Abstract

A device includes a handle, an expandable structure including a plurality of splines extending from a proximal hub to a distal hub, a first electrode on a first spline of the plurality of splines, an outer tube extending from the handle to the proximal hub, and a shaft extending through the outer tube from the handle to the distal hub. The expandable structure has a collapsed state and a self-expanded state. The handle is configured to retract the shaft. Retracting the shaft may expand the expandable structure outward of the self-expanded state.

Claims (63)

1. A method of positioning an electrode array for application of neuromodulation at a target area, the method comprising:

inserting a distal portion of a first catheter through an access point, the access point being an internal jugular vein, the distal portion of the first catheter comprising a balloon in an uninflated state during the inserting;

after the inserting, inflating the balloon;

after the inflating, floating the distal portion of first catheter to the target area, the balloon being carried by blood flow, the target area being a right pulmonary artery;

routing a guidewire through a lumen of the first catheter to the target area until the guidewire protrudes distal to a distal end of the first catheter;

withdrawing the first catheter;

routing a second catheter over the guidewire to the target area, the second catheter comprising:

a longitudinal axis;

an outer sheath;

an inner element; and

an electrode system coupled to the inner element, the electrode system comprising:

a self-expanding scaffold;

the electrode array comprising four circumferentially adjacent electrode structures, each of the electrode structures comprising two to five electrodes; and

tethers to aid in positioning the electrode system in and out of the outer sheath;

proximally retracting the outer sheath comprising:

allowing the scaffold to self-expand from a collapsed state towards an expanded state; and

pushing the electrode array against the target area;

locking the scaffold and the electrode array in an expended position; and

applying neurostimulation to the target area using the electrode array, the neurostimulation increasing cardiac contractility.

2. The method of claim 1 , wherein the first catheter comprises an 8 Fr Swan-Ganz catheter having a 1.5 cm 3 balloon.

3. The method of claim 1 , wherein a plane extends through a longitudinal axis of the electrode system, wherein the electrode array is on a first side of the plane, and wherein a second side of the plane opposite the first side is free of electrodes.

4. A method of positioning an electrode array for application of neuromodulation at a target area, the method comprising:

inserting a distal portion of a first catheter through an access point, the distal portion of the first catheter comprising a balloon in an uninflated state during the inserting;

after the inserting, inflating the balloon;

after the inflating, floating the distal portion of the first catheter to the target area, the balloon being carried by blood flow, the target area being a right pulmonary artery;

routing a guidewire through a lumen of the first catheter to the target area until the guidewire protrudes distal to a distal end of the first catheter;

withdrawing the first catheter;

routing a second catheter over the guidewire to the target area, the second catheter comprising:

a longitudinal axis;

an outer sheath;

an inner element; and

an electrode system coupled to the inner element, the electrode system comprising:

a self-expanding scaffold; and

the electrode array;

proximally retracting the outer sheath comprising:

allowing the scaffold to self-expand from a collapsed state towards an expanded state; and

pushing the electrode array against the target area; and

applying neurostimulation to the target area using the electrode array, the neurostimulation increasing cardiac contractility.

5. The method of claim 4 , wherein the electrode array comprises a plurality of circumferentially adjacent electrode structures.

6. The method of claim 5 , wherein each of the electrode structures comprises two to four electrodes.

7. The method of claim 4 , further comprising locking the scaffold and the electrode array in an expended position.

8. The method of claim 4 , wherein the access point is a jugular vein.

9. The method of claim 4 , wherein the access point is an internal jugular vein, wherein the electrode array comprises four circumferentially adjacent electrode structures, and wherein the first catheter comprises an 8 Fr Swan-Ganz catheter having a 1.5 cm 3 balloon.

10. The method of claim 4 , wherein a plane extends through a longitudinal axis of the electrode system, wherein the electrode array is on a first side of the plane, and wherein a second side of the plane opposite the first side is free of electrodes.

11. A method of positioning an electrode array for application of neuromodulation at a target area, the method comprising:

floating a distal portion of a first catheter to the target area, the distal portion comprising a balloon carried by blood flow, the target area being a right pulmonary artery;

advancing a guidewire through a lumen of the first catheter to the target area until the guidewire protrudes distal to a distal end of the first catheter;

withdrawing the first catheter;

advancing a second catheter over the guidewire to the target area, the second catheter comprising:

a longitudinal axis;

an electrode system comprising:

a scaffold; and

the electrode array;

expanding the scaffold from a collapsed state towards an expanded state; and

applying neurostimulation to the target area using the electrode array, the neurostimulation increasing cardiac contractility.

12. The method of claim 11 , wherein the electrode array comprises a plurality of circumferentially adjacent electrode structures.

13. The method of claim 12 , wherein each of the electrode structures comprises two to five electrodes.

14. The method of claim 12 , wherein each of the electrode structures comprises four electrodes.

15. The method of claim 11 , further comprising locking the scaffold and the electrode array in an expended position.

16. The method of claim 11 , wherein the electrode system comprises tethers to aid in positioning the electrode system in and out of the outer sheath.

17. The method of claim 11 , wherein the access point is a jugular vein.

18. The method of claim 11 , wherein the access point is an internal jugular vein, wherein the electrode array comprises four circumferentially adjacent electrode structures, wherein each of the splines comprises two to five electrodes, wherein expanding the scaffold comprises pushing the electrode array against the target area, and wherein the first catheter comprises an 8 Fr Swan-Ganz catheter having a 1.5 cm 3 balloon.

19. The method of claim 11 , wherein a plane extends through a longitudinal axis of the electrode system, wherein the electrode array is on a first side of the plane, and wherein a second side of the plane opposite the first side is free of electrodes.
