Abstract

Prostate treatment using fluid stream to resect prostate tissue, thereby relieving symptoms of conditions such as BPH, prostatitis, and prostatic carcinoma. A device having a fluid delivery element is positioned within a lumen of the urethra within the prostate. A fluid stream is directed outwardly from the fluid delivery element toward a wall of the urethral lumen. The fluid delivery element is moved to scan the fluid stream over the wall to remove a volume of tissue surrounding the lumen. The fluid may be combined with therapeutically active substances or with substances that increase resection efficiency. Fluid force may be adjusted to provide selective tissue resection such that soft tissue is removed while harder tissue is left undamaged. In order to gain a working space within the urethra, another fluid may be introduced to insufflate the urethra in the region of treatment.

Claims (24)

1. A method for tissue resection modification, said method comprising:

positioning a catheter assembly having a shaft which carries an energy source within a tissue;

directing the energy source radially outwardly from an energy source toward a wall of the tissue; and

moving the energy source within the catheter assembly relative to the tissue, wherein energy from the energy source modifies a volume of the tissue, wherein the catheter assembly remains positioned in the tissue while the energy source is moved relative to the tissue.

2. The method of claim 1 , further comprising expanding an anchor within a bladder.

3. The method of claim 2 , wherein expanding an anchor comprises expanding a balloon.

4. The method of claim 3 , wherein expanding the balloon comprises inflating the balloon to occupy substantially an entire volume of the tissue, and wherein the tissue includes a urethra.

5. The method of claim 2 , wherein the anchor and the energy source are mounted on the catheter assembly, wherein the catheter assembly remains stationary while the energy source is one or more of axially translated or rotated relative to the catheter assembly.

6. The method of claim 1 , wherein the volume of tissue is resected by delivering the energy to prostate tissue.

7. The method of claim 1 , wherein directing the energy comprises delivery a conductive fluid which carries a radiofrequency current to ablate tissue.

8. The method of claim 1 , wherein directing the energy comprises generating a plasma of a conductive fluid.

9. The method of claim 1 , wherein directing the energy comprises delivering laser energy to ablate tissue.

10. The method of claim 1 , wherein directing the enery comprises engaging an electrode against the tissue and delivering radiofrequency energy to ablate the tissue.

11. The method of claim 1 , further comprising delivering energy from the energy source outwardly at an angle in the range of 10 to 90 degrees with respect to a longitudinal axis of the shaft and towards the tissue, wherein the tissue includes a prostate.

12. The method of claim 1 , further comprising delivering an insufflating fluid.

13. The method of claim 1 , further comprising aspirating resection products.

14. The method of claim 13 , further comprising flushing a lumen of a urethra to help remove the resection products.

15. The method of claim 14 , wherein aspirating and flushing are performed with a device which also carries the energy source.

16. The method of claim 1 , wherein moving the energy source within the catheter assembly is performed at a treatment site, and the energy results in limited bleeding at the treatment site.

17. The method of claim 1 , wherein moving the energy source comprises radially advancing the energy source toward the tissue as the tissue is removed.

18. The method of claim 1 , wherein directing the energy comprises directing one or more of mechanical, vibrational, thermal, optical, or electrical energy.

19. The method of claim 1 , wherein the catheter assembly is a rigid tube and positioning the catheter assembly comprises positioning the rigid tube.

20. The method of claim 1 , further comprising moving the energy source relative to the shaft to treat a predefined surface region of a prostate.

21. The method of claim 20 , wherein the predefined surface region of the prostate defines a generally cylindrical shape.
