Abstract

Systems and methods can involve wedge dissectors attached to strips in turn attached to medical balloons, for forming serrations within vascular wall tissue for angioplasty as well as drug delivery.

Claims (26)

1. A method of creating serrations at a treatment site in a vessel, comprising: providing a serration balloon comprising a strip including a plurality of wedge dissectors spaced apart along a surface of the strip, the strip extending longitudinally along an outer surface of the serration balloon, each wedge dissector comprising a radially outward facing surface and lateral surfaces; expanding the serration balloon at the site such that the radially outward facing surface of the plurality of wedge dissectors directly contact tissue of the intima layer of the vessel wall creating nucleation sites for a cleavage; and continuing to expand the serration balloon such that radially outward facing surface of the plurality of wedge dissectors no longer contact tissue, but the lateral surfaces of the wedge dissector contact tissue to expand the cleavage plane along the entire length of the strip.

2. The method of claim 1 , wherein the cleavage plane has a depth of between about 0.3 mm and about 1.5 mm.

3. The method of claim 1 , wherein the cleavage plane has a depth of between about 0.5 mm and about 1.2 mm.

4. The method of claim 1 , wherein cleaving stress overcomes the need for penetration after the initial penetration of the plurality of wedge dissectors.

5. The method of claim 1 , further comprising penetrating thick calcium or fibrotic laden tissue.

6. The method of claim 1 , wherein the radially outward facing surfaces of the plurality of wedge dissectors comprise an angle less than 15 degrees for penetrating deeper into the vessel for nucleation sites for crack propagation.

7. The method of claim 1 , wherein the radially outward facing surfaces of the plurality of wedge dissectors are configured for penetrating circumferential plaques.

8. The method of claim 1 , further comprising low pressure dilation less than 5 atm.

9. The method of claim 1 , further comprising drug delivery.

10. The method of claim 1 , where the width of the wedge dissectors W U are between 20% and 40% of the base width W B .

11. A method of creating serrations at a treatment site in a vessel, comprising: providing a serration balloon comprising a strip including a plurality of wedge dissectors spaced apart along a surface of the strip, the strip extending along an outer surface of the serration balloon, each wedge dissector comprising a outward facing surface- and lateral surfaces; expanding the serration balloon at the site such that the outward facing surface of the plurality of wedge dissectors directly contact tissue of the intima layer of the vessel wall creating nucleation sites for a cleavage plane; and continuing to expand the serration balloon for a fissuring event along the intra-luminal surface such that outward facing surface of the plurality of wedge dissectors no longer contact tissue, but the lateral surfaces of the wedge dissector contact tissue to expand the cleavage plane along the intra-luminal surface and along the length of the strip.

12. The method of claim 11 , wherein crack generation is aided by the lateral surfaces of the wedge dissector and the strain produced by expanding the serration balloon.

13. The method of claim 11 , wherein the radially outward facing surfaces of the plurality of wedge dissectors comprise an angle less than 15 degrees.

14. The method of claim 11 , further comprising low pressure dilation less than 4 atm and maintaining the pressure for 30 seconds.

15. The method of claim 11 , further comprising delivering a drug into media layer of the vessel wall.

16. The method of claim 11 , where the width of the wedge dissectors W U are between 20% and 40% of the base width W B .

17. A method of creating serrations at a treatment site in a vessel, comprising:

providing a serration balloon comprising a strip including a plurality of wedge dissectors spaced apart along a surface of the strip, the strip extending along an outer surface of the serration balloon, each wedge dissector comprising an outward facing surface and lateral surfaces;

expanding the serration balloon at the site such that the outward facing surfaces of the plurality of wedge dissectors directly contact tissue of the intima layer of the vessel wall creating nucleation sites for a cleavage plane; and

continuing to expand the serration balloon such that outward facing surfaces of the plurality of wedge dissectors no longer contact tissue, but the lateral surfaces of the wedge dissector contact tissue to expand the cleavage plane greater than the length of the strip.

18. The method of claim 17 , wherein the depth of penetration aids in reducing the pressure needed to initiate a crack and allow for increase in crack opening displacement.

19. The method of claim 17 , wherein a crack migrates into spaces beyond the outward facing surfaces creating the cleavage plane into a media layer of the vessel wall.

20. The method of claim 17 , further comprising low pressure dilation less than 5 atm.

21. The method of claim 17 , wherein the outward facing surfaces of the plurality of wedge dissectors comprise an angle greater than 20 degrees for cracking hard, calcium rich vessels.

22. The method of claim 17 , further comprising delivering one or more pharmaceutical agents.

23. The method of claim 17 , where the width of the wedge dissectors W U are between 20% and 40% of the base width W B .
