Abstract

Embodiments of the invention provide swallowable devices, preparations and methods for delivering drugs and other therapeutic agents within the GI tract. Many embodiments provide a swallowable device for delivering the agents. Particular embodiments provide a swallowable device such as a capsule for delivering drugs into the intestinal wall or other GI lumen. Embodiments also provide various drug preparations that are configured to be contained within the capsule, advanced from the capsule into the intestinal wall and degrade to release the drug into the bloodstream to produce a therapeutic effect. The preparation can be operably coupled to delivery means having a first configuration where the preparation is contained in the capsule and a second configuration where the preparation is advanced out of the capsule into the intestinal wall. Embodiments of the invention are particularly useful for the delivery of drugs which are poorly absorbed, tolerated and/or degraded within the GI tract.

Claims (29)

1. A method for delivering etanercept to a patient in need thereof, the method comprising:

orally delivering a dosage of etanercept contained within a tissue penetrating member into a lumen of the GI-tract of the patient;

protecting the dosage of etanercept from degradation due to action of digestive fluids within the GI tract; and

at a location within the GI tract lumen, advancing the tissue penetrating member into a gastro-intestinal wall to penetrate the tissue penetrating member into the gastro-intestinal wall, wherein the etanercept is released into the patient's blood stream at the location.

2. The method of claim 1 , wherein the tissue penetrating member is advanced into the gastro-intestinal wall by the application of a mechanical force on a surface of the tissue penetrating member.

3. The method of claim 1 , wherein the tissue penetrating member is advanced into the gastro-intestinal wall responsive to a condition in the gastro-intestinal tract.

4. The method of claim 3 , wherein the condition is a pH.

5. The method of claim 1 , wherein the gastro-intestinal wall is the stomach wall.

6. The method of claim 1 , wherein the gastro-intestinal wall is the wall of the small intestine.

7. The method of claim 1 , wherein the dosage of etanercept comprises at least one pharmaceutical excipient.

8. The method of claim 7 , wherein the at least one pharmaceutical excipient comprises at least one of a binder, a preservative or a disintegrant.

9. A method as in claim 8 , wherein the binder comprises polyethylene glycol (PEG).

10. The method of claim 1 , wherein the tissue penetrating member comprises a biodegradable material which degrades within the gastro-intestinal wall to release the etanercept into the blood stream.

11. The method of claim 10 , wherein the gastro-intestinal wall is the wall of the small intestine and the tissue penetrating member degrades within the wall of the small intestine to release the etanercept into the blood stream.

12. The method of claim 1 , wherein the dosage of etanercept in the tissue penetrating member is in a range from about 1 to 5 mg.

13. The method of claim 12 , wherein the dosage of etanercept is in a range from about 2 to about 4 mg.

14. A method as in claim 1 , wherein the dosage of etanercept comprises a solid dosage, the method further comprising retaining the solid dosage of etanercept within the intestinal wall after insertion.

15. The method of claim 14 , wherein the solid dosage of etanercept produces a long-term release of etanercept.

16. The method of claim 15 , wherein the long term release is over a period of about 6 to 24 hours.

17. The method of claim 1 , further comprising: using the dosage of delivered etanercept to treat an autoimmune disease in the patient.

18. The method of claim 17 , where the autoimmune disease is rheumatoid arthritis.

19. The method of claim 17 , where the autoimmune disease is psoriatic arthritis.

20. The method of claim 17 , where the autoimmune disease is ankylosing spondylitis.

21. The method of claim 17 , where the autoimmune disease is juvenile idiopathic arthritis.

The method of claim 1 , wherein tissue penetrating member is contained in a swallowable enclosure and operably coupled to an actuator having a first configuration and a second configuration, the solid dosage being contained within the enclosure in the first configuration and advanced out of the enclosure and into the gastro-intestinal wall in the second configuration.

23. The method of claim 22 , wherein the swallowable enclosure comprises a swallowable capsule.

24. The method of claim 1 , wherein protecting the dosage of etanercept from degradation comprises protecting the etanercept from exposure to the digestive fluids within the GI tract while the dosage of etanercept passes through at least a portion of the GI tract to the location.

25. The method of claim 24 , protecting the etanercept from exposure comprises encapsulating the tissue penetrating member.

26. The method of claim 24 , wherein the etanercept is released into the patient's blood stream at a high percentage bioavailability.
