Abstract

Embodiments of the invention provide swallowable devices, preparations and methods for delivering drugs and other therapeutic agents within the GI tract. Many embodiments provide a swallowable device for delivering the agents. Particular embodiments provide a swallowable device such as a capsule for delivering drugs into the intestinal wall or other GI lumen. Embodiments also provide various drug preparations that are configured to be contained within the capsule, advanced from the capsule into the intestinal wall and degrade to release the drug into the bloodstream to produce a therapeutic effect. The preparation can be operably coupled to delivery means having a first configuration where the preparation is contained in the capsule and a second configuration where the preparation is advanced out of the capsule into the intestinal wall. Embodiments of the invention are particularly useful for the delivery of drugs which are poorly absorbed, tolerated and/or degraded within the GI tract.

Claims (27)

1. A therapeutic preparation comprising somatostatin or somatostatin analogue, the preparation shaped as a solid tissue penetrating member, the preparation configured to be contained in a swallowable capsule to penetrate and be inserted into an intestinal wall after oral ingestion, wherein upon insertion, the preparation releases somatostatin or somatostatin analogue into the blood stream from the intestinal wall.

The therapeutic preparation of claim 1 , wherein the preparation releases the somatostatin or somatostatin analogue into the blood stream from the intestinal wall to achieve a C max in a shorter time period than a time period to achieve a C max for an extravascularly injected dose of somatostatin or somatostatin analogue.

3. The therapeutic preparation of claim 1 , wherein the C max achieved by delivering the preparation by insertion into the intestinal wall is substantially greater than a C max achieved when the preparation is delivered orally without insertion into the intestinal wall.

4. The therapeutic preparation of claim 1 , wherein a t max for the somatostatin or somatostatin analogue released from the therapeutic preparation is about 80%, about 50%, or about 10% of a t max for the extravascularly injected dose of somatostatin or somatostatin analogue.

5. The therapeutic preparation of claim 1 , wherein the extravascular injection is a subcutaneous injection or an intramuscular injection.

The therapeutic preparation of claim 1 , wherein the preparation is adapted to be operably coupled to delivery means having a first configuration and a second configuration, the preparation being contained within the capsule in the first configuration and advanced out of the capsule and into the intestinal wall in the second configuration.

7. The therapeutic preparation of claim 1 , wherein the preparation comprises a biodegradable material which degrades within the intestinal wall to release somatostatin or somatostatin analogue into the blood stream.

8. The therapeutic preparation of claim 1 , wherein the preparation comprises at least one pharmaceutical excipient.

9. The therapeutic preparation of claim 1 , wherein a weight percent of somatostatin or somatostatin analogue in the tissue penetrating member comprises between about 0.3 to 8%.

10. The therapeutic preparation of claim 1 , wherein a dose of somatostatin or somatostatin analogue in the preparation is in a range from about 50 to 600 mg.

11. The therapeutic preparation of claim 1 , wherein the somatostatin analogue is octreotide.

12. A method for delivering somatostatin or somatostatin analogue into a wall of a gastrointestinal tract, the method comprising:

configuring a drug delivery device to comprise a capsule, an actuator having a first configuration and a second configuration, and a solid somatostatin or somatostatin analogue preparation operably coupled to the actuator, the solid somatostatin or somatostatin analogue preparation comprising a therapeutically effective dose of somatostatin or somatostatin analogue, the solid somatostatin or somatostatin analogue preparation being contained within the capsule in the first configuration and advanced into the wall of the gastrointestinal tract in the second configuration, wherein the solid somatostatin or somatostatin analogue preparation has a shape configured for penetration of the wall of the gastrointestinal tract; and

providing the ingestible device to be swallowed by a patient.

13. The method of claim 12 , wherein a weight percent of somatostatin compound in the preparation is in a range between about 0.3 to 8%.

14. The method of claim 12 , wherein a dose of somatostatin compound in the somatostatin preparation is in a range from about 50 to 600 mg.

15. The method of claim 12 , wherein the somatostatin analogue is octreotide.

16. The method of claim 12 , wherein the somatostatin or somatostatin analogue preparation comprises at least one pharmaceutical excipient.

17. The method of claim 12 , wherein the somatostatin or somatostatin analogue preparation treats a somatostatin responsive condition comprising acromegaly, diarrhea, diarrhea associated with carcinoid syndrome, or diarrhea associated with vasoactive intestinal peptide-secreting tumors.

18. A method for delivering somatostatin or somatostatin analogue into a wall of a gastrointestinal tract, the method comprising:

providing a capsule, an actuator having a first configuration and a second configuration, and a solid somatostatin or somatostatin analogue preparation that (i) comprises a therapeutically effective dose of somatostatin or somatostatin analogue, (ii) is operably coupled to the actuator, and (iii) is contained within the capsule in the first configuration and is advanced into a wall of the gastrointestinal tract in the second configuration; and

providing the capsule to be swallowed.

19. The method of claim 18 , wherein a weight percent of somatostatin compound in the preparation is in a range between about 0.3 to 8%.

20. The method of claim 18 , wherein a dose of somatostatin compound in the somatostatin preparation is in a range from about 50 to 600 mg.

21. The method of claim 18 , wherein the somatostatin analogue is octreotide.

22. The method of claim 18 , wherein the somatostatin or somatostatin analogue preparation comprises at least one pharmaceutical excipient.

23. The method of claim 18 , wherein the somatostatin or somatostatin analogue preparation treats a somatostatin responsive condition comprising acromegaly, diarrhea, or diarrhea associated with carcinoid syndrome, or diarrhea associated with vasoactive intestinal peptide-secreting tumors.
