Abstract

Various embodiments disclosed relate to drug-coated balloon catheters for treating strictures in body lumens and methods of using the same. A drug-coated balloon catheter for delivering a therapeutic agent to a target site of a body lumen stricture includes an elongated balloon having a main diameter. The balloon catheter includes a coating layer overlying an exterior surface of the balloon. The coating layer includes one or more water-soluble additives and an initial drug load of a therapeutic agent.

Claims (58)

1. A drug-coated balloon catheter comprising:

an elongated balloon having a nominal diameter of 4 mm to 50 mm and a length of 20 mm to 160 mm; and

a coating layer overlaying an exterior surface of the balloon, the coating layer comprising

an initial drug load of a therapeutic agent, the therapeutic agent comprising paclitaxel, a paclitaxel analogue, docetaxel, a docetaxel analogue, taxol, a taxol analogue, rapamycin, a rapamycin analogue, sirolimus, a sirolimus analogue, everolimus, an everolimus analogue, tacrolimus, a tacrolimus analogue, mammalian target of rapamycin (mTOR) inhibitors, an mTOR inhibitor analogue, or a combination thereof, and

one or more additives; wherein a ratio of the therapeutic agent to the one or more additives in the coating layer is 0.1 to

wherein flushing a target site in a body lumen and holding the drug-coated balloon catheter in position at the target site to hydrate the coating layer is sufficient for the coating layer to release more than 30 wt % of the initial drug load of the therapeutic agent from the coating layer to the target site in the body lumen.

2. The drug-coated balloon catheter of claim 1 , wherein the flushing comprises flushing the target site with a flushing media comprising water, saline solution, or a water solution comprising at least one water-soluble additive.

The drug-coated balloon catheter of claim 1 , wherein holding the drug-coated balloon catheter at the target site comprises holding the drug-coated balloon catheter in position at the target site for a soaking period prior to any inflation to hydrate the coating layer, wherein the soaking period is at least 1 minute.

4. The drug-coated balloon catheter of claim 1 , wherein holding the drug-coated balloon catheter at the target site comprises

holding the drug-coated balloon catheter in position at the target site prior to any inflation to hydrate the coating layer, and

inflating the drug-coated balloon catheter to contact walls of the body lumen at the target site with the coating layer for an inflation period.

5. The drug-coated balloon catheter of claim 4 , wherein the inflation period is 0.1 minute to 10 minutes.

6. The drug-coated balloon catheter of claim 1 , wherein holding the drug-coated balloon catheter at the target site comprises

holding the drug-coated balloon catheter in position at the target site prior to any inflation to hydrate the coating layer,

inflating the drug-coated balloon catheter to contact walls of the body lumen at the target site with the coating layer for an inflation period, and

deflating the drug-coated balloon catheter and withdrawing the drug-coated balloon catheter from the target site.

7. The drug-coated balloon catheter of claim 1 , wherein the flushing and holding is sufficient for the coating layer to release 37%-97% of the initial drug load to the target site in the body lumen.

The drug-coated balloon catheter of claim 1 , wherein the body lumen comprises a vascular lumen, a nonvascular lumen, a coronary artery, a peripheral artery, a prostatic urethra, a urethra, a ureter, an esophagus, a biliary tract, a stomach, a small intestine, a duodenum, a jejunum, an ileum, a colon, a rectum, a large intestine, a sinus, or a bronchial airway.

9. The drug-coated balloon catheter of claim 1 , wherein the initial drug load of the therapeutic agent is from 1 microgram to 20 micrograms per square millimeter of the balloon at the nominal diameter of the balloon.

10. The drug-coated balloon catheter of claim 1 , wherein the one or more additives in the coating layer comprise one or more water-soluble additives.

11. The drug-coated balloon catheter of claim 1 , wherein the one or more additives in the coating layer comprise one or more surfactants.

12. The drug-coated balloon catheter of claim 11 , wherein the surfactant is a cationic, anionic, or zwitterionic surfactant.

The drug-coated balloon catheter of claim 12 , wherein the surfactant is a quaternary ammonium salt, a fatty acid salt, a bile salt, benzalkonium chloride, benzethonium chloride, cetylpyridinium chloride, docecyl trimethyl ammonium bromide, sodium docecylsulfates, dialkyl methylbenzyl ammonium chloride, edrophonium chloride, domiphen bromide, dialkylester of sodium sulfonsuccinic acid, sodium dioctyl sulfosuccinate, sodium cholate, sodium taurocholate, or a combination thereof.

14. The drug-coated balloon catheter of claim 1 , wherein the one or more additives in the coating layer comprise one or more sterols and/or sterol derivatives.

15. The drug-coated balloon catheter of claim 14 , wherein the one or more sterols and/or sterol derivatives comprise PEG-24 cholesterol ether.

16. The drug-coated balloon catheter of claim 1 , wherein the one or more additives in the coating layer comprise one or more polymers.

17. The drug-coated balloon catheter of claim 1 , wherein the one or more additives in the coating layer comprise

a polymer, and

a sterol and/or sterol derivative.

18. The drug-coated balloon catheter of claim 1 , wherein the one or more additives in the coating layer comprise a polymer and a surfactant.

The drug-coated balloon catheter of claim 1 , wherein the drug-coated balloon catheter further comprises an adherent layer between the exterior surface of the balloon and the coating layer, wherein the adherent layer improves the adherence of the coating layer to the exterior surface of the balloon.

20. A drug-coated balloon catheter comprising:

an elongated balloon having a nominal diameter of 4 mm to 50 mm and a length of 20 mm to 160 mm; and

a coating layer overlaying an exterior surface of the balloon, the coating layer comprising

an initial drug load of a therapeutic agent of 1 micrograms to 20 micrograms of a therapeutic agent per square millimeter of the balloon at the nominal diameter of the balloon, the therapeutic agent comprising paclitaxel, a paclitaxel analogue, docetaxel, a docetaxel analogue, taxol, a taxol analogue, rapamycin, a rapamycin analogue, sirolimus, a sirolimus analogue, everolimus, an everolimus analogue, tacrolimus, a tacrolimus analogue, mammalian target of rapamycin (mTOR) inhibitors, an mTOR inhibitor analogue, or a combination thereof, and

one or more additives, wherein a ratio of the therapeutic agent to the one or more additives in the coating layer is 0.1 to 10;

wherein flushing a target site in a body lumen and holding the drug-coated balloon catheter in position at the target site to hydrate the coating layer is sufficient for the body lumen tissue at the target site to have a concentration of the therapeutic agent of 0.083 μg/g, to 582 μg/g at 1 day.

21. The drug-coated balloon catheter of claim 20 , wherein the flushing comprises flushing the target site with a flushing media comprising water, saline solution, or a water solution comprising at least one water-soluble additive, wherein the holding comprises

holding the drug-coated balloon catheter in position at the target site prior to any inflation for a soaking period to hydrate the coating layer in the flushing media,

inflating the drug-coated balloon catheter to contact walls of the body lumen at the target site with the coating layer for an inflation period,

deflating the drug-coated balloon catheter and withdrawing the drug-coated balloon catheter from the target site, and

withdrawing the drug-coated balloon catheter from the target site.

22. The drug-coated balloon catheter of claim 21 , wherein the inflation period is 0.1 minute to 10 minutes.

23. The drug-coated balloon catheter of claim 21 , wherein the soaking period is at least 1 minute.

The drug-coated balloon catheter of claim 20 , wherein the flushing and holding are sufficient for the coating layer to release the therapeutic agent from the coating layer into body lumen tissue at the target site such that the body lumen tissue at the target site has a concentration of the therapeutic agent of 0.003 μg/g to 62 μg/g at 7 days.

The drug-coated balloon catheter of claim 20 , wherein the flushing and holding are sufficient for the coating layer to release the therapeutic agent from the coating layer into body lumen tissue at the target site such that the body lumen tissue at the target site has a concentration of the therapeutic agent of 0.025 μg/g to 33 at 28 days.

26. A drug-coated balloon catheter comprising:

an elongated balloon having a nominal diameter of 4 mm to 50 mm and a length of 20 mm to 160 mm; and

a coating layer overlaying an exterior surface of the balloon, the coating layer comprising

an initial drug load of a therapeutic agent, the therapeutic agent comprising paclitaxel, a paclitaxel analogue, docetaxel, a docetaxel analogue, taxol, a taxol analogue, rapamycin, a rapamycin analogue, sirolimus, a sirolimus analogue, everolimus, an everolimus analogue, tacrolimus, a tacrolimus analogue, mammalian target of rapamycin (mTOR) inhibitors, an mTOR inhibitor analogue, or a combination thereof, and

one or more additives, wherein a ratio of the therapeutic agent to the one or more additives in the coating layer is 0.1 to

wherein flushing a target site in a body lumen and holding the drug-coated balloon catheter in position at the target site to hydrate the coating layer is sufficient for the coating layer to release 0.1% to 25% of the initial drug load of the therapeutic agent from the coating layer to a body lumen wall at the target site in the body lumen.

27. The drug-coated balloon catheter of claim 26 , wherein the flushing comprises flushing the target site with a flushing media comprising water, saline solution, or a water solution comprising at least one water-soluble additive, wherein the holding comprises

holding the drug-coated balloon catheter in position at the target site for a soaking period of at least 1 minute prior to any inflation to hydrate the coating layer in the flushing media,

inflating the drug-coated balloon catheter to contact walls of the body lumen at the target site with the coating layer for an inflation period of 0.1 minute to 10 minutes,

deflating the drug-coated balloon catheter, and

withdrawing the drug-coated balloon catheter from the target site.

28. The drug-coated balloon catheter of claim 26 , wherein the initial drug load of the therapeutic agent is from 1 microgram to 20 micrograms per square millimeter of the balloon at the nominal diameter of the balloon.
