Abstract

Disclosed is a surgical instrument comprising an end effector with independently activatable segmented electrodes.

Claims (80)

1 . A surgical instrument, comprising:

an anvil, comprising:

a row of staple pockets; and

a first electrode assembly, wherein the first electrode assembly comprises:

a first segmented electrode; and

a second segmented electrode spaced apart from the first segmented electrode; and

a cartridge, wherein the anvil and the cartridge are configured to grasp tissue therebetween, and wherein the cartridge comprises an asymmetric cartridge body, comprising:

a cartridge deck;

a row of staple cavities defined in the cartridge deck, wherein the row of staple cavities comprises staples deformable against the row of staple pockets; and

a second electrode assembly, wherein the second electrode assembly comprises:

a third segmented electrode; and

a fourth segmented electrode spaced apart from the third segmented electrode; and

a control circuit configured to:

pass a first sub-therapeutic signal through a first tissue portion between the first segmented electrode and the third segmented electrode;

monitor a first tissue impedance of the first tissue portion based on the first sub-therapeutic signal;

pass a second sub-therapeutic signal through a second tissue portion between the second segmented electrode and the fourth segmented electrode;

monitor a second tissue impedance of the second tissue portion based on the second sub-therapeutic signal;

adjust a first therapeutic signal configured to be passed between the first segmented electrode and the third segmented electrode based on the first tissue impedance;

adjust a second therapeutic signal configured to be passed between the second segmented electrode and the fourth segmented electrode based on the first tissue impedance and the second tissue impedance; and

issue an alert indicative of a short circuit based on the first tissue impedance.

2 . The surgical instrument of claim 1 , wherein the short circuit is detected between the first segmented electrode and the third segmented electrode if the first tissue impedance is different than a predetermined tissue impedance.

3 . The surgical instrument of claim 1 , wherein adjusting the first therapeutic signal comprises a reduction in a power parameter of the first therapeutic signal.

4 . The surgical instrument of claim 3 , wherein adjusting the second therapeutic signal comprises an increase in a power parameter of the first therapeutic signal.

5 . The surgical instrument of claim 1 , wherein adjusting the first therapeutic signal comprises reducing the first therapeutic signal to a sub-therapeutic level.

6 . The surgical instrument of claim 1 , wherein adjusting the first therapeutic signal comprises reducing the first therapeutic signal to a tissue warm-up only level.

7 . The surgical instrument of claim 1 , wherein adjusting the first therapeutic signal comprises deactivating the first segmented electrode.

8 . A surgical instrument, comprising:

an anvil, comprising:

a row of staple pockets; and

a first electrode assembly, wherein the first electrode assembly comprises:

a first segmented electrode; and

a second segmented electrode spaced apart from the first segmented electrode; and

a cartridge, wherein the anvil and the cartridge are configured to grasp tissue therebetween, and wherein the cartridge comprises an asymmetric cartridge body, comprising:

a cartridge deck;

a row of staple cavities comprising staples deformable against the row of staple pockets; and

a second electrode assembly, wherein the second electrode assembly comprises:

a third segmented electrode; and

a fourth segmented electrode spaced apart from the third segmented electrode; and

a control circuit configured to:

pass a first sub-therapeutic signal into a first tissue portion between the first segmented electrode and the third segmented electrode;

monitor a first tissue impedance based on the first sub-therapeutic signal;

pass a second sub-therapeutic signal into a second tissue portion between the second segmented electrode and the fourth segmented electrode;

monitor a second tissue impedance based on the second sub-therapeutic signal;

select a first therapeutic energy profile for the first tissue portion based on the first tissue impedance; and

select a second therapeutic energy profile for the second tissue portion based on the second tissue impedance, wherein the first therapeutic energy profile is different than the second therapeutic energy profile.

The surgical instrument of claim 8 , wherein control circuit is configured to determine whether to sequentially or simultaneously apply the first therapeutic energy profile and the second therapeutic energy profile based on whether available power is less than or equal to a threshold based on combined values of a first power requirement of the first therapeutic energy profile and a second power requirement of the second therapeutic energy profile.

10 . The surgical instrument of claim 8 , wherein the control circuit is further configured to:

calculate a first tissue-sealing time based on the first tissue impedance;

calculate a second tissue-sealing time based on the second tissue impedance;

compare the first tissue-sealing time to the second tissue-sealing time; and

select starting times for application of the first therapeutic energy profile to the first tissue portion and application of the second therapeutic energy profile to the second tissue portion based on the first tissue-sealing time and the second tissue-sealing time.

The surgical instrument of claim 10 , wherein the control circuit is configured to begin application of the second therapeutic energy profile to the second tissue portion prior to application of the first therapeutic energy profile to the first tissue portion if the second tissue-sealing time is greater than the first tissue-sealing time.

12 . The surgical instrument of claim 8 , wherein the control circuit is further configured to alternate application of the first therapeutic energy profile and the second therapeutic energy profile.

13 . The surgical instrument of claim 8 , wherein the control circuit is further configured to alternate activation of the first segmented electrode and the second segmented electrode.

The surgical instrument of claim 8 , wherein the first therapeutic energy profile is configured to yield a first minimum tissue impedance of the first tissue portion, wherein the second therapeutic energy profile is configured to yield a second minimum tissue impedance of the first tissue portion, and wherein the control circuit is configured to cause the first minimum tissue impedance and the second minimum tissue impedance to be achieved at different times.

The surgical instrument of claim 8 , wherein the first therapeutic energy profile comprises a first maximum power level, wherein the second therapeutic energy profile comprises a second maximum power level, and wherein the control circuit is configured to cause the first maximum power level and the second maximum power level to be reached at different times.

16 . A surgical instrument, comprising:

an anvil, comprising:

a row of staple pockets; and

a first electrode assembly, wherein the first electrode assembly comprises:

a first segmented electrode; and

a second segmented electrode spaced apart from the first segmented electrode; and

a cartridge, wherein the anvil and the cartridge are configured to grasp tissue therebetween, and wherein the cartridge comprises an asymmetric cartridge body, comprising:

a cartridge deck;

a row of staple cavities defined in the cartridge deck, wherein the row of staple cavities comprises staples deformable against the row of staple pockets; and

a second electrode assembly, wherein the second electrode assembly comprises:

a third segmented electrode; and

a fourth segmented electrode spaced apart from the third segmented electrode; and

a control circuit configured to:

pass a first sub-therapeutic signal through a first tissue portion between the first segmented electrode and the third segmented electrode;

monitor a first tissue impedance of the first tissue portion based on the first sub-therapeutic signal;

pass a second sub-therapeutic signal through a second tissue portion between the second segmented electrode and the fourth segmented electrode;

monitor a second tissue impedance of the second tissue portion based on the second sub-therapeutic signal;

detect a short circuit between the first segmented electrode and the third segmented electrode based on the first tissue impedance;

in response to detecting the short circuit, apply a therapeutic energy profile to the tissue comprising alternating between (i) a bipolar energy excluding at least one of the first segmented electrode and the third segmented electrode, and (ii) a monopolar energy.

17 . The surgical instrument of claim 16 , wherein the control circuit is configured to issue an alert indicative of the short circuit.

18 . The surgical instrument of claim 16 , wherein detecting the short circuit comprises determining that the first tissue impedance is less than or equal to a threshold.

19 . The surgical instrument of claim 16 , wherein the bipolar energy, comprises:

a first bipolar energy between the second segmented electrode and the fourth segmented electrode; and

a second bipolar energy between the second segmented electrode and the third segmented electrode.
