Abstract

Hysteroscopic system includes a hysteroscope having a main body coupled to an extension portion. The extension portion may be a shaft configured to extend transcervically to a patient's uterine cavity. First, second, and third channels extend from the main body to a distal end of the extension portion. A fluid source is coupleable to a proximal end of the first channel, and a pressure sensor is coupleable to a proximal end of the second channel. A tissue resecting probe is configured for introduction through the third channel. At least one resistance feature is included which is configured to provide a selected level of resistance to axial sliding of the probe through the third channel while permitting rotation of the probe within the third channel.

Claims (29)

1. An endoscopic system, comprising:

an endoscope having a metallic main body and an elongate shaft extending distally from the main body, the endoscope including:

a working channel extending through the main body and the elongate shaft, and

at least one fluid flow channel extending from the main body through the elongate shaft;

a tissue resecting device including an elongate outer sleeve configured for introduction through the working channel of the endoscope; and

a plastic disposable adapter detachably coupled to a proximal end of the main body;

wherein the working channel has a straight channel portion extending longitudinally through the elongate shaft to a distal end of the elongate shaft;

wherein the working channel extends proximally to an aperture in a proximal face of the main body;

wherein a center of the aperture is offset from an axis of the straight channel portion extended to the proximal face;

wherein the disposable adapter is configured to seal the working channel with and without the tissue resecting device disposed in the working channel.

2. The endoscopic system of claim 1 , wherein the disposable adapter includes a proximal seal and a distal seal disposed therein.

3. The endoscopic system of claim 2 , wherein the proximal seal is configured to stretch and impinge upon the tissue resecting device when the tissue resecting device is disposed in the working channel.

4. The endoscopic system of claim 2 , wherein the distal seal is configured to seal the working channel when the tissue resecting device is absent from the working channel.

5. The endoscopic system of claim 4 , wherein the distal seal is a duck-bill seal.

6. The endoscopic system of claim 1 , wherein the disposable adapter includes an o-ring disposed between the disposable adapter and the main body when the disposable adapter is coupled to the proximal end of the main body.

7. The endoscopic system of claim 1 , wherein the disposable adapter includes a proximal port in communication with the working channel.

8. The endoscopic system of claim 7 , wherein the disposable adapter includes a lumen having a centerline extending from the proximal port to the aperture in the proximal face of the main body, the lumen being configured to receive the tissue resecting device therein.

9. The endoscopic system of claim 8 , wherein the lumen of the disposable adapter extends through an enlarged interior chamber of the disposable adapter.

10. The endoscopic system of claim 9 , wherein at least a portion of the interior chamber is offset from the centerline of the lumen.

11. The endoscopic system of claim 9 , wherein at least a portion of the interior chamber is non-concentric with the centerline of the lumen.

12. The endoscopic system of claim 9 , wherein the interior chamber is configured with a pressure relief valve.

13. The endoscopic system of claim 9 , wherein the axis of the straight channel portion, when extended proximal of the proximal face, extends into the interior chamber of the disposable adapter.

14. The endoscopic system of claim 1 , wherein the disposable adapter is detachably coupled to the proximal end of the main body by a J-lock.

15. The endoscopic system of claim 1 , wherein the endoscope includes an optics channel extending from the main body through the elongate shaft.

16. The endoscopic system of claim 15 , wherein the main body includes an angled extension portion having optics communicating with the optics channel.

17. The endoscopic system of claim 1 , wherein the tissue resecting device includes an inner sleeve configured to move within and relative to the outer sleeve.

18. The endoscopic system of claim 1 , wherein the at least one fluid flow channel includes a first fluid channel configured to be coupled to a fluid source and a second fluid channel configured to be coupled to a pressure sensor.

19. The endoscopic system of claim 1 , wherein the working channel includes a proximal portion extending within the main body to the aperture.

20. The endoscopic system of claim 19 , wherein a centerline of the proximal portion is non-parallel to the axis of the straight channel portion.
