Abstract

A method for intraoperatively planning and facilitating a revision arthroplasty procedure. The method includes capturing positions of a tracked probe as the tracked probe contacts an interface area between a bone and a primary implant component implanted on the bone, intraoperatively generating a virtual boundary corresponding to a portion of the interface area to be removed to detach the primary implant component from the bone using the positions of the tracked probe and without use of pre-operative medical imaging, facilitating removal of the primary implant component from the bone by providing a constraint on operation of a cutting tool while the cutting tool removes the portion of the interface area, the constraint based on a relationship between the cutting tool and the virtual boundary.

Claims (41)

1. A method for intraoperatively planning and facilitating a revision arthroplasty procedure, the method comprising:

capturing positions of a tracked probe as the tracked probe contacts an interface area between a bone and a primary implant component implanted on the bone;

intraoperatively generating a virtual boundary corresponding to a portion of the interface area to be removed to detach the primary implant component from the bone using the positions of the tracked probe and without use of pre-operative medical imaging;

facilitating removal of the primary implant component from the bone by providing a constraint on operation of a cutting tool while the cutting tool removes the portion of the interface area, the constraint based on a relationship between the cutting tool and the virtual boundary.

2. The method of claim 1 , wherein:

capturing the positions of the tracked probe comprises collecting points on a plurality of sides of the primary implant component; and

intraoperatively generating the virtual boundary comprises identifying one or more planar surfaces of the primary implant component based on the points on the plurality of sides of the primary implant component.

3. The method of claim 1 , further comprising providing a second constraint on operation of the cutting tool as the cutting tool is used to perform one or more additional cuts to prepare the bone to receive a revision implant.

4. The method of claim 1 , further comprising creating, after removal of the primary implant component from the bone, a model of a bone defect.

5. The method of claim 4 , comprising creating the model of the bone defect using a video camera.

6. The method of claim 4 , comprising creating the model of the bone defect using intraoperative imaging.

7. The method of claim 4 , further comprising updating the model of the bone defect using additional positions of the tracked probe.

8. The method of claim 4 , further comprising determining whether an augment is needed and selecting a revision implant based on the model of the bone defect.

9. The method of claim 4 , further comprising:

intraoperatively generating one or more additional virtual boundaries using the model of the bone defect; and

providing a second constraint on the cutting tool while the cutting tool performs one or more additional cuts to prepare the bone, the constraint based on a relationship between the cutting tool and the one or more additional virtual boundaries virtual boundaries.

10. The method of claim 1 , wherein the virtual boundary is a haptic boundary and wherein providing the constraint comprises providing haptic feedback to the cutting tool.

11. A surgical system comprising:

a robotic device;

a surgical tool coupled to the robotic device;

a probe;

a navigation system configured to track positions of the probe; and

a computing system programmed to:

obtain positional data from the navigation system indicative of positions of the probe as the probe contacts an interface area between a bone and a primary implant component implanted on the bone;

intraoperatively generate a virtual boundary corresponding to a portion of the interface area to be removed to detach the primary implant component from the bone using the positions of the probe and without using pre-operative medical imaging;

control the robotic device based on a relationship between a cutting tool and the virtual boundary to facilitate removal of the primary implant component using the surgical tool.

12. The surgical system of claim 11 , wherein the positions of the probe correspond to points on a plurality of sides of the primary implant component; and

the computing system is programed to generate the virtual boundary by identifying one or more planar surfaces of the primary implant component based on the points on the plurality of sides of the primary implant component.

13. The surgical system of claim 11 , wherein the computing system is further configured to control the robotic device based on a second relationship between the cutting tool and a second virtual boundary to facilitate preparation of the bone to receive a revision implant component.

14. The surgical system of claim 11 , wherein the computing system is further programmed to create, after the removal of the primary implant component from the bone, a model of a bone defect.

15. The surgical system of claim 14 , further comprising a video camera, wherein the computing system is programmed to create the model of the bone defect based on input from the video camera.

16. The surgical system of claim 14 , wherein the computing system is further programmed to update the model of the bone defect using additional positions of the probe as tracked by the navigation system.

17. The surgical system of claim 14 , wherein the computing system is further programmed to generate a plan for placing a revision implant and an augment or filler based on the model of the bone defect.

18. Non-transitory computer-readable memory storing programs executable by a processor to perform actions comprising:

obtaining positional data from a navigation system indicative of positions of a probe as the probe contacts an interface area between a bone and a primary implant component implanted on the bone;

intraoperatively generating a virtual boundary corresponding to a portion of the interface area to be removed to detach the primary implant component from the bone using the positions of the probe and without using pre-operative medical imaging; and

generating controls configured to provide a constraint on operation of a cutting tool while the cutting tool removes the portion of the interface area to remove the primary implant component from the bone.

19. The non-transitory computer-readable memory of claim 18 , wherein the actions further comprise:

intraoperatively creating a model of the bone after removal of the primary implant component from the bone; and

planning placement of a revision implant on the bone using the model.

The non-transitory computer-readable memory of claim 19 , wherein the actions further comprise generating controls configured to provide a second constraint on operation of the cutting tool while the cutting tool prepares the bone in accordance with the planned placement of the revision implant.
