Abstract

Devices, systems, and methods treat cosmetic defects, and often apply cooling with at least one tissue-penetrating probe inserted through of the skin of a patient. The cooling may remodel one or more target tissue so as to effect a desired change in a composition of the target tissue and/or a change in its behavior. Exemplary embodiments of the cooling treatments will interfere with the nerve/muscle contractile function chain so as to mitigate wrinkles of the skin. Related treatments may be used therapeutically for treatment of back and other muscle spasms, chronic pain, and the like. Some embodiments may remodel subcutaneous adipose tissue so as to alter a shape or appearance of the skin surface.

Claims (27)

1. A method for cryogenically treating target tissue for alleviating pain, said method comprising:

providing a cryogenic device having a tissue penetrating needle probe, wherein the tissue penetrating needle probe has a plurality of spaced apart tissue penetrating needles, each of the plurality of spaced apart tissue penetrating needles having a distal portion, a proximal portion, a length less than 5 cm and a diameter in a range from about 0.006 inches to about 0.1 inches, and wherein adjacent needles of the plurality of spaced apart tissue penetrating needles are spaced apart between 0.25 mm and 2 mm;

advancing the distal portion of each of the plurality of spaced apart tissue penetrating needles through a skin surface into the target tissue;

cooling the target tissue below the skin surface with the distal portions of the plurality of spaced apart tissue penetrating needles;

applying a warming energy to collateral tissue near the target tissue, wherein the warming energy is applied along the skin surface with a heater on a surface of the cryogenic device proximal to the distal portions of the plurality of spaced apart tissue penetrating needles to prevent collateral tissue damage by avoiding visible necrosis of dermal tissues of the skin surface; and

remodeling the target tissue such that the remodeled target tissue temporarily alleviates the pain.

2. The method of claim 1 , wherein the target tissue comprises a nerve, muscle, or neuromuscular junction.

3. The method of claim 1 , wherein each of the plurality of spaced apart tissue penetrating needles has a diameter of 0.012 inches.

4. The method of claim 1 , wherein each of the distal portions of the plurality of spaced apart tissue penetrating needle has a sharpened distal end.

5. The method of claim 4 , wherein the advancing comprises piercing the sharpened distal ends of the plurality of spaced apart tissue penetrating needles through the skin surface.

The method of claim 1 , wherein the cooling comprises introducing a cooling fluid into a needle probe lumen of each of the plurality of spaced apart tissue penetrating needles so that the cooling fluid is configured to vaporize within the needle probe lumen to provide the cooling to the target tissue below the skin surface.

7. The method of claim 6 , wherein the cooling fluid comprises nitrous oxide or carbon dioxide.

The method of claim 1 , wherein the cooling includes delivering a cooling fluid from a cooling fluid source disposed within a handpiece of the cryogenic device to each of the spaced apart tissue penetrating needles via a fluidic pathway, the handpiece having a size and shape suitable for supporting a hand of an operator, and the fluidic pathway comprising the cooling fluid source, an inner lumen, and an outer lumen of each of the spaced apart tissue penetrating needles.

9. A method for cryogenically treating target tissue for alleviating pain, said method comprising:

providing a cryogenic device having a tissue penetrating needle probe, wherein the tissue penetrating needle probe has a plurality of spaced apart tissue penetrating needles, each of the plurality of spaced apart tissue penetrating needles having a distal portion, a proximal portion, a length less than 5 cm and a diameter in a range from about 0.006 inches to about 0.1 inches, and wherein adjacent needles of the plurality of spaced apart tissue penetrating needles are spaced apart between 0.25 mm and 2 mm, wherein each of the plurality of spaced apart tissue penetrating needles comprises:

an outer lumen extending distally toward a distal end of the needle, and

an inner lumen extending distally within the outer lumen, wherein the inner lumen comprises a distal opening that is open to the outer lumen;

advancing the distal portion of each of the plurality of spaced apart tissue penetrating needles through a skin surface into the target tissue;

cooling the target tissue below the skin surface with the distal portions of the plurality of spaced apart tissue penetrating needles, wherein the cooling includes delivering a cooling fluid from a cooling fluid source disposed within a handpiece of the cryogenic device to each of the spaced apart tissue penetrating needles via a fluidic pathway, the handpiece having a size and shape suitable for supporting a hand of an operator, and the fluidic pathway comprising the cooling fluid source, the inner lumen, and the outer lumen of each of the spaced apart tissue penetrating needles;

applying a warming energy to collateral tissue near the target tissue, wherein the warming energy is applied along the skin surface with a heater on a surface of the cryogenic device proximal to the distal ends of the plurality of spaced apart tissue penetrating needles to prevent collateral tissue damage by avoiding visible necrosis of dermal tissues of the skin surface; and

remodeling the target tissue such that the remodeled target tissue temporarily alleviates the pain.

10. The method of claim 9 , wherein the target tissue comprises a nerve, muscle, or neuromuscular junction.

11. The method of claim 9 , wherein each of the plurality of spaced apart tissue penetrating needles has a diameter of 0.012 inches.

12. The method of claim 9 , wherein each of the distal portions of the plurality of spaced apart tissue penetrating needles has a sharpened distal end.

13. The method of claim 12 , wherein the advancing comprises piercing the sharpened distal ends of the plurality of spaced apart tissue penetrating needles through the skin surface.

14. The method of claim 9 , wherein the cooling comprises introducing a cooling fluid into the inner lumen of each of the plurality of spaced apart tissue penetrating needles so that the cooling fluid is configured to vaporize within the needle probe lumen to provide the cooling to the target tissue below the skin surface.

15. The method of claim 14 , wherein the cooling fluid comprises nitrous oxide or carbon dioxide.
