Tensioning apparatus for hemostasis and maintaining catheter placement
Disclosed herein are improved methods and apparatuses for providing hemostasis within a cavity defined by an internal surface of a bleeding tissue space. A catheter comprising a proximal end and a distal end may be advanced into the cavity through a proximal opening of the tissue space into the cavity. A distal balloon coupled to the catheter may be expanded, and the catheter tensioned to apply pressure to the internal surface of the tissue space to inhibit bleeding of the tissue space.
1. An apparatus for tensioning a catheter inserted into a patient, the apparatus comprising:
a base to couple to a pelvis of the patient, the base having a perimeter and an opening through the perimeter configured to permit the catheter inserted into the patient to pass radially through the opening from a first location outside the perimeter of the base to a second location within the perimeter of the base;
a plurality of extension struts extending from the base;
a retainer element carried by the plurality of extension struts to releasably couple to the catheter; and
a tensioning mechanism coupled to the base and the retainer element, the tensioning mechanism to adjust tension to the catheter inserted into the patient.
2. The apparatus of claim 1 , wherein the catheter comprises an expandable support and wherein the tensioning mechanism is configured to adjust an amount of tension to the catheter to decrease bleeding within an enclosed tissue space of the patient when the expandable support has expanded.
3. The apparatus of claim 2 , wherein the tensioning mechanism is configured to tension the catheter to apply an amount of pressure to tissue with the expandable support.
4. The apparatus of claim 3 , wherein the amount of pressure is greater than a blood pressure of the patient.
5. The apparatus of claim 3 , wherein the expandable support comprises a balloon.
6. The apparatus of claim 1 , wherein the tensioning mechanism comprises a spring.
7. The apparatus of claim 1 , wherein the tensioning mechanism comprises a constant force spring configured to apply a substantially constant tension to the catheter over a stroke length of the catheter.
8. The apparatus of claim 7 , wherein the stroke length of the catheter is in a range of about 0.5 cm to about 8 cm.
9. The apparatus of claim 1 , wherein the retainer element comprises a clamp for clamping one or more of the catheter or a medical tape flag coupled to the catheter.
10. The apparatus of claim 1 , wherein a position of the retainer element relative to the base is adjustable to adjust an amount of tension to the catheter.
11. The apparatus of claim 1 , wherein the base comprises an enclosure shaped to receive a penis of the patient.
12. The apparatus of claim 1 , wherein the base defines an accommodation space for receiving a penis of the patient.
13. The apparatus of claim 1 , wherein the base is configured to be placed over the pelvis or a groin.
14. The apparatus of claim 1 , wherein the base comprises a pelvic or groin mount contoured to match a shape of the pelvis or a groin.
15. The apparatus of claim 1 , wherein the opening is configured to allow the catheter to pass from the exterior of the base to within the perimeter of the base and within an accommodation space.
16. The apparatus as in claim 1 , wherein the base is U-shaped.
17. The apparatus as in claim 1 , wherein the base comprises a malleable wire with a soft covering.
18. The apparatus of claim 1 , wherein the base and the plurality of extension struts defines an accommodation space for one or more of a penis or scrotum to pass therethrough.
19. The apparatus of claim 18 , wherein the plurality of extension struts is coupled to the base with a hinge.
20. The apparatus of claim 19 , wherein the hinge is adjustable.
21. The apparatus of claim 1 , wherein the plurality of extension struts is coupled to the base with a rigid coupling.
22. The apparatus of claim 1 , wherein the tensioning mechanism is coupled to at least one of the plurality of extension struts.
23. The apparatus of claim 1 , further comprising a tension measurement element or scale coupled to at least one of the plurality of extension struts.
24. The apparatus of claim 1 , further comprising a cross-member coupling the plurality of extension struts to one another.
25. The apparatus of claim 24 , wherein the retainer element is positioned on the cross-member.
26. The apparatus of claim 1 , wherein the apparatus is collapsible into a flat package.
27. The apparatus of claim 1 , further comprising a belt or strap coupled to the base, the belt or strap configured to be wrapped around and strapped to at least a portion of the patient to maintain a position of the base over the pelvis or a groin of the patient.
28. The apparatus of claim 1 , wherein the tensioning mechanism comprises an adjustable tensioning mechanism.
29. The apparatus of claim 1 , wherein a location the catheter enters into the patient is spaced apart from the base coupled to the patient.
30. The apparatus of claim 1 , wherein the plurality of extension struts is pivotally coupled to the base and further comprising a locking mechanism configured to selectively maintain an angle between the plurality of extension struts and the base.
