Abstract

A fixation system for repairing a pars fracture includes a needle guide configured to be docked on a lamina of the pars, a needle, a guidewire, and a screw assembly. A portion of the needle is configured for insertion through a passage of the needle guide. A portion of the guidewire is configured for insertion through a passage of the needle. The screw assembly includes a collar and an elongated portion, the collar being movable with respect to the elongated portion. A portion of the screw assembly is configured to contact the lamina co-axially with the guidewire.

Claims (33)

1. A fixation system for repairing a pars fracture, comprising:

a needle guide configured to be docked on a lamina of the pars;

a needle, a portion of the needle configured for insertion through a passage of the needle guide;

a guidewire, a portion of the guidewire configured for insertion through a passage of the needle;

a screw assembly including a collar and an elongated portion, the collar being movable with respect to the elongated portion, a portion of the screw assembly configured to be inserted into the lamina co-axially over the guidewire; and

a surgical instrument configured to engage the screw assembly and contact the collar, the surgical instrument including a first movable handle configured to be actuated to lock the collar of the screw assembly with respect to the elongated portion of the screw assembly,

wherein the surgical instrument includes a second movable handle configured to be actuated to cause a least a portion of the collar of the screw assembly to move distally toward a threaded portion of the screw assembly.

2. The fixation system of claim 1 , wherein the first movable handle is configured to fix a longitudinal position of a first portion of the collar to the elongated portion of the screw assembly and move a second portion of the collar relative to the first portion.

3. The fixation system of claim 2 , wherein the first portion of the collar is an inner sleeve defining a longitudinal axis extending along a first direction, the inner sleeve defining a body and a plurality of legs extending from the body along the first direction.

4. The fixation system of claim 3 , wherein the second portion of the collar is an outer ring and an annular ring positionable within a bore of the outer ring, the annular ring rotatable with respect to the outer ring.

5. The fixation system of claim 4 , wherein the inner sleeve is positionable within a bore of the annular ring.

The fixation system of claim 1 , wherein the surgical instrument includes a third movable handle configured to be actuated to release the surgical instrument from engagement with the screw assembly while the collar remains secured in place with respect to the elongated portion of the screw assembly.

7. The fixation system of claim 1 , wherein the elongated portion of the screw assembly includes a channel extending along its length, the channel configured to accept at least a portion of the guidewire therethrough.

8. The fixation system of claim 1 , wherein the screw assembly includes the threaded portion on a distal portion of the screw assembly and the screw assembly is configured to be rotated by a hex driver to be inserted into the lamina of the pars.

9. The fixation system of claim 1 , wherein the screw assembly is configured to bridge the pars fracture to facilitate healing.

10. The fixation system of claim 1 , wherein the screw assembly includes a frangible portion configured to be severed to separate at least part of the elongate portion of the screw assembly.

11. The fixation system of claim 10 , wherein the frangible portion is located near a proximal end of the threaded portion of the screw assembly.

12. The fixation system of claim 1 , further comprising a stylet, a portion of the stylet configured for insertion through the passage of the needle.

13. The fixation system of claim 12 , wherein the stylet includes a handle portion configured to engage a handle portion of the needle.

14. The fixation system of claim 12 , wherein the stylet includes a distal tip configured to engage the lamina of the pars.

15. The fixation system of claim 12 , wherein the stylet is configured to removably dock to the needle.

16. The fixation system of claim 1 , wherein the guidewire includes a distal tip configured to engage the lamina of the pars.

17. A fixation system for repairing a pars fracture, comprising:

a needle guide configured to be docked on a lamina of the pars;

a needle, a portion of the needle configured for insertion through a passage of the needle guide;

a stylet, a portion of the stylet configured for insertion through a passage of the needle;

a guidewire, a portion of the guidewire configured for insertion through a passage of the needle;

a screw assembly including a collar and an elongated portion, the collar being movable with respect to the elongated portion; and

a surgical instrument including a first movable handle configured to be actuated to move the collar relative to the elongated portion and a second movable handle configured to be actuated to lock the collar of the screw assembly with respect to the elongated portion of the screw assembly.

18. The fixation system of claim 17 , wherein the surgical instrument includes a third movable handle configured to be actuated to release the surgical instrument from engagement with the screw assembly.

19. A fixation system for repairing a pars fracture, comprising:

a screw assembly including a collar and an elongated portion, the collar being slidably movable with respect to the elongated portion, a portion of the screw assembly configured to be inserted into a lamina co-axially over a guidewire; and

a surgical instrument including a first movable handle configured to be actuated to move the collar relative to the elongated portion, a second movable handle configured to be actuated to lock the collar of the screw assembly with respect to the elongated portion of the screw assembly, and a third movable handle configured to be actuated to release the surgical instrument from engagement with the screw assembly while the collar remains secured in place with respect to the elongated portion of the screw assembly.
