for treating chronic cardiac dysfunction, comprising:

storing, by a recordable memory of a vagus nerve stimulator, stimulation parameters comprising a programmable ON-time during which a pulse generator of the vagus nerve stimulator delivers electrical therapeutic stimulation, a programmable OFF-time during which the electrical therapeutic stimulation is inhibited, and a periodic duty cycle comprising a percentage calculated from a ratio of the ON-time to the OFF-time, wherein the periodic duty cycle is between a lower predefined percentage and an upper predefined percentage; and

operating the pulse generator based on the stimulation parameters to deliver electrical therapeutic stimulation comprising a stimulation waveform, via a nerve stimulation therapy lead, to helical electrodes electrically coupled to the nerve stimulation therapy lead.

9. The method of claim 8 , wherein the lower predefined percentage is 5% and the upper predefined percentage 30%.

10. The method of claim 8 , wherein the stimulation parameters further comprise a programmable output current, a programmable signal frequency, and a programmable pulse width.

11. The method of claim 10 , wherein the pulse width is within a range of 100 to 250 μsec, the output current is within a range of 0.02 to 50 mA, and the signal frequency is at least 10 Hz.

The method of claim 10 , wherein operating the pulse generator further comprises operating the pulse generator to provide at least one of a ramp-up time during which the output current is progressively increased preceding therapeutic stimulation at a full output current or a ramp-down time during which the output current is progressively decreased following the therapeutic stimulation at the full output current.

13. The method of claim 8 , wherein the stimulation waveform comprises a form that causes propagation of action potentials in both an afferent direction and an efferent direction within neuronal fibers of a cervical vagus nerve.

14. The method of claim 8 , further comprising receiving modified stimulation parameters at the vagus nerve stimulator via wireless telemetry from a programming wand.

15. An implantable device for treating chronic cardiac dysfunction, comprising:

an implantable neurostimulator comprising a pulse generator configured to deliver electrical therapeutic stimulation in alternating cycles of stimuli application and stimuli inhibition;

a cervical vagus nerve stimulation therapy lead electrically coupled to the pulse generator and terminated by a pair of helical electrodes through which therapeutic stimulation is delivered to the patient's cervical vagus nerve; and

a recordable memory configured to store stimulation parameters configured to define a periodic duty cycle for the alternating cycles of stimuli application and stimuli inhibition, the stimulation parameters comprising a programmable ON-time for the stimuli application and a programmable OFF-time for the stimuli inhibition;

wherein the periodic duty cycle comprises a percentage calculated from a ratio of the ON-time to the OFF-time, the percentage between 5% and 30%; and

wherein the pulse generator is further configured to operate under the stimulation parameters stored in the recordable memory.

16. The implantable device of claim 15 , wherein the stimulation parameters further comprise a programmable output current, a programmable signal frequency, and a programmable pulse width.

17. The implantable device of claim 16 , wherein the pulse width is within a range of 100 to 250 μsec, the output current is within a range of 0.02 to 50 mA, and the signal frequency is at least 10 Hz.

18. The implantable device of claim 16 , wherein the pulse generator is further configured to provide at least one of a ramp-up time during which the output current is progressively increased preceding therapeutic stimulation at a full output current or a ramp-down time during which the output current is progressively decreased following the therapeutic stimulation at the full output current.

19. The implantable device of claim 15 , wherein the stimulation waveform comprises a form that causes propagation of action potentials in both an afferent direction and an efferent direction within neuronal fibers of the cervical vagus nerve.

20. The implantable device of claim 15 , wherein the recordable memory is further configured to receive modified stimulation parameters via wireless telemetry from a programming wand.
