Abstract

Devices, methods, and systems for the treatment of tissue using energy delivery. Specifically, certain embodiments may be used for the treatment of lung tissue, such as lung nodules, using RF ablation, via a catheter provided with a first electrode attached to a distal end of the catheter, wherein the first electrode is hollow, wherein the first electrode comprises a piercing tip configured to pierce through an airway wall and a second electrode received in a movable manner within the first electrode, wherein the second electrode is extendable from the first electrode to form a first extended configuration.

Claims (55)

1. A device comprising:

a guide sheath;

a catheter including an elongate body configured to be advanced through the guide sheath to a target location in a lumen within a patient, the catheter comprising a channel terminating in a distal port at a distal end of the catheter, the distal end of the catheter including the distal port and a first electrode forming at least an outer surface of the distal port of the catheter, wherein the channel of the catheter is hollow and configured to allow delivery of a liquid solution through the catheter and out of the distal port, wherein the distal end of the catheter further comprises a piercing tip configured to pierce through an airway wall;

a second electrode configured to be received in a longitudinally movable manner within the channel and configured to allow delivery of the liquid solution within the channel,

wherein at least one of an inner surface of the first electrode or an outer surface of the second electrode includes an insulating layer to electrically insulate the first electrode from the second electrode within at least a portion of the channel,

wherein a distal section of the second electrode is in a first configuration when at a first longitudinal position that is within the channel, and

wherein the distal section of the second electrode is in a second configuration when at a second longitudinal position that is distal from the distal port;

a first electrical lead in electric communication with the first electrode; and

a second electrical lead in electric communication with the second electrode, wherein the first and second electrical leads are connectable to a source of electric power,

wherein the second configuration comprises a helical configuration.

2. The device of claim 1 , further comprising a handle attached at a proximal end of the catheter, wherein the handle comprises an activation device configured to extend the second electrode from the first electrode.

3. The device of claim 1 , wherein the second electrode extends from within the channel of the catheter and through the distal port.

4. The device of claim 1 , wherein the catheter is dimensioned to be insertable into a bronchoscope comprising a side-facing ultrasound probe.

The device of claim 1 , wherein the second electrode is constructed at least in part from a shape-memory material having a martensite configuration at a first lower temperature, and an austenite configuration at a second temperature above body temperature, and wherein the second electrode is configured to adopt a first straight configuration while in the martensite configuration, and a bend or coil while in the austenite configuration.

6. The device of claim 1 , wherein the first and second electrodes comprise a bipolar radiofrequency ablation electrode.

7. The device of claim 1 , further comprising a fluid source connected to a proximal end of the catheter and in fluid communication with at least one of the first or second electrodes.

8. A system for the delivery of energy to a region of lung tissue, the system comprising:

an energy delivery device comprising:

an airway component configured to be inserted into an airway, the airway component comprising an elongate body including a channel, a first electrode, and a second electrode; and

a handle attached at a proximal end of the airway component, the handle comprising a first activation toggle configured to extend the second electrode from within the channel, wherein a distal end of the channel is formed by the first electrode,

wherein the first electrode forms at least a portion of an outer surface of a distal end of the elongate body and surrounds a distal end of the channel;

wherein the second electrode is received in a movable manner within the channel and the first electrode,

wherein at least one of an inner surface of the first electrode or an outer surface of the second electrode includes an insulating layer to electrically insulate the first electrode from the second electrode within at least a portion of the channel,

wherein the second electrode is extendable from a distal end of the first electrode to form a first extended configuration,

wherein the second electrode is in a first configuration when within the first electrode,

wherein when a distal end of the second electrode extends distally from the first electrode, a portion of the second electrode that is extended from the first electrode transitions to a second configuration;

a power source connected to the first electrode and the second electrode, the power source configured to deliver bipolar RF energy; and

a conduit insertable into an airway and selected from the group consisting of a bronchoscope and a guide sheath, the airway component being insertable and movable therein,

wherein when in the first configuration the second electrode conforms to the first electrode,

wherein the second configuration is at least one of a helical or a coiled configuration.

9. The system of claim 8 , wherein when in the first configuration a centerline of the distal end of the second electrode is parallel to a centerline of the first electrode,

wherein when in the second configuration the centerline of the distal end of the second electrode is not parallel to the centerline of the first electrode.

10. A device comprising:

a guide sheath;

a catheter configured to be inserted into the guide sheath that together are configured to be inserted into an airway of a patient and extend to a target location in a lumen within the airway, the catheter comprising a first electrode forming an outer surface of a distal port located at a distal end of the catheter, wherein the catheter is hollow, thereby forming a channel configured to allow delivery of a liquid solution to the distal port of the catheter, wherein the distal end of the catheter comprises a piercing tip configured to pierce through an airway wall;

a second electrode configured to be received in a movable manner within the channel of the catheter and configured to allow delivery of the liquid solution within the channel, wherein a distal section of the second electrode is in a first configuration when within the channel, wherein when the distal section of the second electrode is extended distally from the distal end of the first electrode and the catheter, the distal section of the second electrode that is extended from the first electrode is in a second configuration;

a first electrical lead in electric communication with the first electrode; and

a second electrical lead in electric communication with the second electrode, wherein the first and second electrical leads are connectable to a source of electric power,

wherein the second configuration comprises a helical configuration.

11. The device of claim 10 , further comprising a handle attached at a proximal end of the catheter, wherein the handle comprises an activation device configured to extend the second electrode from the first electrode.

12. The device of claim 10 , wherein the second electrode extends from within the channel through the distal port at the distal end of the first electrode.

13. The device of claim 10 , wherein the catheter is dimensioned to be insertable into a bronchoscope comprising a side-facing ultrasound probe.

14. The device of claim 10 , wherein the second electrode is constructed at least in part from a shape-memory material having a martensite configuration at a first lower temperature, and an austenite configuration at a second temperature above body temperature, and wherein the second electrode is configured to adopt a first straight configuration while in the martensite configuration, and a bend or coil while in the austenite configuration.

15. The device of claim 10 , wherein the first and second electrodes comprise a bipolar radiofrequency ablation electrode.

16. The device of claim 10 , further comprising a fluid source connected to a proximal end of the catheter and in fluid communication with at least one of the first or second electrodes.

17. A ablation system for use through a bronchoscope, the system comprising:

a catheter configured to be inserted into an airway using a bronchoscope, the catheter comprising an elongate body including a hollow channel terminating in a distal port;

a first electrode including a piercing tip and forming an outer surface of the distal port of the hollow channel in the catheter, wherein the piercing tip is configured to pierce through an airway wall;

a second electrode movably disposed within the hollow channel and extendable out through the distal port, the second electrode including a distal section in a first configuration while disposed within the hollow channel transitioning to a second configuration when extended out of the distal port;

a first electrical lead in electric communication with the first electrode; and

a second electrical lead in electric communication with the second electrode, wherein the first and second electrical leads are connectable to a source of electric power,

wherein the second configuration comprises a helical configuration,

wherein at least one of an inner surface of the first electrode or an outer surface of the second electrode includes an insulating layer to electrically insulate the first electrode from the second electrode within at least a portion of the channel.

18. The system of claim 17 , wherein the helical configuration of the distal section of the second electrode includes a major diameter larger than a diameter of the distal port.

19. The system of claim 18 , wherein a proximal section of the second electrode remains disposed within the hollow channel inside the distal port.
