Abstract

The system and method provide for electrocardiogram analysis and optimization of patient-customized cardiopulmonary resuscitation and therapy delivery. An external medical device includes a housing and a processor within the housing. The processor can be configured to receive an input signal for a patient receiving chest compressions and to select at least one filter mechanism and to apply the filter mechanism to the signal to at least substantially remove chest compression artifacts from the signal. A real time dynamic analysis of a cardiac rhythm is applied to adjust and integrate CPR prompting of a medical device. Real-time cardiac rhythm quality is facilitated using a rhythm assessment meter.

Claims (50)

1. A method performed by a defibrillator, the method comprising:

outputting an instruction to perform chest compressions on a patient during a time period extending from a first time to a second time;

identifying physiological parameter data corresponding to a physiological parameter of the patient detected after the first time and before the second time;

generating filtered physiological parameter data by removing, from the physiological parameter data, an artifact associated with the chest compressions using a comb filter, wherein a Q of the comb filter is greater than 4;

determining, based on the filtered physiological parameter data, that a heart rhythm of the patient is shockable; and

in response to determining that the heart rhythm is shockable, charging an energy storage module before the second time.

2. The method of claim 1 , wherein the physiological parameter comprises an electrocardiogram (ECG) or an electrical impedance.

3. The method of claim 1 , wherein identifying the physiological parameter data comprises identifying the physiological parameter detected between a third time and the second time, the third time occurring after the first time and before the second time,

wherein the method further comprises determining the third time based on a charging time of the defibrillator, an interval between the third time and the second time being at least as long as the charging time.

4. The method of claim 1 , further comprising:

determining a quality of the heart rhythm based on the filtered physiological parameter data, and

determining the time period based on the quality of the heart rhythm.

5. The method of claim 1 , wherein determining that the heart rhythm of the patient is shockable comprises:

determining a shock index based on the filtered physiological parameter data; and

determining that the shock index exceeds a threshold.

6. The method of claim 5 , wherein determining the shock index based on the filtered physiological parameter data comprises determining a similarity between the filtered physiological parameter data and ventricular fibrillation (VF); or

wherein determining the shock index comprises:

determining a median frequency of the physiological parameter data; and

determining the shock index based on the median frequency.

7. The method of claim 1 , the instruction being a first instruction, the method further comprising:

outputting a second instruction to administer a defibrillation shock to the patient;

in response to outputting the second instruction, receiving an input; and

discharging, by the energy storage module, the energy to the patient after the second time in response to receiving the input.

8. A medical device comprising:

a processor configured to perform the method of claim 7 ;

a measurement circuit configured to generate electrocardiogram (ECG) data by detecting an ECG signal of the patient, the physiological parameter data comprising the ECG data;

the energy storage module comprising a capacitor configured to store energy; and

a power source configured to charge the capacitor.

The medical device of claim 8 , the ECG data comprising a first portion and a second portion, the first portion corresponding to a first portion of the ECG signal detected after the first time and before the second time, the filtered physiological parameter data comprising filtered ECG data, wherein determining, based on the filtered physiological parameter data, that the heart rhythm of the patient is shockable comprises:

determining based on a first portion of the filtered ECG data, a first shock index;

determining that a heart rhythm of the patient is indeterminate by determining that the first shock index is greater than a first threshold and less than a second threshold; and

in response to determining that the heart rhythm of the patient is indeterminate, identifying the second portion of the ECG data;

determining, based on a second portion of the filtered ECG data corresponding to the second portion of the ECG data, a second shock index; and

determining that the second shock index is greater than the first threshold and greater than the second threshold.

10. The medical device of claim 9 , wherein the processor is further configured to:

determine, at the first time, that greater than a threshold amount of the chest compressions have been performed on the patient.

11. The medical device of claim 8 , wherein charging the energy storage module before the second time comprises causing the power source to charge the capacitor before the second time.

12. The method of claim 1 , wherein the chest compressions are administered by a mechanical chest compression device.

The method of claim 1 , wherein the comb filter comprises a first band configured to remove a first component of the physiological parameter data corresponding to a frequency of the chest compressions and second components of the physiological parameter respectively corresponding to twelve harmonics of the frequency.

The method of claim 1 , wherein generating the filtered physiological parameter data by removing, from the physiological parameter data, the artifact associated with the chest compressions using the comb filter is delayed less than one second from identifying the physiological parameter data corresponding to the physiological parameter of the patient.

15. The method of claim 1 , further comprising:

in response to charging the energy storage module, discharging, at the second time, the energy to the patient without a Cardiopulmonary Resuscitation (CPR) pause.

16. The method of claim 1 , wherein determining that the heart rhythm of the patient is shockable comprises:

determining a shock index by:

determining a median frequency of the filtered physiological parameter data; and

determining a similarity between the filtered physiological parameter data and ventricular fibrillation (VF); and

comparing the shock index to a threshold.

17. The method of claim 16 , further comprising:

determining whether a previous shock has been administered to the patient; and

determining the threshold by analyzing whether the previous shock has been administered to the patient.
