Abstract

Stomach tissue is manipulated and fastened with a tissue grasper and a tissue shaper to form folds in the stomach. The tissue grasper and the tissue shaper rotate and translate relative to one another. The tissue grasper has a plurality of vacuum orifices to adhere the tissue grasper to tissue.

Claims (50)

1 . A method of forming stomach tissue at an intersection of the stomach and the esophagus adjacent the lesser curvature, comprising the steps of:

providing a device having a tissue shaper;

introducing the tissue shaper into an esophageal tract in a first position;

maintaining the tissue shaper in the first position while inducing a curvature about one or more elongate elements, wherein:

each elongate element is coupled to at least one coil-shaped tissue displacing element, and wherein:

each coil-shaped tissue displacing element is coupled with only one elongate element;

maintaining the tissue shaper in the first position while engaging stomach tissue with three coil-shaped tissue displacing elements;

displacing the stomach tissue into the tissue shaper after the introducing, inducing, and engaging steps while the tissue shaper remains in the first position;

individually displacing the three coil-shaped tissue displacing elements until a threshold tension is reached and then applying a lock to lock the three coil-shaped tissue displacing elements;

applying a plurality of fasteners through the stomach tissue moved into the first tissue shaper with the three coil-shaped tissue displacing elements;

the applying step is carried out to form a fold having a convex surface, the convex surface facing inward relative to the esophagus; and

releasing the stomach tissue after the applying step so that the stomach tissue positioned in the tissue shaper falls back into the stomach.

2 . The method of claim 1 , wherein:

the providing step is carried out with an open end of the tissue shaper being at a distal end.

3 . The method of claim 1 , wherein:

the engaging step is carried out with the three coil-shaped tissue displacing elements extending distally through an open end of the tissue shaper and then proximally to engage fundus along the lesser curvature.

4 . The method of claim 1 , wherein:

the positioning step is carried out with the tissue shaper being positioned in the esophageal tract; and

the applying step being carried out to apply the plurality of fasteners at a location above the junction between the esophageal tract and the stomach along the greater curvature side.

5 . The method of claim 4 , wherein:

the applying step is carried out to apply the plurality of fasteners at the location at least 3 cm above the junction.

6 . The method of claim 1 , further comprising the step of:

releasing the fold after the applying step so that the fold falls back into the stomach and is positioned on the lesser curvature opposite the junction between the esophageal tract and the stomach along the greater curvature side;

the moving step being carried out until enough stomach tissue has entered the tissue shaper so that the fold opposes the junction on the greater curvature side when released.

7 . The method of claim 1 , wherein:

the applying step is carried out so that the fold opposes a junction on the greater curvature side and extends proximally and distally of the junction on the greater curvature side.

8 . The method of claim 1 , wherein:

inducing a curvature about one or more elongate elements coupled to one or more coil-shaped tissue displacing elements comprises:

retracting a sheath from the one or more elongate elements.

9 . A method of forming stomach tissue at an intersection of the stomach and the esophagus adjacent the lesser curvature, comprising the steps of:

providing a device having a tissue shaper;

introducing the tissue shaper into an esophageal tract in a first position;

maintaining the tissue shaper in the first position while retracting a sheath from one or more coil-shaped tissue displacing elements;

maintaining the tissue shaper in the first position while engaging stomach tissue with three coil-shaped tissue displacing elements;

displacing the stomach tissue into the tissue shaper after the introducing, retracting and engaging steps while the tissue shaper remains in the first position;

individually displacing the three coil-shaped tissue displacing elements until a threshold tension is reached and then applying a lock to lock the three coil-shaped tissue displacing elements;

applying a plurality of fasteners through the stomach tissue moved into the first tissue shaper with the three coil-shaped tissue displacing elements;

the applying step is carried out to form a fold having a convex surface, the convex surface facing inward relative to the esophagus; and

releasing the stomach tissue after the applying step so that the stomach tissue positioned in the tissue shaper falls back into the stomach.

10 . A method of forming stomach tissue at an intersection of the stomach and the esophagus adjacent the lesser curvature, comprising the steps of:

providing a device having a tissue shaper;

introducing the tissue shaper into an esophageal tract in a first position;

maintaining the tissue shaper in the first position while engaging stomach tissue with three coil-shaped tissue displacing elements;

displacing the stomach tissue into the tissue shaper after the introducing and engaging steps while the tissue shaper remains in the first position;

individually displacing the three coil-shaped tissue displacing elements until a threshold tension is reached and then applying a lock to lock the three coil-shaped tissue displacing elements, and;

applying a plurality of fasteners through the stomach tissue moved into the tissue shaper with the three coil-shaped tissue displacing elements, wherein:

the applying step is carried out to form a fold having a convex surface, the convex surface facing inward relative to the esophagus, and;

releasing the stomach tissue after the applying step so that the stomach tissue positioned in the tissue shaper falls back into the stomach.

11 . The method of claim 10 , between the applying and releasing steps, further comprising the step of:

displacing the stomach tissue using one or more of the three coil-shaped tissue displacing elements to position it for one or more additional applying steps.
