Abstract

The invention provides, in various embodiments, systems, devices and methods relating to ex-vivo organ care. In certain embodiments, the invention relates to maintaining an organ ex-vivo at near-physiologic conditions. The present application describes a method for using lactate measurement in the arterial and the venous blood lines of the Organ Care System Heart perfusion device to evaluate the: 1 ) The overall perfusion status of an isolated heart and 2 ) The metabolic status of an isolated heart and 3 ) the overall vascular patency of an isolated donor heart. This aspect of the present invention uses the property of myocardial cell's unique ability to produce/generate lactate when they are starved for oxygen and metabolize/utilize lactate for energy production when they are well perfused with oxygen.

Claims (71)

1. A system comprising:

an organ chamber assembly comprising:

a fluid conduit comprising:

a first interface configured to connect to an aorta of a heart; and

a second interface configured to connect to a pulmonary artery of the heart;

a first sensor configured to measure a first lactate value in the first interface; and

a second sensor configured to measure a second lactate value in the second interface; and

a processor configured to:

calculate a lactate differential based on the first lactate value and the second lactate value; and

correlate the lactate differential with a suitability of the heart for transplantation.

2. The system of claim 1 , wherein the organ chamber assembly is disposable.

3. The system of claim 1 , wherein the processor is configured to:

calculate the lactate differential at least once per hour.

4. The system of claim 1 , comprising a flow mode selector valve configured to:

control a flow of a perfusion fluid to the first interface and to the second interface.

5. The system of claim 1 , comprising:

a chassis; and

a reservoir mounted to the chassis, the reservoir in fluid communication with the organ chamber assembly and configured to hold a perfusion fluid.

6. The system of claim 1 , comprising:

a third sensor configured to measure an aortic pressure.

7. The system of claim 6 , wherein the processor is configured to:

receive an aortic pressure measurement from the third sensor; and

evaluate a coronary vascular patency of the heart using the aortic pressure measurement and at least one of the first lactate value or the second lactate value.

8. The system of claim 1 , wherein the processor is configured to:

calculate the lactate differential at a periodic interval.

9. The system of claim 8 , wherein the periodic interval is less than one hour.

10. The system of claim 9 , wherein the processor is configured to:

receive, over a total length of time that is more than one hour, the first lactate value and the second lactate value.

11. An apparatus comprising:

a first fluid channel configured to be coupled to an aorta of a heart;

a second fluid channel configured to be coupled to a pulmonary artery of the heart;

a first sensor configured to measure a first lactate level in the first fluid channel;

a second sensor configured to measure a second lactate level in the second fluid channel;

a processor coupled to the first sensor and to the second sensor; and

a memory coupled to the processor and including computer-readable instructions that, when executed by the processor, cause the processor to:

calculate a lactate differential at two different times using the first lactate level and the second lactate level.

12. The apparatus of claim 11 , wherein the first fluid channel and the second fluid channel are disposable.

13. The apparatus of claim 11 , wherein the computer-readable instructions cause the processor to:

calculate the lactate differential at least once per hour.

14. The apparatus of claim 11 , comprising a flow mode selector valve configured to:

control a flow of a perfusion fluid through the first fluid channel and through the second fluid channel.

15. The apparatus of claim 11 , comprising:

a chassis; and

a reservoir mounted to the chassis, the reservoir in fluid communication with at least one of the first fluid channel or the second fluid channel and configured to hold a perfusion fluid.

16. The apparatus of claim 11 , comprising:

a third sensor configured to measure an aortic pressure.

17. The apparatus of claim 16 , wherein the computer-readable instructions cause the processor to:

receive an aortic pressure measurement from the third sensor; and

evaluate a coronary vascular patency of the heart using the aortic pressure measurement and at least one of the first lactate value or the second lactate value.

18. The apparatus of claim 11 , wherein the computer-readable instructions cause the processor to:

calculate the lactate differential at a periodic interval.

19. The apparatus of claim 18 , wherein the periodic interval is less than one hour.

20. The apparatus of claim 19 , wherein the computer-readable instructions cause the processor to:

receive, over a total length of time that is more than one hour, the first lactate value and the second lactate value.

21. A method comprising:

placing an ex-vivo heart in an organ chamber assembly;

connecting an aorta of the ex-vivo heart to a first interface of a fluid conduit;

connecting a pulmonary artery of the ex-vivo heart to a second interface of the fluid conduit;

measuring a first lactate value using a first sensor located in the first interface;

measuring a second lactate value using a second sensor located in the second interface;

calculating a lactate differential based on the first lactate value and the second lactate value; and

correlating the lactate differential with a suitability of the ex-vivo heart for transplantation.

22. The method of claim 21 , comprising:

calculating the lactate differential at least once per hour.

23. The method of claim 21 , comprising:

measuring an aortic pressure using a third sensor.

24. The method of claim 23 , comprising:

evaluating a coronary vascular patency of the ex-vivo heart using the aortic pressure and at least one of the first lactate value or the second lactate value.

25. The method of claim 21 , comprising:

calculating the lactate differential at a periodic interval.

26. The method of claim 25 , wherein the periodic interval is less than one hour.
