Abstract

Apparatuses and methods for treating a heart failure patient by ablating a nerve of the thoracic splanchnic sympathetic nervous system to increase venous capacitance and reduce pulmonary blood pressure. A method comprising: inserting a catheter into a vein adjacent the nerve, applying stimulation energy and observing hemodynamic effects, applying ablation energy and observing hemodynamic effects, applying stimulation energy after the ablation and observing hemodynamic effects and monitoring for presence of the lung in the ablation zone. An alternative method comprising: inserting a catheter into a vein adjacent the nerve, detecting that lung tissue is a safe distance from an ablation zone, and delivering ablation energy to the target nerve when lung tissue is a safe distance from the ablation zone.

Claims (65)

1. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein, wherein decreasing the size of the lumen comprises at least one of mechanically irritating at least a portion of the intercostal vein, applying a vacuum to at least a portion of intercostal vein, and delivering a vasoconstrictor to at least a portion of the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve; and

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension.

2. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein, wherein decreasing the size of the lumen comprises contracting smooth muscle of the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve; and

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension.

3. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein, wherein decreasing the size of the lumen comprises denaturing collagen fibers in the intercostal vein wall;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve; and

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension.

4. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve; and

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension, further comprising, before commencing delivering ablation energy, diverting blood flow away from or at least partially occluding a portion of an adjacent azygos system vein.

5. A method of claim 4 diverting blood flow away from or at least partially occluding a portion of an adjacent azygos system vein comprises occluding an ostium where the intercostal vein meets the azygos system vein.

6. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve; and

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension, wherein the location is upstream to a location where the ablation element is positioned in the intercostal vein when delivering ablation energy from the ablation element towards the thoracic splanchnic nerve.

7. The method of claim 6 , wherein decreasing the size of the lumen comprises delivering energy from the ablation element.

8. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve; and

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension,

wherein delivering ablation energy comprises delivering ablative radiofrequency electrical current, and wherein decreasing the size of the lumen comprises delivering radiofrequency electrical current to the intercostal vein, and wherein the ablative radiofrequency electrical current is delivered at a lower power and for a longer duration than the radiofrequency electrical current delivered when decreasing the size of the lumen.

9. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve;

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension; and

determining proximity to the thoracic splanchnic nerve by delivering a nerve stimulation signal towards the thoracic splanchnic nerve and measuring a physiological response to the stimulation signal.

10. The method of claim 9 , wherein decreasing the size of the lumen comprises fully occluding the lumen at the location.

11. The method of claim 9 , wherein decreasing the size of the lumen comprises delivering energy to the intercostal vein.

12. The method of claim 11 , wherein delivering energy to the intercostal vein comprises at least one of heating at least a portion of the intercostal vein, applying an electric signal to at least a portion of the intercostal vein.

13. A method of claim 9 configured to electrically stimulate a target thoracic splanchnic nerve without electrically stimulating pain fibers.

A method of claim 13 wherein an energy delivery electrode is positioned in the intercostal vein that is within 0 to 5 mm from the target thoracic splanchnic nerve, a stimulation signal is delivered from the energy delivery electrode, wherein the stimulation signal comprises a voltage in a range of 0.05 V to 5 V and a pulse duration in a range of 0.12 ms to 0.9 ms.

15. A method of claim 13 wherein the stimulation element has a caliber of 3 French to 8 French and a length of 0.5 mm to 4 mm and the waveform is configured to generate an electric field intensity in a range of 30 to 500 V/m with a pulse duration of 0.12 ms.

16. The method of claim 9 , wherein the location at least partially overlaps with a location where the ablation element is positioned in the intercostal vein when delivering ablation energy from the ablation element towards the thoracic splanchnic nerve.

17. The method of claim 9 further comprising decreasing the size of the lumen at a plurality of locations.

18. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve;

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension; and

after delivering ablation energy, moving the ablation element within the intercostal vein to a different location, and again delivering ablation energy from the ablation element towards the thoracic splanchnic nerve.

19. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve;

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension; and

positioning a second elongate shaft into an azygos vein, and wherein advancing the elongate shaft carrying an ablation element into an intercostal vein comprises extending the elongate shaft away from the second elongate shaft and into the intercostal vein.

20. A method of transvascular ablation of a thoracic splanchnic nerve to treat at least heart failure or symptoms of heart failure, comprising:

advancing an elongate shaft carrying an ablation element into an intercostal vein proximate to a thoracic splanchnic nerve, the intercostal vein having a lumen;

decreasing the size of the lumen at a location along the intercostal vein;

after the decreasing step, delivering ablation energy from the ablation element towards the thoracic splanchnic nerve;

ablating at least a portion of the thoracic splanchnic nerve with the ablation energy, to treat at least hypertension; and

diverting blood away from an ostium where the intercostal vein meets an azygos system vein.
