Abstract

A method is disclosed for making a glenoid guide that includes obtaining scapula surface profile information of a specific patient, providing a manufacturing plan for making the glenoid guide having at least one patient specific contact surface configured to contact a portion of the glenoid rim or glenoid surface and for making the guide having a rotation control feature forming member, and adapting the manufacturing plan to specify the location of the rotation control forming feature relative to a portion of the glenoid determined to benefit from augmentation based on the surface profile information.

Claims (48)

1. A surgical instrument for implanting an augmented glenoid implant, comprising:

an elongate body comprising a first end, a second end, and a glenoid implant component retention feature disposed on the first end configured to retain the augmented glenoid implant to the elongate body during implantation of the augmented glenoid implant; and

a rotation guide comprising an instrument interface and a bone interface portion disposed on another end thereof, the rotation guide comprising a rigid body disposed between the instrument interface and the bone interface portion wherein a shape and/or a length of the rigid body is configured for a specific patient to control the rotational position of the augmented glenoid implant,

wherein the elongate body comprises an interface portion configured to releaseably engage the rotation guide.

The surgical instrument of claim 1 , further comprising an outer shell comprising the elongate body and an inner shell slideably disposed in the outer shell, the inner shell having a first position and a second position within the outer shell, the first position being closer to the first end of the outer shell than the second position, the inner shell configured to actuate the glenoid implant component retention feature to a retention configuration when in the first position relative to the outer shell.

3. The surgical instrument of claim 1 , wherein the rigid body of the rotation guide is configured such that the instrument interface portion is disposed at a position relative to the glenoid of the specific patient when the bone interface portion is placed in a superior glenoid position.

4. The surgical instrument of claim 3 , wherein the rigid body is configured such that the position of the instrument interface is disposed on a side of the glenoid opposite a portion of the glenoid of the specific patient to be augmented by the augmented glenoid implant.

5. The surgical instrument of claim 3 , wherein the rigid body is configured such that the position of the bone interface portion is on an anterior section of the glenoid when the bone interface portion is placed in a superior position of the glenoid of the specific patient.

6. The surgical instrument of claim 1 , wherein the interface portion of the instrument comprises a lateral peg.

7. The surgical instrument of claim 6 , wherein the lateral peg is disposed on an outer portion of the instrument.

8. The surgical instrument of claim 7 , wherein the instrument interface of the rotation guide comprises an aperture.

9. The surgical instrument of claim 1 , wherein the rotation guide comprises a first member configured to be inserted into a channel formed in or adjacent to a glenoid, the rigid body configured to couple with the first member after the first member is inserted into the channel.

10. The surgical instrument of claim 9 , wherein the first member comprises a peg and the bone interface portion is adapted to be slideably advanced over a proximal portion of the peg.

11. The surgical instrument of claim 10 , wherein the peg comprises a patient specific medial surface.

12. The surgical instrument of claim 9 , wherein the first member comprises a wire and the rigid body is adapted to be slideably advanced over the wire.

13. The surgical instrument of claim 12 , wherein the rigid body comprises a patient specific medial surface.

14. The surgical instrument of claim 2 , wherein the inner shell comprises a first end, a second end, and a lumen disposed therebetween, the lumen configured to receive a shaft to control a position of an anchor member.

15. The surgical instrument of claim 14 , further comprising an inner shaft disposed in the lumen of the inner shell, the inner shaft comprising a torque interface on a first end thereof and a handle on a second end opposite the torque interface.

16. A surgical instrument for implanting an augmented glenoid implant comprising:

an elongate body including:

an outer shell extending from a first end to a second end, the first end of the outer shell including a glenoid implant component retention feature, and

an inner shell slideably disposed in the outer shell, the inner shell configured to move between a first position within the outer shell and a second position within the outer shell, the first position being closer to the first end of the outer shell than the second position, wherein the inner shell is configured to actuate the glenoid implant component retention feature to a retention configuration when in the first position; and

a rotation guide comprising an instrument interface and a bone interface portion, the rotation guide comprising a rigid body disposed between the instrument interface and the bone interface portion wherein a shape and/or a length of the rigid body is configured for a specific patient to control the rotational position of the glenoid implant component,

wherein the elongate body comprises an interface portion configured to releasably engage the rotation guide.

17. The surgical instrument of claim 16 , wherein the rigid body of the rotation guide is configured such that:

the instrument interface portion is disposed at a position relative to the glenoid of the specific patient when the bone interface portion is placed in a superior glenoid position, and

the position of the bone interface portion is on an anterior section of the glenoid when the bone interface portion is placed in a superior position of the glenoid of the specific patient.

18. The surgical instrument of claim 16 , wherein the interface portion includes a lateral peg disposed on an outer portion of the instrument.

19. The surgical instrument of claim 16 , wherein the instrument interface of the rotation guide includes an aperture.

20. The surgical instrument of claim 16 , wherein the rotation guide includes a first member configured to be inserted into a channel formed in or adjacent to a glenoid, and wherein the rigid body is configured to couple with the first member when the first member is disposed in the channel.

21. The surgical instrument of claim 20 , wherein the first member includes a peg, and wherein the bone interface portion is adapted to be advanced over a proximal portion of the peg.

22. The surgical instrument of claim 21 , wherein the peg includes a patient-specific surface.

23. The surgical instrument of claim 20 , wherein the first member include a wire, and wherein the rigid body is adapted to be advanced over the wire.

24. The surgical instrument of claim 16 , wherein the rigid body includes a patient-specific surface.

25. The surgical instrument of claim 16 , wherein the inner shell extends from a first end to a second end, the inner shell defining a lumen between the first end of the inner shell and the second end of the inner shell, the lumen sized and configured to receive a shaft for controlling a position of an anchor.

26. The surgical instrument of claim 25 , further comprising:

an inner shaft disposed in the lumen of the inner shell, the inner shaft including a torque interface on a first end of the shaft and a handle on a second end of the shaft.

27. A surgical instrument for implanting a glenoid implant comprising:

an elongate body including:

an outer shell extending from a first end to a second end, the first end of the outer shell including a glenoid implant component retention feature configured to retain the glenoid implant to the elongate body during implantation of the glenoid implant, and

an inner shell slideably disposed in the outer shell, the inner shell configured to move between a first position within the outer shell and a second position within the outer shell, the first position being closer to the first end of the outer shell than the second position, wherein the inner shell is configured to actuate the glenoid implant component retention feature to a retention configuration when in the first position; and

a rotation guide comprising an instrument interface and a bone interface portion, the rotation guide comprising a rigid body disposed between the instrument interface and the bone interface portion, the rigid body configured to control the rotational position of the glenoid implant,

wherein the elongate body comprises an interface portion configured to releasably engage the rotation guide.

28. The surgical instrument of claim 27 , wherein the inner shell extends from a first end to a second end, the inner shell defining a lumen between the first end of the inner shell and the second end of the inner shell, the lumen sized and configured to receive a shaft for controlling a position of an anchor.

29. The surgical instrument of claim 28 , further comprising:

an inner shaft disposed in the lumen of the inner shell, the inner shaft including a torque interface on a first end and a handle on a second end.

30. The surgical instrument of claim 29 , wherein the shape and/or length of the rigid body is configured for a specific patient.

31. The surgical instrument of claim 30 , wherein the rigid body includes a patient-specific surface.
