Abstract

A system for percutaneous spinal fusion may include two spaced apart blades connected together by a coupling such that the blades define a percutaneous pathway from a skin incision to an implanted pedicle fastener. The coupling may be c-shaped and may have at least one flexible tab for engaging one or more holes along the length of the blades. If one of the blades becomes disconnected from the pedicle fastener, a supplemental access device may be provided comprising a tubular body having a channel therein for receiving the other of the blades. If both of the blades become disconnected, a supplemental access device may be provided comprising a gripping member received within a locking member. The gripping member may have two legs engageable with the pedicle fastener, and the locking member may move along the gripping member to prevent the legs from disengaging the pedicle fastener.

Claims (27)

1. A method of spinal fusion, comprising:

coupling a first coupling to an elongated first percutaneous access device extending proximally from a first pedicle fastener along a first longitudinal axis, the first percutaneous access device defining a first pathway therein along the first longitudinal axis such that the first pathway terminates at the first pedicle fastener at a distal end of the first percutaneous access device;

coupling a second coupling to an elongated second percutaneous access device extending proximally from a second pedicle fastener along a second longitudinal axis, the second percutaneous access device defining a second pathway therein along the second longitudinal axis such that the second pathway terminates at the second pedicle fastener at a distal end of the second percutaneous access device;

moving the first coupling to a first position along the first longitudinal axis while the first coupling is coupled to the first percutaneous access device, the first coupling defining a first reference location;

moving the second coupling to a second position along the second longitudinal axis while the second coupling is coupled to the second percutaneous access device, the second coupling defining a second reference location; and

configuring or selecting a fixation rod based on relative positions of the first and second reference locations.

2. The method of claim 1 , further comprising inserting the first pedicle fastener into a body of a patient and affixing the first pedicle fastener to a pedicle of a vertebra of the patient's spine while the first percutaneous access device is connected to the first pedicle fastener.

3. The method of claim 2 , wherein the first coupling is coupled to the first percutaneous access device during the inserting and affixing steps.

4. The method of claim 1 , further comprising positioning the fixation rod in simultaneous engagement with the first reference location of the first coupling and the second reference location of the second coupling .

The method of claim 4 , wherein the step of positioning the fixation rod in simultaneous engagement with the first and second reference locations includes positioning the fixation rod in a position simultaneously received by a first recess of the first coupling and a second recess of the second coupling.

The method of claim 4 , wherein the first position of the first coupling is spaced a first distance away from the pedicle fastener, and wherein the second position of the second coupling is spaced a second distance away from the second pedicle fastener, the first and second distances being the same.

7. The method of claim 1 , wherein the first percutaneous access device comprises a first blade and a second blade positioned adjacent to one another to provide the first pathway therebetween.

8. The method of claim 7 , wherein the step of coupling the first coupling to the first percutaneous access device includes receiving the first blade within a first channel of the first coupling and receiving the second blade within a second channel of the first coupling.

9. The method of claim 1 , wherein the first position is defined by a first alignment feature of the first coupling moving into alignment with at least one second alignment feature of the first percutaneous access device.

10. The method of claim 9 , wherein the at least one second alignment feature of the first percutaneous access device comprises a plurality of second alignment features spaced apart from one another along the longitudinal axis of the first percutaneous access device.

11. The method of claim 10 , wherein the step of moving the first coupling to the first position along the longitudinal axis includes moving the first coupling past at least one of the plurality of second alignment features.

12. The method of claim 11 , wherein the step of moving the first coupling past at least one of the plurality of second alignment features includes receiving feedback when the first alignment feature of the first coupling passes the at least one of the plurality of second alignment features.

13. The method of claim 9 , wherein the at least one second alignment feature of the first percutaneous access device includes a plurality of apertures spaced apart from one another along the longitudinal axis of the first percutaneous access device.

14. The method of claim 13 , wherein the first alignment feature of the first coupling includes a protuberance configured to selectively engage the apertures of the first percutaneous access device.

15. The method of claim 1 , further comprising inserting the fixation rod into a body of a patient along at least a portion of the first and second pathways defined by the respective first and second percutaneous access devices.

16. A method of spinal fusion, comprising:

inserting a first pedicle fastener into a body of a patient and affixing the first pedicle fastener to a first pedicle of a first vertebra of the patient's spine while a first coupling is coupled to an elongated first percutaneous access device connected to the first pedicle fastener and extending proximally therefrom, the first percutaneous access device defining a first pathway therein along a first longitudinal axis such that the first pathway terminates at the first pedicle fastener at a distal end of the first percutaneous access device, the first coupling being coupled to the first percutaneous access device at a first position along the first longitudinal axis, the first coupling defining a first reference location, wherein the first position is defined by a first alignment feature of the first coupling being aligned with at least one second alignment feature of the first percutaneous access device, the at least one second alignment feature of the first percutaneous access device comprising a plurality of second alignment features spaced apart from one another along the first longitudinal axis of the first percutaneous access device;

inserting a second pedicle fastener into the body of the patient and affixing the second pedicle fastener to a second pedicle of a second vertebra of the patient's spine while a second coupling is coupled to an elongated second percutaneous access device connected to the second pedicle fastener and extending proximally therefrom, the second percutaneous access device defining a second pathway therein along a second longitudinal axis such that the second pathway terminates at the second pedicle fastener at a distal end of the second percutaneous access device, the second coupling being coupled to the second percutaneous access device at a second position along the second longitudinal axis, the second coupling defining a second reference location, wherein the second position is defined by a first alignment feature of the second coupling being aligned with at least one second alignment feature of the second percutaneous access device, the at least one second alignment feature of the second percutaneous access device comprising a plurality of second alignment features spaced apart from one another along the second longitudinal axis of the second percutaneous access device; and

configuring or selecting a fixation rod based on relative positions of the first and second reference locations.

17. The method of claim 16 , wherein the first position of the first coupling is spaced a first distance away from the first pedicle fastener, and wherein the second position of the second coupling is spaced a second distance away from the second pedicle fastener, the first and second distances being the same.

18. The method of claim 17 , further comprising positioning a fixation rod in simultaneous engagement with the first reference location of the first coupling and the second reference location of the second coupling.

19. The method of claim 16 , further comprising inserting the fixation rod into the body of the patient along at least a portion of the first pathway defined by the first percutaneous access device and along at least a portion of the second pathway defined by the second percutaneous access device.
