Abstract

Described here are methods and systems for the manipulation of ovarian tissues. The methods and systems may be used in the treatment of polycystic ovary syndrome (PCOS). The systems and methods may be useful in the treatment of infertility associated with PCOS.

Claims (31)

1. A method for treating a patient having polycystic ovary syndrome, the method comprising:

advancing a docking device into a needle guide of an ultrasound probe, the ultrasound probe further comprising a probe handle and an ultrasound transducer, the docking device comprising a therapeutic element slidable within and deployable from a lumen of the docking device;

advancing the ultrasound probe and the docking device into the vaginal canal of the patient;

imaging the ovary using the ultrasound transducer in an imaging plane;

advancing the docking device through a vaginal wall;

penetrating an ovary at a single entry point with the docking device or the therapeutic element;

advancing the therapeutic element from the docking device into the ovary in the same planar orientation as the imaging plane; and

delivering energy into tissue from inside the ovary using the therapeutic element positioned inside the ovary to ablate 3% to 20% of the tissue within the ovary to treat infertility associated with polycystic ovary syndrome.

2. The method of claim 1 , wherein the step of delivering energy is repeated to ablate the 3% to 20% of the tissue within the ovary.

3. The method of claim 1 , wherein the step of delivering energy is repeated through the single entry point in the ovary.

4. The method of claim 1 , wherein the delivered energy is radiofrequency energy.

5. The method of claim 4 , wherein the radiofrequency energy is delivered for about 20 seconds to about 60 seconds.

6. The method of claim 5 , wherein the therapeutic element comprises an elongate body having a straightened length and a radius of curvature, an active electrode, and a return electrode.

7. The method of claim 6 , wherein the straightened length ranges between about 5.0 and about 40 mm.

8. The method of claim 7 , wherein the radius of curvature ranges between about 3.0 mm and about 10 mm.

9. The method of claim 1 , further comprising maintaining the planar orientation of the therapeutic element relative to the ultrasound transducer during one or multiple ablation procedures on the ovary.

The method of claim 1 , further comprising coupling an alignment element coupled to the docking device to the ultrasound probe via an adapter removably coupled to the probe handle, the adapter comprising a receiving alignment channel having a geometry that allows engagement with the alignment element in a manner that maintains planar orientation and limits rotation of the docking device and therapeutic element relative to the ultrasound probe during one or multiple ablation procedures on the ovary.

11. A method for treating a patient having polycystic ovary syndrome, the method comprising:

advancing a docking device into a vaginal canal of the patient;

penetrating a vaginal wall and ovary of the patient via a distal tip of the docking device;

advancing a therapeutic element from the docking device into the ovary; and

delivering energy into tissue from inside the ovary using the therapeutic element positioned inside the ovary to ablate 3% to 20% of the tissue within the ovary to treat infertility associated with polycystic ovary syndrome.

12. The method of claim 11 , wherein penetrating the vaginal wall and ovary of the patient comprises penetrating the vaginal wall and ovary of the patient at a single entry point.

13. The method of claim 11 , wherein the delivered energy is radiofrequency energy.

14. The method of claim 11 , wherein the tissue within the ovary comprises a polycystic ovary or ovarian cyst.

15. The method of claim 11 , wherein delivering energy into the tissue within the ovary comprises delivering energy for 20 to 60 seconds.

16. The method of claim 11 , further comprising imaging the ovary via an ultrasound transducer in a field of view of the ultrasound transducer.

17. The method of claim 16 , wherein advancing the therapeutic element from the docking device into the ovary comprises advancing the therapeutic element from the docking device into the ovary in the field of view of the ultrasound transducer.

18. The method of claim 16 , further comprising maintaining an orientation of the therapeutic element relative to the ultrasound transducer during one or more additional deliveries of energy within the ovary to ablate the 3% to 20% of the tissue within the ovary.

19. The method of claim 11 , further comprising reorienting the docking device within a field of view of the ultrasound transducer within the ovary, and then delivering additional energy within the ovary via the therapeutic element for one or more additional deliveries of energy within the ovary to ablate the 3% to 20% of the tissue within the ovary.

20. The method of claim 11 , further comprising removably coupling the docking device to a handle of a transvaginal probe via an adapter removably coupled to the handle of the transvaginal probe.
