Abstract

Implementations described herein may include a system for manipulating implants coupled by a cord. The system may include a tensioner and a counter tensioner. The tensioner may include a nose assembly and a cord lock assembly for applying tension to the cord. The nose assembly may include a piston having a lumen extending therethrough for receiving the cord and a spring positionable in contact with an indicator region of the piston. The counter may be releasably coupleable to a head of an implant at a proximal end thereof and may guide the cord to a port proximate the distal end thereof, the port for receiving the nose assembly. The counter tensioner may enable translation of the implant relative to another implant implanted in an adjacent or nearby vertebrae. The indicator region of the piston may be visible through a window in a tensioner main body and may indicate cord tension when the nose assembly of the tensioner is coupled to the port of the counter tensioner and engaged with the cord.

Claims (25)

1. A system for manipulating an implant connected to a cord, the system comprising:

a counter tensioner to guide the cord from a tensioner into a head of an implant, comprising:

an elongate body with a proximal end and a distal end, the distal end including an implant interface that is releasably coupleable to the head of the implant thereby enabling translation of the implant relative to another implant implanted in an adjacent or nearby vertebrae; and

a handle at the proximal end of the elongate body, the handle configured to secure the implant interface to a proximal end of the head of the implant, wherein the handle includes a lock actuated relative to the handle to engage a locking mechanism configured to secure the implant interface on the distal end of the head of the implant, and wherein the locking mechanism includes a lock cylinder coupled to the lock in the handle.

2. The system of claim 1 , wherein the counter tensioner has a lumen disposed within the elongate body and extending from the proximal end to the distal end thereof, and wherein the lumen is capable of delivering a set screw engageable with the head of the implant to fix the cord.

The system of claim 2 , wherein the proximal end of the elongate body includes a tensioner port to receive a nose assembly of the tensioner, wherein the handle is coupled to the proximal end adjacent to the tensioner port, and wherein the tensioner port is disposed at an angle that is not parallel to a longitudinal axis of the lumen.

4. The system of claim 1 , the counter tensioner comprising:

an inner sleeve couplable to the head of the implant, wherein the inner sleeve includes a plurality of outwardly biased arms at a distal end thereof that are engageable with cooperating features disposed on the head of the implant when an outer sleeve of the counter tensioner is translated distally over the inner sleeve of the counter tensioner and at least partially over the head of the implant to urge the plurality of outwardly biased arms into secure engagement with the cooperating features disposed on the head of the implant.

5. The system of claim 1 , the counter tensioner comprising:

a fulcrum pivotably coupled to a distal portion of an outer sleeve, wherein the fulcrum articulates from an undeployed position to a deployed position to change an angle of the cord as the cord extends towards the tensioner from the distal end of the counter tensioner.

6. The system of claim 1 , wherein the locking mechanism includes opposing locking extensions extending along a longitudinal length of the elongate body between the handle and the implant interface.

7. The system of claim 6 , wherein the implant interface includes opposing pivot locks disposed on lateral sides of the distal end of the elongate body.

8. The system of claim 7 , wherein the locking extensions include extension wedges at distal ends that interface with the pivot locks to engage the head of the implant.

9. The system of claim 8 , wherein the locking extensions are disposed within channels running on opposite sides along the longitudinal length of the elongate body.

10. The system of claim 7 , wherein each pivot lock of the opposing pivot locks includes a spring recess and a spring to bias the pivot lock into an unlocked position.

11. The system of claim 6 , wherein the distal end of the elongate body includes a cord fulcrum configured to transition a cord out of a cord channel running along a longitudinal length of the elongate body and into a cord guide running transverse to the cord channel.

12. The system of claim 11 , wherein the cord guide includes a pair of radiused recesses on opposing edges of the distal end of the elongate body that operate to guide the cord into the head of the implant.

13. The system of claim 1 , wherein the lock cylinder includes an internally threaded surface to threadably engage an externally threaded internal cylinder that linearly translates along a longitudinal axis of the elongate body.

14. The system of claim 13 , wherein locking extensions engage the externally threaded internal cylinder to transmit the linear translation of the externally threaded internal cylinder to the locking extensions.

15. The system of claim 1 ,

the tensioner having a proximal end with a series of indicators to illustrate an amount of force applied by the tensioner on the cord.

16. The system of claim 15 , wherein the series of indicators are positioned proximate to a nose assembly of the tensioner through which the cord exits prior to being received within the head of the implant.

17. The system of claim 15 , wherein the cord passes through the proximal end of the tensioner proximate to the series of indicators.

18. The system of claim 15 , the tensioner further comprising a first grip actuatable with respect to a second grip, wherein the cord advances through the tensioner and is tensioned within the head of the implant and the distal end of the counter tensioner following actuation of the first grip relative to the second grip.

19. The system of claim 18 , wherein actuation of the first grip relative to the second grip includes pivoting of the first grip about an axis through a pin.
