Abstract

An example method includes obtaining, a virtual model of a portion of an anatomy of a patient obtained from a virtual surgical plan for an orthopedic joint repair surgical procedure to attach a prosthetic to the anatomy; identifying, based on data obtained by one or more sensors, positions of one or more physical markers positioned relative to the anatomy of the patient; and registering, based on the identified positions, the virtual model of the portion of the anatomy with a corresponding observed portion of the anatomy.

Claims (100)

1. A method comprising:

obtaining, a virtual model of a portion of an anatomy of a patient obtained from a virtual surgical plan for an orthopedic joint repair surgical procedure to attach a prosthetic to the anatomy;

identifying, by one or more processors of a head-mounted visualization device and based on data obtained by one or more optical sensors of the head-mounted visualization device, positions of one or more optical three-dimensional physical markers attached to the anatomy of the patient;

identifying, by the one or more processors, positions of one or more virtual markers on the anatomy of the patient, wherein identifying a position of a particular virtual marker the one or more virtual markers comprises:

receiving user input to virtually position the particular virtual marker on a surface of the virtual model of an observed portion of the anatomy within a corresponding region of interest on a surface of a portion of the anatomy in direct line of sight of a wearer of the head-mounted visualization device; and

determining the position of the particular virtual marker based on the received user input;

registering, by the one or more processors and based on the identified positions of the one or more physical markers and the identified positions of the one or more virtual markers, the virtual model of the portion of the anatomy with a corresponding observed portion of the anatomy; and

displaying, via the visualization device and overlaid on the portion of the anatomy, a virtual guide that guides at least one of preparation of the anatomy for attachment of the prosthetic or attachment of the prosthetic to the anatomy, wherein a position of the virtual guide is determined based on the registration.

2. The method of claim 1 , wherein registering the virtual model comprises initially registering the virtual model.

3. The method of claim 2 , wherein registering the virtual model comprises re-registering the virtual model.

4. The method of claim 3 , wherein re-registering the virtual model comprises:

responsive to determining that registration of the virtual model has been lost, re-registering the virtual model based on the identified positions.

5. The method of claim 1 , wherein registering the virtual model of the portion of the anatomy with the corresponding observed portion of the anatomy comprises:

generating, based on an alignment between the virtual model of the portion of the anatomy with a corresponding observed portion of the anatomy, a transformation matrix between the virtual model of the portion of the anatomy and the corresponding observed portion of the anatomy, wherein the transformation matrix provides a coordinate system for translating virtual guides indicated by the virtual surgical plan to the observed portion of the anatomy.

6. The method of claim 5 , further comprising determining the alignment by at least:

adjusting an orientation of the virtual model so that a virtual surface normal vector associated with the virtual marker is aligned with a real surface normal vector associated with the corresponding region of interest.

7. The method of claim 6 , wherein registering the virtual model comprises:

performing an initialization procedure; and

performing an optimization procedure.

8. The method of claim 7 , wherein performing the initialization procedure comprises determining the alignment, and wherein performing the optimization procedure comprises generating the transformation matrix.

9. The method of claim 6 , wherein the region of interest is an anatomical landmark of the portion of the anatomy, wherein the anatomical landmark is a center region of a glenoid or a center region of a humeral head.

10. The method of claim 6 , wherein receiving the user input comprises one or more of:

receiving motion data from one or more sensors of the visualization device;

receiving audio data indicating one or more voice commands; and

receiving data indicating performance of one or more hand gestures.

11. The method of claim 1 , further comprising:

displaying, via a visualization device and overlaid on the portion of the anatomy viewable via the visualization device, the virtual model of the portion of the anatomy.

12. The method of claim 1 , further comprising:

outputting an indication that the physical markers may be removed.

13. The method of claim 12 , wherein outputting the indication that the physical markers may be removed comprises:

outputting the indication in response to determining that registration can be maintained without the physical markers remaining present.

14. The method of claim 12 , wherein outputting the indication that the physical markers may be removed comprises:

outputting the indication in response to determining that registration is no longer needed.

15. The method of claim 1 , wherein the physical markers comprise one or more passive physical markers.

16. The method of claim 15 , wherein at least one of the passive physical markers comprises a sticker.

17. The method of claim 1 , further comprising:

obtaining, a virtual model of an instrument;

identifying, based on data obtained by the one or more optical sensors, positions of one or more second optical physical markers attached to the instrument; and

registering, based on the identified positions of the one or more second optical physical markers, the virtual model of the instrument with a corresponding observed portion of the instrument.

18. The method of claim 1 , further comprising:

determining a confidence of the registration of the virtual model.

19. The method of claim 18 , wherein determining the confidence of the registration comprises:

determining a confidence distance that represents a maximum distance between a point on the virtual model and a corresponding point on the observed portion of the anatomy.

20. The method of claim 19 , further comprising:

determining that registration of the virtual model has been lost in response to determining that the confidence distance is greater than a threshold confidence distance; and

responsive to determining that registration of the virtual model has been lost, registering the virtual model.

21. The method of claim 20 , wherein the threshold confidence distance is the same for all work steps of the orthopedic joint repair surgical procedure.

22. The method of claim 20 , further comprising:

selecting a first threshold confidence distance for a first work step of the orthopedic joint repair surgical procedure; and

selecting a second threshold confidence distance for a second work step of the orthopedic joint repair surgical procedure.

23. The method of claim 21 , further comprising:

outputting a representation of the confidence of the registration.

24. The method of claim 23 , wherein outputting the representation of the confidence comprises one or more of:

displaying a numerical value of the confidence distance; and

displaying a graphical representation of the confidence distance.

25. The method of claim 1 , wherein the anatomy is bone.

26. A mixed reality system comprising:

a memory that stores at least a portion of a virtual surgical plan for an orthopedic joint repair surgical procedure to attach a prosthetic to an anatomy of a patient;

one or more optical three-dimensional physical markers;

a head-mounted visualization device comprising:

one or more optical sensors; and

one or more processors implemented configured to:

obtain, a virtual model of a portion of an anatomy of a patient obtained from a virtual surgical plan for an orthopedic joint repair surgical procedure to attach a prosthetic to the anatomy;

identify, based on data obtained by one or more optical sensors, positions of one or more physical markers attached to the anatomy of the patient;

identify positions of one or more virtual markers on the anatomy of the patient, wherein, to identify a position of a particular virtual marker the one or more virtual markers, the one or more processors are configured to:

receive user input to virtually position the particular virtual marker on a surface of the virtual model of an observed portion of the anatomy within a corresponding region of interest on a surface of a portion of the anatomy in direct line of sight of a wearer of the head-mounted visualization device; and

determine the position of the particular virtual marker based on the received user input;

register, based on the identified positions of the one or more physical markers and the identified positions of the one or more virtual markers, the virtual model of the portion of the anatomy with a corresponding observed portion of the anatomy; and

display, overlaid on the portion of the anatomy, a virtual guide that guides at least one of preparation of the anatomy for attachment of the prosthetic or attachment of the prosthetic to the anatomy, wherein a position of the virtual guide is determined based on the registration.

27. The mixed reality system of claim 26 , wherein, to register the virtual model, the one or more processors are configured to:

initially registering the virtual model at a first time; and

re-register the virtual model at a second time that is after the first time.

28. The mixed reality system of claim 26 , wherein the one or more processors are further configured to:

output an indication that the physical markers may be removed.

29. The mixed reality system of claim 28 , wherein, to output the indication that the physical markers may be removed, the one or more processors are configured to:

output the indication in response to determining that registration can be maintained without the physical markers remaining present.

30. The mixed reality system of claim 28 , wherein, to output the indication that the physical markers may be removed, the one or more processors are configured to:

output the indication in response to determining that registration is no longer needed.

31. The mixed reality system of claim 26 , wherein the physical markers comprise one or more passive physical markers.

32. The mixed reality system of claim 26 , wherein the physical markers comprise one or more active physical markers.

33. The mixed reality system of claim 32 , wherein at least one of the active physical markers comprises an electromagnetic marker.

34. The mixed reality system of claim 26 , wherein the one or more processors are further configured to:

obtain, a virtual model of an instrument;

identify, based on data obtained by the one or more sensors, positions of one or more second optical physical markers attached to the instrument; and

register, based on the identified positions of the one or more second optical physical markers, the virtual model of the instrument with a corresponding observed portion of the instrument.

35. The mixed reality system of claim 26 , wherein the one or more processors are further configured to:

determine a confidence of the registration of the virtual model.

36. The mixed reality system of claim 35 , wherein, to determine the confidence of the registration, the one or more processors are configured to:

determine a confidence distance that represents a maximum distance between a point on the virtual model and a corresponding point on the observed portion of the anatomy.

37. The mixed reality system of claim 36 , wherein the one or more processors are further configured to:

determine that registration of the virtual model has been lost in response to determining that the confidence distance is greater than a threshold confidence distance; and

responsive to determining that registration of the virtual model has been lost, register the virtual model.

38. A computer-readable storage medium storing instructions that, when executed, cause one or more processors of a head-mounted visualization device of a mixed reality system to:

obtain, a virtual model of a portion of an anatomy of a patient obtained from a virtual surgical plan for an orthopedic joint repair surgical procedure to attach a prosthetic to the anatomy;

identify, based on data obtained by one or more sensors, positions of one or more physical markers attached to the anatomy of the patient;

identify positions of one or more virtual markers on the anatomy of the patient, wherein the instructions that cause the one or more processors to identify a position of a particular virtual marker the one or more virtual markers comprise instructions that cause the one or more processors to:

receive user input to virtually position the particular virtual marker on a surface of the virtual model of an observed portion of the anatomy within a corresponding region of interest on a surface of a portion of the anatomy in direct line of sight of a wearer of the head-mounted visualization device; and

determine the position of the particular virtual marker based on the received user input;

register, based on the identified positions of the one or more physical markers and the identified positions of the one or more virtual markers, the virtual model of the portion of the anatomy with a corresponding observed portion of the anatomy; and

display, via the visualization device and overlaid on the portion of the anatomy, a virtual guide that guides at least one of preparation of the anatomy for attachment of the prosthetic or attachment of the prosthetic to the anatomy, wherein a position of the virtual guide is determined based on the registration.
