Adjusted multi-biomarker disease activity score for inflammatory disease assessment
Provided herein are methods for assessing response to inflammatory disease therapy. The methods include performing immunoassays to generate scores based on quantitative data for expression of biomarkers relating to inflammatory biomarkers to assess disease activity in inflammatory diseases, e.g., rheumatoid arthritis. Also provided are methods of adjusting disease activity scores to account for variables that can influence such scores.
1. A method for assessing a subject for treatment for rheumatoid arthritis disease activity, the method comprising:
obtaining a blood sample from the subject;
performing an immunoassay to measure protein expression values of at least six protein markers in the sample, the at least six markers comprising chitinase 3-like 1 (cartilage glycoprotein-39) (CHI3L1), interleukin 6 (interferon, beta 2) (IL6), leptin (LEP), matrix metallopeptidase 1 (interstitial collagenase) (MMP1), matrix metallopeptidase 3 (stromelysin 1, progelatinase) (MMP3), and serum amyloid Al (SAA1);
generating a multi-biomarker disease activity (MBDA) score by (a) weighting the determined expression of each protein marker with a predefined coefficient, and (b) combining the weighted expression; and
calculating a disease activity score for the subject according to the formula:

wherein the values+0.437, +3.31, +0.0502,−0.0247,−0.000483, and +0.00254 are stated to a 95% Confidence Interval;
wherein age is the subject's age;
wherein leptin is the subject's leptin value; and
wherein 1male (sex) is 1 for a male subject and is zero otherwise;
determining that the subject has a disease activity score of 45 to 100 and a high level of rheumatoid arthritis disease activity; and
administering a DMARD or biologic DMARD therapy to the subject having a disease activity score of 45 to 100 and a high level of rheumatoid arthritis disease activity.

The method of claim 1 , wherein the protein markers further comprise an additional protein marker comprising at least one of C-reactive protein pentraxin-related (CRP), epidermal growth factor (beta-urogastrone) (EGF), resistin (RETN), tumor necrosis factor receptor superfamily, member 1A (TNFRSF1A), vascular cell adhesion molecule 1 (VCAM1), and vascular endothelial growth factor A (VEGFA).
3. The method of claim 2 , wherein the at least one additional protein marker is CRP.
4. The method of claim 2 , wherein the disease activity score is calculated using markers comprising IL6, EGF, VEGFA, LEP, SAA1, VCAMI, CRP, MMP1, MMP3, TNFRSF1A, RETN, and CHI3L1.
