Abstract

Apparatuses, systems and methods are provided for treating pulmonary tissues via delivery of energy, generally characterized by high voltage pulses, to target tissue using a pulmonary tissue modification system (e.g., an energy delivery catheter system). Example pulmonary tissues include, without limitation, the epithelium (the goblet cells, ciliated pseudostratified columnar epithelial cells, and basal cells), lamina propria, submucosa, submucosal glands, basement membrane, smooth muscle, cartilage, nerves, pathogens resident near or within the tissue, or a combination of any of these. The system may be used to treat a variety of pulmonary diseases or disorders such as or associated with COPD (e.g., chronic bronchitis, emphysema), asthma, interstitial pulmonary fibrosis, cystic fibrosis, bronchiectasis, primary ciliary dyskinesia (PCD), acute bronchitis and/or other pulmonary diseases or disorders.

Claims (43)

1. A method for treating mucus hypersecretion by a portion of a wall of a lung passageway of a patient, the method comprising:

positioning an electrode within a lumen of the lung passageway so that the electrode is able to transmit energy to the portion of the wall of the lung passageway; and

providing an electrical signal to energize the electrode so as to transmit the energy to the portion of the wall of the lung passageway for a duration of treatment to treat mucus hypersecretion by the portion of the wall of the lung passageway, wherein the electrical signal includes distinct energy packets and each energy packet comprises a series of biphasic pulses.

2. The method of claim 1 , wherein the energy selectively treats particular cells associated with mucus hypersecretion of the lung passageway.

3. The method of claim 1 , wherein the energy is below a threshold for thermal ablation of the wall of the lung passageway throughout the duration of treatment.

4. The method of claim 1 , wherein providing the electrical signal to energize the electrode transmits the energy to a select target layer of the portion of the wall.

5. The method of claim 1 , wherein each energy packet has a cycle count of up to 2,000 biphasic pulses.

6. The method of claim 5 , wherein each energy packet has a cycle count of up to 60 biphasic pulses.

7. The method of claim 1 , wherein the electrical signal includes five distinct energy packets and each of the five distinct energy packets has a duration of 100 microseconds.

8. The method of claim 1 , wherein the electrical signal includes rest periods of 0.0014 seconds to about 10 seconds between each of the distinct energy packets.

9. The method of claim 1 , wherein the treatment kills particular pathogens within the lung passageway.

10. The method of claim 1 , wherein the electrode includes a plurality of wires forming a basket, and wherein positioning the electrode within the lumen of the lung passageway comprises expanding the basket within the lumen of the lung passageway.

11. The method of claim 1 , further comprising:

positioning a return electrode on the patient, and

wherein the energy is transmitted in a monopolar fashion toward the return electrode.

12. A method for selectively treating particular cells associated with mucus hypersecretion of a portion of a wall of a lung passageway of a patient, the method comprising:

positioning an electrode within a lumen of the lung passageway so that the electrode is able to transmit energy to the portion of the wall of the lung passageway; and

providing an electrical signal to energize the electrode so as to transmit non-thermal energy to the portion of the wall of the lung passageway for a duration of treatment to selectively treat the particular cells associated with mucus hypersecretion of the lung passageway,

wherein the non-thermal energy is below a threshold for thermal ablation of the wall of the lung passageway throughout the duration of treatment.

13. The method of claim 12 , wherein the electrical signal includes distinct energy packets and each energy packet comprises a series of biphasic pulses.

14. The method of claim 13 , wherein the electrical signal includes up to 20 distinct energy packets.

15. The method of claim 14 , wherein the electrical signal includes five distinct energy packets and each of the five distinct energy packets has a duration of 100 microseconds.

16. The method of claim 13 , wherein the electrical signal includes rest periods of 0.0014 seconds to about 10 seconds between each of the distinct energy packets.

17. The method of claim 12 , wherein selectively treats comprises selectively removes and/or kills the particular cells.

18. The method of claim 12 , wherein selectively treats comprises selectively modifies the particular cells.

19. The method of claim 12 , wherein the particular cells include endothelial cells.

20. The method of claim 12 , wherein the particular cells include submucosal glands.

21. A method for treating a portion of a wall of a lung passageway of a patient, the method comprising:

positioning an electrode within a lumen of the lung passageway so that the electrode is able to transmit energy to the portion of the wall of the lung passageway; and

providing an electrical signal to energize the electrode so as to transmit the energy up to and at least partially including a target layer within a mucosa of the portion of the wall of the lung passageway for a duration of treatment,

wherein the energy is below a threshold for thermal ablation of the wall of the lung passageway throughout the duration of treatment.

22. The method of claim 21 , wherein the target layer is a basement membrane.

23. The method of claim 21 , wherein the target layer is an epithelial layer.

24. The method of claim 21 , wherein the electrical signal is discontinuous and includes distinct energy packets and each energy packet includes a series of biphasic pulses.

25. The method of claim 21 , further comprising:

selectively treating particular cells associated with mucus hypersecretion.

26. The method of claim 25 , wherein the particular cells include epithelial cells.

27. A method of treating a portion of a wall of a lung passageway of a patient with a catheter having an electrode disposed near its distal end, the method comprising:

advancing the distal end of the catheter into a lumen of the lung passageway and positioning the electrode within the lumen so that the electrode is able to transmit energy to the portion of the wall of the lung passageway; and

providing a discontinuous electrical signal to energize the electrode so as to transmit the energy to the portion of the wall of the lung passageway for a duration of treatment, the energy transmitted to the portion of the wall of the lung passageway over the duration of treatment operable to modify cells within the portion of the wall of the lung passageway.

28. The method of claim 27 , wherein the discontinuous electrical signal includes five distinct energy packets and each of the five distinct energy packets has a duration of 100 microseconds.

29. The method of claim 27 , wherein the discontinuous electrical signal includes distinct energy packets and each of the distinct energy packets includes a series of biphasic pulses.

30. The method of claim 27 , wherein the energy transmitted is insufficient to cause muscle contraction throughout the duration of treatment.
