Abstract

An arrhythmia monitoring device includes a plurality of pairs of ECG sensing electrodes disposed in a garment configured to be worn about a torso of a patient, and signal acquisition circuit configured to be electrically coupled to the plurality pairs of ECG sensing electrodes. The device includes a processor in communication with the signal acquisition circuit configured to execute a selection process comprising identifying a quality of each of a plurality of ECG signals, ranking the signals based on the identified quality, and selecting at least two ECG signals to monitor having highest quality. The processor is configured to re-execute the selection process following an event including at least one of the garment shifting on the torso, the garment being repositioned on the torso, and the garment being removed and returned to the torso of the patient, and monitor the selected at least two ECG signals for a cardiac arrhythmia.

Claims (44)

1. An arrhythmia monitoring device comprising:

a plurality of ECG sensing electrodes configured to be disposed in the garment, the plurality of ECG sensing electrodes comprising a plurality of pairs of ECG sensing electrodes configured to provide a plurality of ECG signals, each of the plurality of ECG signals corresponding to one of the plurality of pairs of ECG sensing electrodes;

a signal acquisition circuit configured to be disposed in the garment, the signal acquisition circuit being configured to be electrically coupled to the plurality of ECG sensing electrodes and configured to sense the plurality of ECG signals;

a processor in communication with the signal acquisition circuit, the processor being configured to

execute a selection process comprising

identifying a quality of each of the plurality of ECG signals,

ranking the plurality of ECG signals relative to a remainder of the plurality of ECG signals based on the identified quality of each of the plurality of ECG signals, and

selecting at least two ECG signals to monitor, the at least two ECG signals selected having highest quality among the plurality of ECG signals based on the ranking;

re-execute the selection process of at least two ECG signals following an event, the event comprising at least one of the garment shifting on the torso, the garment being repositioned on the torso, or the garment being removed and returned to the torso of the patient; and

monitor the selected at least two ECG signals from the re-executed selection process for a cardiac arrhythmia.

2. The arrhythmia monitoring device of claim 1 , wherein the quality of each of the plurality of ECG signals is based on signal to noise ratio and maximum phase discrimination.

3. The arrhythmia monitoring device of claim 1 , wherein the processor is coupled via a therapy delivery interface to therapy electrodes configured to deliver an electrical shock to a heart of the patient.

4. The arrhythmia monitoring device of claim 1 , wherein the processor is coupled via a therapy delivery interface to therapy electrodes configured to deliver defibrillation therapy to the patient in response to detection of the cardiac arrhythmia.

5. The arrhythmia monitoring device of claim 1 , wherein the plurality of ECG sensing electrodes are distributed about the torso of the patient at spaced apart positions.

6. The arrhythmia monitoring device of claim 5 , wherein the plurality of ECG sensing electrodes includes ECG sensing electrodes that are located about sides, a front, and a back of the torso.

7. The arrhythmia monitoring device of claim 5 , wherein the plurality of ECG sensing electrodes are not all located on a common plane.

8. The arrhythmia monitoring device of claim 1 , wherein the plurality of ECG sensing electrodes are formed from electrically conductive threads sewn into the garment.

9. The arrhythmia monitoring device of claim 1 , wherein the plurality of ECG sensing electrodes comprises dry-sensing capacitive or conductive electrodes.

10. The arrhythmia monitoring device of claim 1 , wherein the plurality of ECG sensing electrodes are configured to be attached to the garment by at least one of an adhesive, a hook and loop fastener, a magnetic attached to the garment, or by being sewn into the garment.

11. The arrhythmia monitoring device of claim 1 , wherein the processor is further configured to re-execute the selection process in response to an additional event.

12. The arrhythmia monitoring device of claim 11 , further comprising at least one activity sensor configured to monitor physical activity of the patient, and wherein the additional event comprises strenuous physical activity of the patient.

13. The arrhythmia monitoring device of claim 11 , wherein the additional event comprises an expiration of a predetermined time interval.

14. The arrhythmia monitoring device of claim 11 , wherein the additional event comprises a powering on of the device.

15. The arrhythmia monitoring device of claim 1 , wherein, in response to detection of the cardiac arrhythmia, the processor is further configured to

monitor an additional ECG signal having the next highest quality to the at least two ECG signals selected from the re-executed selection process, and

increase a confidence level indicative of the detected cardiac arrhythmia requiring treatment.

16. The arrhythmia monitoring device of claim 15 , wherein the processor is further configured to execute one or more instructions to deliver defibrillation treatment to the torso of the patient when the confidence level is above a threshold.

17. An arrhythmia monitoring and treatment device comprising:

a garment configured to be worn about a torso of a patient;

a plurality of ECG sensing electrodes configured to be disposed in the garment, the plurality of ECG sensing electrodes comprising a plurality of pairs of ECG sensing electrodes configured to provide a plurality of ECG signals, each of the plurality of ECG signals corresponding to one of the plurality of pairs of ECG sensing electrodes;

a signal acquisition circuit configured to be disposed in the garment, the signal acquisition circuit being configured to be electrically coupled to the plurality of ECG sensing electrodes and configured to sense the plurality of ECG signals;

a plurality of therapy electrodes configured to deliver an electrical shock to a heart of the patient, the plurality of therapy electrodes in communication with a therapy delivery interface;

a processor in communication with the signal acquisition circuit and the therapy delivery interface, the processor being configured to

execute a selection process comprising

identifying a quality of each of the plurality of ECG signals,

ranking the plurality of ECG signals relative to a remainder of the plurality of ECG signals based on the identified quality of each of the plurality of ECG signals, and
