Abstract

A catheter device for crossing occlusions includes an elongate catheter shaft, a rotatable tip configured to rotate relative to the elongate catheter shaft, a drive shaft, and an OCT imaging sensor. The rotatable tip includes a central guidewire lumen, a distal grinding surface, and a fluted portion. The distal grinding surface can be substantially perpendicular to the guidewire lumen and include divots and sharp points that form pyramidal features, where the sharp points extend distally from the distal grinding surface. The fluted portion can have flutes extending around a circumference of the rotatable tip proximal to the distal grinding surface. The fluted portion may not include the divots and sharp points that form the pyramidal features of the distal grinding surface.

Claims (26)

1. A catheter device for crossing occlusions, the catheter device comprising:

an elongate catheter shaft;

a rotatable tip configured to rotate relative to the elongate catheter shaft, wherein the rotatable tip comprises a central guidewire lumen, wherein the rotatable tip further comprises:

a distal grinding surface that is substantially perpendicular to the guidewire lumen, wherein the distal grinding surface comprises a plurality of divots and sharp points that form pyramidal features, wherein the sharp points extend distally from the distal grinding surface; and

a fluted portion having a plurality of flutes extending around a circumference of the rotatable tip proximal to the distal grinding surface, wherein the fluted portion does not include the plurality of divots and sharp points that form the pyramidal features of the distal grinding surface;

a drive shaft extending within the elongate catheter shaft and configured to rotate the rotatable tip; and

an OCT imaging sensor comprising an optical fiber coupled with the rotatable tip and configured to rotate therewith.

2. The catheter device of claim 1 , wherein a pitch of the plurality of flutes is greater than 0.10 inches.

3. The catheter device of claim 1 , wherein the fluted portion of the rotatable tip is tapered so as to have a smaller diameter at a distal end of the portion than a proximal end of the portion.

4. The catheter device of claim 1 , wherein there are only 2-3 flutes in the plurality of flutes.

5. The catheter device of claim 1 , wherein the plurality of flutes are configured to cut more aggressively when rotated in one direction than another.

6. The rotatable tip of claim 1 , wherein the rotatable tip further comprises a fiber channel along an outer surface thereof, the fiber channel configured to hold the optical fiber therein.

7. The catheter device of claim 1 , wherein the rotatable tip further comprises a countersink surrounding a distal portion of the guidewire lumen.

8. The catheter device of claim 1 , wherein the plurality of divots and sharp points are configured to grind or slice through hard tissue as the rotatable tip is rotated.

9. The catheter device of claim 1 , wherein the rotatable tip is configured to rotate in a first direction until a set tension or resistance threshold is reached and then switch to rotate in a second direction.

10. The catheter device of claim 1 , wherein the optical fiber is embedded in a transparent or translucent medium within the rotatable tip.

11. The catheter device of claim 10 , wherein the optical fiber is loose within the elongate catheter shaft.

12. The catheter device of claim 1 , wherein the elongate catheter shaft is configured form a fixed bend proximal to the rotatable tip.

13. The catheter device of claim 1 , wherein the imaging sensor is positioned within 5 mm of a distal end of the rotatable tip.

14. The catheter device of claim 1 , wherein the imaging sensor is configured to image laterally.

15. The catheter device of claim 1 , further comprising a chassis that is fixed relative to the elongate catheter shaft, the chassis configured to enable the rotatable distal tip to rotate relative to the elongate catheter shaft.

16. The catheter device of claim 15 , wherein the chassis comprises a plurality of window regions therein configured to enable the imaging sensor to image therethrough.

17. The catheter device of claim 1 , wherein the plurality of flutes is arranged in spirals around the rotatable tip, and wherein the plurality of divots and sharp points forms a non-spiral arrangement of pyramidal features.

18. The catheter device of claim 1 , wherein the pyramidal features are arranged in rows.

19. The catheter device of claim 1 , wherein the pyramidal features have a shallower pitch than the plurality of flutes.

20. The catheter device of claim 7 , wherein the countersink corresponds to an indented surface that is proximal to the distal grinding surface.
