Abstract

IUI insemination (introduction of washed sperm into the uterus) should occur during a period of time when sperm Fc receptor (FcR) expression is going through a minimum, i.e., in a window of time about the minimum. In order to be ready to inseminate at the time when the FcR expression goes through a minimum, sperm are prepared for insemination according to standard practice. Time of the IUI procedure itself is adjusted to occur at the preferred sperm state, again within the clinic's permitted timeframe for the physician to treat the patient by instilling the washed sperm into the uterus. The level of FcR expression is determined by time-based assay and insemination is performed based on the fertility state of sperm as determined by the assay.

Claims (42)

1 . A method for determining an optimum time for utilizing a semen sample for insemination or storage comprising the steps of:

i. providing a semen sample from a male subject to be tested and incubating the sample under controlled conditions;

ii. selecting a marker that is indicative of fertility status of sperm, wherein expression of the marker correlates with fertility status of the sperm, and wherein the marker is Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both;

iii. detecting the level of expression of the FcR or a marker whose expression correlates with FcR expression or both by sperm in the semen sample at a plurality of time points;

iv. determining the time point at which the sperm in the semen sample express the FcR or a marker whose expression correlates with FcR expression or both at about a maximum or a predetermined level that correlates to approaching a maximum of FcR expression of the sperm; and

v. selecting a time point where the sperm display a minimum level of expression of the Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both, and thereby providing an optimum time for administering or the semen sample for insemination or freezing to provide improved pregnancy results.

2 . The method of claim 1 , wherein the controlled condition includes incubating the semen under a constant temperature, pH or both.

3 . The method of claim 1 , where the controlled condition includes incubating the semen at a temperature of from about 0° C. to about 37° C.

4 . The method of claim 1 , where the controlled condition includes incubating the semen at a temperature of about 12° C. to about 37° C.

6 . The method of claim 1 , wherein at step (iii) the plurality of time points are at least 15 minutes apart.

7 . The method of claim 1 , wherein at step (iii) the plurality of time points are at least 20 minutes apart.

8 . The method of claim 1 , wherein at step (iii) the plurality of time points are at least 30 minutes apart.

9 . The method of claim 1 , wherein the time point of step (v) is from about 20 minutes before to about 20 minutes after the minimum level of expression of the Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both.

10 . A method for obtaining improved pregnancy results the method comprising the steps of:

i. providing a semen sample from a male subject to be tested and incubating the sample under a controlled condition;

ii. selecting a marker that is indicative of fertility status of sperm, wherein expression of the marker changes with time, and wherein the marker is Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both;

iii. detecting the level of expression by sperm in the semen sample of the FcR or a marker whose expression correlates with FcR expression or both at a plurality of time points;

iv. determining the time point at which the sperm in the semen sample express the FcR or a marker whose expression correlates with FcR expression or both at about a maximum or a predetermined level that correlates to approaching a maximum of FcR expression of the sperm;

v. selecting a time point which occurs before the sperm display a minimum level of expression of the Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both, and thereby providing an optimum time for administering the semen sample for insemination or freezing; and

vi. inseminating the processed sperm into a female by IUI or freezing, thereby obtaining improved pregnancy results.

11 . The method of claim 10 , where the controlled condition includes incubating the semen at a temperature of from about 20° C. to about 37° C.

12 . The method of claim 10 , where the controlled condition includes incubating the semen at a temperature of about 37° C.

14 . The method of claim 10 , wherein at step (iii) the plurality of time points are at least 15 minutes apart.

15 . The method of claim 10 , wherein at step (iii) the plurality of time points are at least 20 minutes apart.

16 . The method of claim 10 , wherein at step (iii) the plurality of time points are at least 30 minutes apart.

17 . The method of claim 10 , wherein the time point of step (v) is from about 20 minutes before to about 20 minutes after the minimum level of expression of the Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both.

18 . A method for performing insemination in a female patient, comprising the steps of:

i. collecting a semen sample from a male subject to be tested and incubating the sample under a controlled condition;

ii. removing a small portion of the collected semen for assaying aliquots of sperm of the ejaculate;

iii. detecting the expression level of an Fc receptor (FcR) marker at a plurality of time points, wherein the expression of the FcR is indicative of a fertility state of said sperm;

iv. determining the time point at which the sperm in the semen sample express the FcR marker at about a maximum, and initiating processing of the sperm in the semen sample;

v. selecting a time point which occurs before the sperm display a minimum level of expression of the Fc receptor (FcR), and thereby providing an optimum time for administering the semen sample for insemination; and

vi. inseminating the processed sperm into a female by IUI or freezing, thereby obtaining improved pregnancy results.

19 . The method of claim 18 , where the controlled condition includes incubating the semen at a temperature of about 37° C.

21 . The method of claim 18 , wherein at step (iii) the plurality of time points are at least 15 minutes apart.

22 . The method of claim 1 , wherein at step (iii) the plurality of time points are at least 20 minutes apart.

23 . The method of claim 1 , wherein at step (iii) the plurality of time points are at least 30 minutes apart.

24 . The method of claim 1 , wherein the time point of step (v) is from about 20 minutes before to about 20 minutes after the minimum level of expression of the Fc receptor (FcR) or a marker whose expression correlates with FcR expression or both.
