Abstract

A catheter has a cannulated guiding rail extending therethrough, an advance segment of the rail extending at least about 10 cm beyond the distal end of the catheter. The catheter and rail are advanced over a guidewire until the distal end of the advance segment is at a target vascular site. The catheter is thereafter advanced along the rail to the target vascular site.

Claims (23)

1. A method of advancing a catheter to a target vascular site, comprising the steps of:

providing a catheter having a lumen with a rail extending therethrough, the catheter having a catheter distal end and the rail having a rail distal end;

with the rail distal end at least about 10 cm distal to the catheter distal end, advancing the rail distal end to the target vascular site; and thereafter

advancing the catheter along the rail to the target vascular site.

2. A method as in claim 1 , wherein the advancing step is accomplished while the rail distal end is at least about 15 cm distal to the catheter distal end.

3. A method as in claim 1 , further comprising the step of unlocking the catheter from the rail prior to the advancing the catheter along the rail step.

4. A method as in claim 1 , wherein the advancing the rail distal end step comprises advancing the rail distal end from the vena cava through the tricuspid valve before advancing the catheter along the rail.

5. A method as in claim 4 , wherein the advancing the catheter step comprises advancing the catheter distal end from the vena cava through the tricuspid and pulmonary valves of the heart into the central pulmonary artery over the rail.

6. A method as in claim 1 , wherein the advancing the rail distal end step comprises advancing the rail distal end from the vena cava through the tricuspid and pulmonary valves of the heart into the central pulmonary artery while the distal end of the catheter remains in the vena cava.

7. A method as in claim 1 , wherein the advancing the catheter step is accomplished over a guidewire.

8. A method as in claim 7 , wherein the advancing the catheter step is accomplished with the guidewire extending through a cannulation in the rail.

9. A method as in claim 7 , wherein the advancing the catheter step is accomplished with the guidewire extending through the catheter.

10. A method as in claim 1 , wherein the catheter is at least about 8 French.

11. A method as in claim 10 , wherein the catheter is at least about 24 French.

12. A method as in claim 11 , wherein the rail substantially fills the lumen.

13. A method as in claim 1 , wherein the advancing the rail distal end step comprises advancing the rail distal end through at least one valve before advancing the catheter along the rail and through the valve.

14. A method as in claim 1 , wherein the advancing the rail distal end step comprises advancing the rail distal end through a vascular obstruction before advancing the catheter along the rail and through the obstruction.

15. A method as in claim 1 , wherein the advancing the rail distal end step comprises advancing the rail distal end through a tissue aperture before advancing the catheter along the rail and through the aperture.

16. A method as in claim 1 , further comprising the step of removing the rail following the advancing the catheter step.

17. A method as in claim 16 , further comprising the step of advancing a clot evacuation catheter through the lumen to the target vascular site.

18. A method as in claim 17 , further comprising the step of applying vacuum to the clot evacuation catheter.

19. A method as in claim 18 , further comprising the step of advancing a thrombus engagement tool through the clot evacuation catheter.

20. A method as in claim 19 , further comprising the step of manually rotating the thrombus engagement tool.
