Abstract

A defibrillator (AED) and method for using a defibrillator using two different ECG analysis algorithms which work sequentially to improve the accuracy of AED shock decisions. A first algorithm, such as (ART), is particularly suited for analysis in the presence of CPR periods. A second algorithm, such as (PAS), is particularly suited for analysis during hands-off periods. The AED switches algorithms depending on the period and on the current analysis of the cardiac rhythm. The inventions thus provide an optimized ECG analysis scheme in a manner that improves the effectiveness of the rescue, resulting in more CPR “hands-on” time, better treatment of refibrillation, and reduced transition times between CPR and electrotherapy.

Claims (43)

1. A method for automatically switching between a first ECG analysis algorithm and a second ECG analysis algorithm in a defibrillator, comprising the steps of:

providing a defibrillator having an ECG signal input, a user interface, a shock delivery circuit, a first ECG analysis algorithm that is operable to determine one of a “shock advised” (SA) and an “undecided” determination from the ECG signal in the presence of a CPR-related signal noise artifact from the input, and a second ECG analysis algorithm that is operable to determine one of a SA and a “No Shock Advised” (NSA) determination from the ECG signal in the absence of CPR-related signal noise artifact from the input;

analyzing the ECG signal with the first ECG analysis algorithm during a first period to determine a SA;

if the SA is determined during the first period, operating the ECG signal with the first ECG analysis algorithm during a subsequent second period to determine a subsequent SA;

if any other than a SA is determined during the first period, automatically switching from the first to the second ECG analysis algorithm,

analyzing the ECG signal with the second ECG analysis algorithm;

if the second ECG analysis algorithm determines an SA, then arming the shock delivery circuit for delivering electrotherapy, and

following the delivery, automatically switching back from the second to the first ECG analysis algorithm in the second period,

wherein the second period comprises a continuous CPR rescue mode of operation, wherein the continuous CPR rescue mode of operation further comprises the steps of:

determining a second SA with the first ECG analysis algorithm;

arming the shock delivery circuit for delivering electrotherapy; and

immediately issuing instructions via the user interface to stop CPR for the delivery.

2. The method of claim 1 , further comprising the step of:

analyzing the ECG signal with the second ECG analysis algorithm during a third period having a fixed duration that is shorter than a duration of either of the first or second periods.

The method of claim 1 , wherein the first period is an initialization period that occurs just after activating the defibrillator, and further wherein the first period comprises a scheduled CPR rescue mode of operation, wherein the scheduled CPR rescue mode of operation provides a predetermined period of uninterruptible CPR regardless of the determination.

4. The method of claim 3 , wherein the predetermined period of uninterruptible CPR comprises a sensed number of CPR compressions, wherein the sensed number is about 30.

5. A method for automatically switching between a first ECG analysis algorithm and a second ECG analysis algorithm in a defibrillator, comprising the steps of:

providing a defibrillator having an ECG signal input, a user interface, a shock delivery circuit, a first ECG analysis algorithm that is operable to determine one of a “shock advised” (SA) and an “undecided” determination from the ECG signal in the presence of a CPR-related signal noise artifact from the input, and a second ECG analysis algorithm that is operable to determine one of a SA and a “No Shock Advised” (NSA) determination from the ECG signal in the absence of CPR-related signal noise artifact from the input;

analyzing the ECG signal with the first ECG analysis algorithm during a first period to determine a SA;

if the SA is determined during the first period, operating the ECG signal with the first ECG analysis algorithm during a subsequent second period to determine a subsequent SA;

if any other than a SA is determined during the first period, automatically switching from the first to the second ECG analysis algorithm,

analyzing the ECG signal with the second ECG analysis algorithm;

if the second ECG analysis algorithm determines an NSA, then automatically switching back from the second to the first ECG analysis algorithm in the second period,

wherein the second period comprises a scheduled CPR rescue mode of operation, wherein the scheduled CPR rescue mode of operation further comprises the steps of:

determining an SA with the first ECG analysis algorithm;

arming the shock delivery circuit for delivering electrotherapy; and

then after a predetermined period of uninterruptible CPR issuing instructions via the user interface to stop CPR for the delivery.

6. The method of claim 5 , further comprising the step of:

analyzing the ECG signal with the second ECG analysis algorithm during a third period having a fixed duration that is shorter than a duration of either of the first or second periods.

The method of claim 5 , wherein the first period is an initialization period that occurs just after activating the defibrillator, and further wherein the first period comprises a scheduled CPR rescue mode of operation, wherein the scheduled CPR rescue mode of operation provides a predetermined period of uninterruptible CPR regardless of the determination.

8. The method of claim 7 , wherein the predetermined period of uninterruptible CPR comprises a sensed number of CPR compressions, wherein the sensed number is about 30.

9. An automated external defibrillator (AED) for use during cardiopulmonary resuscitation (CPR) comprising:

a user interface having at least one of an aural instruction output and a visual display;

an input of an ECG signal;

a shock delivery circuit;

a memory for storing instructions related to

a first ECG analysis algorithm that is operable to determine one of a “shock advised” (SA) and an “undecided” determination from the ECG signal in the presence of a CPR-related signal noise artifact from the input,

a second ECG analysis algorithm that is operable to determine one of a SA and a “No Shock Advised” (NSA) determination from the ECG signal in the absence of CPR-related signal noise artifact from the input, and

a CPR rescue protocol that includes at least two periods for providing CPR compressions, wherein one of the CPR rescue protocol periods is an initialization period that occurs just after activating the AED, and further wherein the initialization period comprises a scheduled CPR rescue mode of operation using the first ECG analysis algorithm, wherein the scheduled CPR rescue mode of operation provides a predetermined period of uninterruptible CPR regardless of the determination; and

a controller disposed in controlling communication with the user interface, the input, the memory and the shock delivery circuit to arm the shock delivery circuit responsive to any SA determination from either of the first or second algorithms,

wherein the controller is further operable to always automatically switch from the first to the second ECG analysis algorithm at an end of one of the periods during which the first ECG analysis algorithm determines any other than a SA determination.

10. The AED of claim 9 , wherein the predetermined period of uninterruptible CPR comprises a sensed number of CPR compressions, wherein the sensed number is about 30.

11. The AED of claim 10 , wherein another CPR rescue protocol period subsequent to the initialization period has a duration longer than the initialization period, wherein the duration of the initialization period is between about 20 and 30 seconds.
