Abstract

A method of administering insulin includes receiving blood glucose measurements of a patient at a data processing device from a glucometer. The blood glucose measurements are separated by a time interval. The method also includes receiving patient information at the data processing device and selecting a subcutaneous insulin treatment from a collection of subcutaneous insulin treatments. The selection is based on the blood glucose measurements and the patient information. The selection includes one or more of a subcutaneous standard program, a subcutaneous program without meal boluses, a meal-by-meal subcutaneous program without carbohydrate counting, a meal-by-meal subcutaneous program with carbohydrate counting, and a subcutaneous program for non-diabetic patients. The method also includes executing, using the data processing device, the selected subcutaneous insulin treatment.

Claims (32)

1. A system comprising:

data processing hardware; and

memory hardware in communication with the data processing hardware and storing instructions that when executed on the data processing hardware cause the data processing hardware to perform operations comprising:

receiving a glucose measurement of a patient on a current day;

determining a glucose type of the received glucose measurement;

when the determined glucose type comprises a pre-breakfast glucose measurement:

obtaining a previous Carbohydrate-to-Insulin Ratio (CIR) for an immediately previous dinner meal consumed by the patient on a day immediately preceding the current day;

obtaining a previous bedtime glucose measurement of the patient on the day immediately preceding the current day;

adjusting the previous CIR for the immediately previous dinner meal consumed by the patient based on the obtained previous bedtime glucose measurement; and

determining a recommended breakfast bolus based on the adjusted previous CIR; and

transmitting to a patient device remotely located from the data processing hardware and in communication with the data processing hardware via a network, the recommended breakfast bolus at the time of day associated with the determined glucose type, the recommended breakfast bolus when received by the patient device causing the patient device to display a number of units of insulin for the patient to administer using the recommended breakfast bolus.

2. The system of claim 1 , wherein adjusting the previous CIR for the immediately previous dinner meal consumed by the patient comprises:

determining an adjustment factor based on the obtained previous bedtime blucose measurement; and

adjusting the previous CIR for the immediately previous dinner meal consumed by the patient by dividing the previous CIR by the adjustment factor.

3. The system of claim 2 , wherein determining the adjustment factor comprises:

determining whether the obtained previous bedtime glucose measurement is within one of a plurality of target ranges of values; and

when the obtained previous bedtime glucose measurement is within one of the plurality of target ranges of values, setting the adjustment factor to a target adjustment factor associated with the corresponding target range of values that includes the obtained previous bedtime glucose measurement.

4. The system of claim 1 , wherein determining the recommended breakfast bolus comprises:

receiving a number of carbohydrates to be consumed by the patient for breakfast; and

dividing the received number of carbohydrates by the adjusted previous CIR.

5. The system of claim 1 , wherein the operations further comprise determining a pre-meal correction bolus by calculating:

CB=(BG−BG Target )/CF,

wherein CB is the pre-meal correction bolus, BG is the received glucose measurement, BG Target is a target glucose value of the patient, and CF is a correction factor.

6. The system of claim 1 , wherein the operations further comprise:

selecting a governing glucose measurement as a lesser one of a previous midsleep glucose measurement or the pre-breakfast glucose measurement;

determining a basal adjustment factor for adjusting a current day's recommended basal dose based on the selected governing glucose measurement;

obtaining a previous day's recommended basal dose; and

determining the current day's recommended basal dose by multiplying the adjustment factor times the previous day's bedtime recommended basal dose.

7. The system of claim 6 , wherein the current day's recommended basal dose corresponds to an insulin dose of long-acting insulin to be administered to the patient at a configurable frequency of one, two, or three times per day.

8. The system of claim 1 , wherein receiving the glucose measurement of the patient comprises receiving the glucose measurement from a continuous glucose monitoring system associated with the patient.

9. The system of claim 1 , wherein receiving the glucose measurement of the patient comprises receiving the glucose measurement from a glucometer associated with the patient.

10. The system of claim 1 , wherein the recommended breakfast bolus corresponds to an insulin dose of rapid-acting insulin to be administered to the patient at the time of day associated with the determined glucose type to bring and maintain the patient's glucose level into a target range.
