Abstract

Devices and methods for treating an abdominal incision or hernia are described. An implant for the restoration or prophylactic treatment of an abdominal wall comprises an elongate element and at least one sheet connected to the elongate element along a longitudinal axis of the elongate element. The elongate element is positioned along the line of incision, and the at least one sheet is secured to the abdominal muscles surrounding the incision.

Claims (26)

1 . A device for treating an incision, hernia, or abdominal wall defect, comprising:

an elongate element having a cross-section and a longitudinal axis perpendicular to the cross-section, the elongate element comprising porcine tendon formed into a cylindrical structure extending along the longitudinal axis;

a first sheet connected to the elongate element between a first end and a second end of the first sheet and extending along the longitudinal axis on a first side of the elongate element; and

a second sheet connected to the elongate element between a first end and a second end of the second sheet and extending along the longitudinal axis on a second side of the elongate element opposite to the first side, wherein the first sheet and the second sheet comprise acellular dermal tissue matrix.

2 . The device of claim 1 , wherein the acellular dermal tissue matrix has the ability to support cellular growth and tissue regeneration.

3 . The device of claim 1 , wherein the first sheet and the second sheet each comprises a porous biocompatible matrix and the elongate element has a strength that is at least twice a strength of the first sheet and the second sheet.

4 . The device of claim 1 , wherein the elongate element has a strength that is at least three times a strength of the first sheet and the second sheet.

5 . The device of claim 1 , wherein the first sheet and the second sheet each comprises a mesh.

6 . The device of claim 1 , wherein the first sheet and the second sheet each comprises a porcine acellular dermal matrix.

7 . The device of claim 1 , wherein the elongate element is rolled concentrically into the cylindrical structure.

8 . The device of claim 1 , wherein the elongate element comprises a mesh.

9 . A method of closing a midline incision, comprising:

providing an implant including (i) an elongate element having a cross-section and a longitudinal axis perpendicular to the cross-section, the elongate element comprising porcine tendon formed into a cylindrical structure extending along the longitudinal axis, (ii) a first sheet connected to the elongate element between a first end and a second end of the first sheet and extending along the longitudinal axis on a first side of the elongate element, and (iii) a second sheet connected to the elongate element between a first end and a second end of the second sheet and extending along the longitudinal axis on a second side of the elongate element opposite to the first side, wherein the first sheet and the second sheet comprise acellular dermal tissue matrix;

positioning the elongate element along the midline incision; and

positioning at least one of the first sheet and the second sheet on at least one of an anterior side and a posterior side of a rectus abdominis muscle surrounding the midline incision.

10 . The method of claim 9 , further comprising creating one or more spaces on the anterior and posterior sides of the rectus abdominis muscle for placement of one least one of the first sheet and second sheet.

11 . The method of claim 9 , further comprising suturing at least one of the first sheet and second sheet to an anterior layer of fascia.

12 . The method of claim 9 , wherein the at least one of the first sheet and second sheet is inserted between an anterior layer of fascia and the rectus abdominis muscle.

13 . The method of claim 9 , wherein the at least one of the first sheet and second sheet is inserted between a posterior layer of fascia and the rectus abdominis muscle.

14 . The method of claim 9 , wherein the at least one of the first sheet and second sheet is inserted over an anterior layer of fascia.

15 . The method of claim 9 , wherein the at least one of the first sheet and second sheet is inserted below a posterior layer of fascia.

16 . The method of claim 9 , wherein the first and second sheets each comprises a porous biocompatible matrix and the elongate element has a strength that is at least twice a strength of the first sheet and the second sheet.

17 . The method of claim 9 , wherein the elongate element has a strength that is at least three times a strength of the first sheet and the second sheet.

18 . The method of claim 9 , wherein the first sheet and the second sheet each comprises a mesh.

19 . The method of claim 9 , wherein the elongate element is rolled concentrically into the cylindrical structure.

20 . The method of claim 9 , wherein the elongate element comprises a mesh.
