Abstract

A replacement heart valve prosthesis can be delivered to or within an existing prosthesis through a valve-in-valve delivery procedure. The existing prosthesis can be a prosthetic heart valve or other device that is implanted at a target site, such as an aortic or mitral valve of the heart. The methods provided herein can allow a surgeon to position a replacement prosthesis within or adjacent to an existing prosthesis and permit the replacement prosthesis to expand into engagement against the existing prosthesis. Thereafter, the surgeon can adjust components of the replacement prosthesis, including the position of separate anchor and frame components in order to ensure accurate and secure placement of the replacement prosthesis at the target site.

Claims (27)

1 . A method for delivering a replacement heart valve prosthesis within an existing prosthesis at a native valve structure of a patient, the method comprising: introducing the replacement prosthesis into the patient, the replacement prosthesis comprising a valve frame, a valve anchor, and a link mechanism coupling the valve frame to the valve anchor, the valve anchor comprising U-shaped members having peak portions that are coupled together to form respective engagement areas between the U-shaped members; positioning the replacement prosthesis within or adjacent to the existing prosthesis; moving the valve frame relative to the valve anchor to slide the link mechanism toward a respective engagement area of the valve anchor to urge the link mechanism into the respective engagement area, thereby capturing the link mechanism therein and restricting a range of movement of the valve frame relative to the valve anchor; while the link mechanism is engaged in the respective engagement area, moving the valve frame until the link mechanism is taut and pulls the valve anchor relative to the existing prosthesis; and permitting expansion of the replacement prosthesis against the existing prosthesis.

2 . The method of claim 1 , wherein the permitting comprises permitting expansion of a base portion of the valve anchor.

3 . The method of claim 2 , further comprising urging the base portion of the valve anchor into engagement with the native valve structure or the existing prosthesis.

The method of claim 1 , wherein after the moving the valve frame, the method further comprises rotating the valve frame, while the link mechanism is engaged at at least one engagement area of the valve anchor, to rotationally adjust a position of the valve anchor relative to the native valve structure.

5 . The method of claim 1 , wherein the existing prosthesis comprises an existing prosthetic heart valve and the permitting comprises permitting expansion of the replacement prosthesis within a lumen of the existing prosthetic heart valve.

6 . The method of claim 1 , wherein the permitting expansion of the replacement prosthesis comprises positioning the valve anchor of the replacement prosthesis within the existing prosthesis and thereafter advancing the valve frame of the replacement prosthesis into the valve anchor.

7 . The method of claim 6 , wherein the advancing the valve frame into the valve anchor comprises advancing the valve frame until further movement of the valve frame relative to the valve anchor is restricted by the link mechanism extending between the valve anchor and the valve frame.

8 . The method of claim 1 , wherein the native valve structure comprises an aortic valve.

A method of valve-in-valve placement of a replacement heart valve prosthesis within a patient, the method comprising: positioning the replacement prosthesis within or adjacent to an existing prosthesis; permitting expansion of a valve anchor of the replacement prosthesis into engagement against the existing prosthesis, the valve anchor comprising U-shaped members having peak portions that are coupled together to form respective engagement areas between the U-shaped members; moving a valve frame of the replacement prosthesis relative to the valve anchor to slide a link mechanism, interconnecting the valve frame and the valve anchor, along the U-shaped members to capture the link mechanism in the respective engagement areas; wherein after the moving the valve frame, advancing the valve frame, with the link mechanism being engaged at the engagement area of the peak portion of the valve anchor, until the link mechanism is taut and further advancing the valve frame to pull the valve anchor relative to a native valve structure or the existing prosthesis; and permitting adjustment and expansion of a valve frame of the replacement prosthesis within the valve anchor, wherein rotation and translation of the valve frame relative to the valve anchor is limited only by the link mechanism.

10 . The method of claim 9 , further comprising urging a base portion of the valve anchor into engagement with a native valve structure or the existing prosthesis.

The method of claim 9 , wherein after the moving the valve frame, the method further comprises rotating the valve frame, with the link mechanism being engaged at the engagement area of the peak portion of the valve anchor, to rotationally adjust a position of the valve anchor relative to a native valve structure or the existing prosthesis.

12 . The method of claim 9 , wherein the existing prosthesis comprises an existing prosthetic heart valve and the permitting comprises permitting expansion of the replacement prosthesis within a lumen of the existing prosthetic heart valve.

13 . The method of claim 9 , wherein the positioning comprises positioning the replacement prosthesis adjacent to or within an aortic valve.

A heart valve replacement method that comprises replacing an existing prosthesis with a replacement prosthesis that includes a valve frame, a valve anchor, and a link mechanism interconnecting the valve anchor and the valve frame, the valve anchor comprising leg members that are coupled together at peak portions and form respective engagement areas at the peak portions, the method comprising moving the valve frame relative to the valve anchor to slide the link mechanism along the leg members to capture the link mechanism in the respective engagement areas and align the valve frame within the valve anchor at a desired position, and permitting expansion of the replacement prosthesis at the desired position within the existing prosthesis.

15 . The method of claim 14 , wherein the leg members of the valve anchor form a plurality of U-shaped members.

The method of claim 14 , wherein after the moving the valve frame, the method further comprises advancing the valve frame, while the link mechanism is engaged at at least one of the respective engagement areas, until the link mechanism is taut and further advancing the valve frame to pull the valve anchor relative to a native valve structure or the existing prosthesis.

The method of claim 14 , wherein after the moving the valve frame, the method further comprises rotating the valve frame, with the link mechanism being engaged at one of the respective engagement areas, to rotationally adjust a position of the valve anchor relative to a native valve structure or the existing prosthesis.

18 . The method of claim 14 , wherein the existing prosthesis comprises an existing prosthetic heart valve and the permitting comprises permitting expansion of the replacement prosthesis within a lumen of the existing prosthetic heart valve.

A method for delivering a replacement heart valve prosthesis within an existing prosthesis at a native valve structure of a patient, the method comprising: introducing the replacement prosthesis into the patient, the replacement prosthesis comprising a valve frame, a valve anchor, and a link mechanism coupling the valve frame to the valve anchor, the valve anchor comprising U-shaped members having peak portions that are coupled together to form respective engagement areas between the U-shaped members; positioning the replacement prosthesis within or adjacent to the existing prosthesis with the valve anchor of the replacement prosthesis being within the existing prosthesis; moving the valve frame relative to the valve anchor to slide the link mechanism toward a respective engagement area of the valve anchor to urge the link mechanism into the respective engagement area, thereby capturing the link mechanism therein and restricting a range of movement of the valve frame relative to the valve anchor; moving the valve frame within the valve anchor until further movement of the valve frame relative to the valve anchor is restricted by a link mechanism extending between the valve anchor and the valve frame; and permitting expansion of the replacement prosthesis against the existing prosthesis.

20 . The method of claim 1 , wherein the method is performed via a transfemoral approach.

21 . The method of claim 1 , wherein the method is performed via a transapical approach.

22 . The method of claim 9 , wherein the method is performed via a transfemoral approach.

23 . The method of claim 9 , wherein the method is performed via a transapical approach.

24 . The method of claim 14 , wherein the method is performed via a transfemoral approach.

25 . The method of claim 14 , wherein the method is performed via a transapical approach.

26 . The method of claim 19 , wherein the method is performed via a transfemoral approach.

27 . The method of claim 19 , wherein the method is performed via a transapical approach.
