Abstract

The use of red or near infrared light upon neurons of the lumbar plexus that are in distress due to retraction-induced ischemia. The surgeon may protect nerves made ischemic in the surgery by: a) making an incision in a patient, b) inserting an access device into the patient through the incision to at least partially create a path to a spine of the patient, and c) irradiating nervous tissue adjacent the path with an amount of NIR or red light effective to provide neuroprotection.

Claims (39)

1. A retractor system, comprising:

a first retractor component adapted to retract tissue, the first retractor component having a proximal end portion, a distal end portion, an outer surface and an inner surface;

a second retractor component configured to form a working channel through tissue, the working channel being sized to receive a portion of the first retractor component therethrough;

wherein a first portion of the second retractor component is configured to move relative to a second portion of the second retractor component to adjust an orientation of the working channel,

wherein the first retractor component is configured to rotate about substantially an entire inner circumference of the second retractor component when the portion of the first retractor component is received within the second retractor component, and

wherein the outer surface of the first retractor component conforms to an inner surface of the second retractor component.

2. The system of claim 1 , wherein the first retractor component is configured to move relative to the second retractor component once disposed within the working channel formed by the second retractor component.

3. The system of claim 1 , wherein the first retractor component includes a light source on a distal end thereof.

4. The system of claim 3 , wherein the light source is removably coupled to the first component.

5. The system of claim 3 , wherein the light source comprises a red/NIR light emitter.

6. The system of claim 3 , wherein the light source runs along a proximal portion of the first retractor component and terminates in the distal end portion of the first retractor component.

7. The system of claim 1 , wherein the proximal end portion of the first retractor component is configured to engage a proximal end of the second retractor component.

8. The system of claim 1 , wherein the second retractor component includes one or more reliefs formed in the inner surface thereof.

9. The system of claim 8 , wherein the first retractor component is secured within the reliefs formed in the inner surface of the second retractor component.

10. The system of claim 1 , wherein the first retractor component is configured to simultaneously extend distal to the working channel of the second retractor component and proximal to the working channel of the second retractor component.

11. The system of claim 1 , further comprising an extension adapted to dock the retractor system to spinal tissue.

12. The system of claim 11 , wherein the extension is axially adjustable with respect to the first component.

13. The system of claim 1 , wherein the first retractor component includes one or more features for interfacing with the second retractor component to couple the first retractor component to the second retractor component.

14. The system of claim 13 , wherein the one or more features include a protrusion configured to be received in a corresponding groove of the second retractor component.

15. The system of claim 1 , wherein a proximal-most end of the first retractor component extends, in a plane perpendicular to a central axis of the working channel, beyond a diameter of the working channel formed by the second retractor component.

16. The system of claim 1 , wherein the proximal end portion of the first retractor component is configured to rotate in tandem with the portion of the first retractor component received in the second retractor component.

17. The system of claim 1 , wherein the second retractor component is substantially stationary with respect to the first retractor component during rotation of the first retractor component within the second retractor component.

18. A retractor system, comprising:

a first retractor component adapted to retract tissue, the first retractor component having a proximal end portion, a distal end portion, an outer surface and an inner surface;

a second retractor component configured to form a working channel through tissue, the working channel being sized to receive a portion of the first retractor component therethrough;

wherein a first portion of the second retractor component is configured to move relative to a second portion of the second retractor component to adjust an orientation of the working channel,

wherein the first retractor component is configured to rotate about substantially an entire inner circumference of the second retractor component when the portion of the first retractor component is received within the second retractor component, and

wherein the first retractor component includes one or more features for interfacing with the second retractor component to couple the first retractor component to the second retractor component.

19. A retractor system, comprising:

a first retractor component adapted to retract tissue, the first retractor component having a proximal end portion, a distal end portion, an outer surface and an inner surface;

a second retractor component configured to form a working channel through tissue, the working channel being sized to receive a portion of the first retractor component therethrough;

wherein a first portion of the second retractor component is configured to move relative to a second portion of the second retractor component to adjust an orientation of the working channel,

wherein the first retractor component is configured to rotate about substantially an entire inner circumference of the second retractor component when the portion of the first retractor component is received within the second retractor component, and

wherein the first retractor component is configured to pass a location along the second retractor component a plurality of times when rotating in a given direction about substantially the entire inner circumference of the second retractor component.

20. A retractor system, comprising:

a first retractor component adapted to retract tissue, the first retractor component having a proximal end portion, a distal end portion, an outer surface and an inner surface;

a second retractor component configured to form a working channel through tissue, the working channel being sized to receive a portion of the first retractor component therethrough;

wherein a first portion of the second retractor component is configured to move relative to a second portion of the second retractor component to adjust an orientation of the working channel, and

wherein the first retractor component is configured to rotate about an entire inner circumference of the second retractor component when the portion of the first retractor component is received within the second retractor component.
