Abstract

A method of treating mitral valve stenosis in a human heart includes advancing a delivery apparatus through a right femoral vein, across a septal wall, and through a calcified mitral valve. A prosthetic heart valve is provided along a distal end portion of the delivery apparatus, wherein the prosthetic heart valve includes a metallic frame having a plurality of tissue-engaging elements positioned along an exterior surface and a leaflet structure for replacing the function of the calcified mitral valve. The prosthetic heart valve is radially expanded within an annulus of the calcified mitral valve such that at least some of the tissue-engaging elements of the prosthetic heart valve embed into the annulus. After radial expansion, the prosthetic heart valve is retained in position within the annulus via the tissue-engaging element and the leaflet structure replaces the function of the native mitral valve.

Claims (35)

1 . A method of treating valve stenosis in a native valve of a human heart of a patient, the method comprising:

advancing a delivery apparatus through a patient's vasculature to a native valve having a calcified annulus, the delivery apparatus including a prosthetic heart valve disposed on a distal end portion of the delivery apparatus in a radially collapsed state, the prosthetic heart valve comprising a metallic, annular inner frame, an outer frame radially outward of the inner frame and sutured to the inner frame, the outer frame comprising an atrial portion and a ventricular portion, the outer frame comprising a plurality of tissue-engaging elements positioned on an exterior surface of the outer frame, the inner frame comprising a leaflet structure for replacing the function of the native valve, the leaflet structure positioned between first and second ends of the metallic inner frame, the prosthetic heart valve comprising an inflow end and an outflow end;

radially expanding the prosthetic heart valve to an expanded state within the calcified annulus of the native valve such that the atrial portion of the outer frame is positioned in an atrium of the heart and extends upwardly beyond an inflow end of the inner frame, wherein at least some of the tissue-engaging elements of the prosthetic heart valve engage the calcified annulus, and wherein a diameter of the outer frame of the prosthetic heart valve increases from a first diameter at the outflow end to a second diameter moving in an upstream direction toward the inflow end; and

removing the delivery apparatus from the patient's vasculature;

wherein, after radial expansion, the prosthetic heart valve is retained within the calcified annulus of the native valve and the leaflet structure replaces the function of the native valve.

2 . The method of claim 1 , wherein at least some of the tissue-engaging elements embed into native leaflets of the native valve during radial expansion of the prosthetic heart valve.

3 . The method of claim 1 , wherein the tissue-engaging elements are barbs.

4 . The method of claim 1 , wherein the tissue-engaging elements deploy radially outwardly as the prosthetic heart valve is expanded.

5 . The method of claim 1 , wherein the prosthetic heart valve comprises a shoulder when expanded, and radially expanding the prosthetic heart valve further comprises radially expanding the prosthetic heart valve such that the shoulder engages native leaflets of the native valve.

6 . The method of claim 1 , wherein the prosthetic heart valve comprises a shoulder when expanded, and radially expanding the prosthetic heart valve further comprises radially expanding the prosthetic heart valve such that the shoulder engages a ventricle of the heart.

7 . The method of claim 1 , wherein continuing in the upstream direction, the diameter of the prosthetic heart valve decreases from the second diameter back to the first diameter.

The method of claim 1 , further comprising radially expanding the prosthetic heart valve such that the inner frame foreshortens from a first length in the collapsed state to a second length in the expanded state, the foreshortening of the inner frame bending the ventricular portion of the outer frame radially outwardly into a curved shape in a ventricle of the heart.

9 . The method of claim 8 , wherein the ventricular portion of the outer frame comprises the tissue-engaging elements.

10 . A method of treating valve stenosis in a native valve of a human heart, the method comprising:

advancing a delivery apparatus through a patient's vasculature to a native valve, the native valve having an annulus that includes calcium deposits, the delivery apparatus including a prosthetic heart valve disposed on a distal end portion of the delivery apparatus in a radially collapsed state, the prosthetic heart valve comprising an annular inner frame, a leaflet structure coupled to the inner frame and configured for replacing the function of the native valve, and an outer frame sutured to an outflow end of the inner frame, the outer frame comprising an atrial portion, a ventricular portion, and a plurality of tissue-engaging elements;

positioning the prosthetic heart valve in the annulus of the native valve;

radially expanding the inner frame and the outer frame such that the atrial portion of the outer frame is located in an atrium of the heart and extends upwardly beyond an inflow end of the inner frame; and

anchoring the prosthetic heart valve in the annulus of the native valve with the tissue-engaging elements.

The method of claim 10 , wherein anchoring the prosthetic heart valve further comprises radially expanding the prosthetic heart valve in the annulus of the native valve to an expanded state such that the tissue-engaging elements of the prosthetic heart valve embed into tissue of the annulus of the native valve.

12 . The method of claim 11 , wherein the tissue-engaging elements deploy radially outwardly as the prosthetic heart valve is expanded.

13 . The method of claim 11 , wherein the tissue-engaging elements are barbs.

14 . The method of claim 10 , wherein:

the native valve is a native mitral valve; and

the method further comprises:

deploying the atrial portion of the outer frame in a left atrium of the heart; and

deploying the ventricular portion of the outer frame in a left ventricle of the heart.

15 . A method of treating mitral valve stenosis in a human heart, the method comprising:

positioning a prosthetic heart valve in a calcified annulus of a native mitral valve of a heart, the prosthetic heart valve being in a radially collapsed state, the prosthetic heart valve comprising an annular inner frame including an inflow end, an outflow end, and a leaflet structure, and an outer frame radially outward of the inner frame and coupled to the inflow end and to the outflow end of the inner frame, the outer frame comprising a ventricular portion, an atrial portion, and a plurality of tissue-engaging elements;

deploying the atrial portion of the outer frame in a left atrium of the heart; and

radially expanding the prosthetic heart valve such that the inner frame foreshortens from a first length in the collapsed state to a second length in an expanded state, the foreshortening of the inner frame bending the ventricular portion of the outer frame radially outwardly into a curved shape in a left ventricle of the heart such that the tissue-engaging elements engage tissue of the calcified annulus of the native mitral valve.

16 . The method of claim 1 , wherein the prosthetic heart valve comprises a skirt member secured to a lower side of the atrial portion of the outer frame.

17 . The method of claim 1 , wherein the prosthetic heart valve further comprises a skirt member between the inner frame and the outer frame.

18 . The method of claim 1 , wherein the inner frame comprises a plurality of commissure windows, and leaflet tabs of the leaflet structure are received in the commissure windows to form commissures.

19 . The method of claim 1 , wherein the outer frame is sutured to the inner frame at an outflow end of the inner frame.

20 . The method of claim 19 , wherein the outer frame is sutured to the inner frame at an inflow end of the inner frame.
