CLOTTING FACTOR PREPARATIONS FOR DELIVERY INTO TISSUE OF THE INTESTINAL TRACT USING A SWALLOWABLE DRUG DELIVERY DEVICE
Embodiments provide devices, preparations and methods for delivering therapeutic agents (TAs) such as clotting factors (CFs), e.g., Factor VIII (FVIII) including PEGylated and other forms of stabilized FVIII, within the GI tract. Many embodiments provide a swallowable device e.g., a capsule for delivering TAs into the intestinal wall (IW). Embodiments also provide TA preparations configured to be contained within the capsule, advanced from the capsule into the IW and/or surrounding tissue (ST) and degrade to release the TA into the bloodstream to produce a therapeutic effect (e.g., improved clotting). The preparation can be operably coupled to delivery means having a first configuration where the preparation is contained in the capsule and a second configuration where the preparation is advanced out of the capsule into the IW or ST (e.g., the peritoneal cavity). Embodiments are particularly useful for delivery of CFs for treatment of clotting disorders (e.g., hemophilia) where such CFs are poorly absorbed and/or degraded within the GI tract.

A therapeutic preparation for treatment of a clotting disorder in a patient, the therapeutic preparation comprising a stabilized form of Factor VIII (FVIII) at a dosage above 75 IU/kg body weight of the patient and configured for insertion through the patient's intestinal wall into the patient's peritoneal cavity, wherein the therapeutic preparation releases the FVIII into the blood stream.
2 . The therapeutic preparation of claim 1 , wherein the stabilized Factor VIII comprises PEGylated Factor VIII.
3 . The therapeutic preparation of claim 2 , wherein the PEGylated Factor VIII is present in the therapeutic preparation in a range from 75 IU/kg body weight of the patient to 400 IU/kg body weight of the patient.
4 . The therapeutic preparation of claim 2 , wherein the PEGylated Factor VIII is present in the therapeutic preparation in a range from 150 IU/kg body weight of the patient to 300 IU/kg body weight of the patient.
5 . The therapeutic preparation of claim 1 , wherein the therapeutic preparation is included in a solid tissue penetrating member configured to penetrate an intestinal wall by an application of force on the tissue penetrating member.
6 . The therapeutic preparation of claim 1 , having a T max in a range from 6 to 10 hours when delivered intraperitoneally to a patient.
7 . The therapeutic preparation of claim 1 , having a C max in a range from about 2.4 IU/ml to 4.0 IU/ml when delivered intraperitoneally to a patient.
8 . The therapeutic preparation of claim 1 , having an area under the curve (AUC) in a range from 60 (IU*h)/mL to 70 (IU*h)/mL when delivered intraperitoneally to a patient.
9 . The therapeutic preparation of claim 1 , having a plasma concentration-time curve with a ratio of a rising curve portion to a falling curve portion in a range from 1:4 to 1:8 when delivered intraperitoneally to a patient.
10 . The therapeutic preparation of claim 1 , wherein the stabilized FVIII comprises FVIII chemically complexed with Von Willebrand factor.
11 . The therapeutic preparation of claim 1 , wherein the stabilized FVIII comprises FVII modified by Fc-fusion.
12 . A method for treating a clotting disorder in a patient, the method comprising:
providing stabilized Factor VIII (FVIII) in a dosage above 75 IU/kg body weight of the patient; and
delivering the FVIII through the patient's intestinal wall and into the patient's peritoneal cavity;
wherein the FVIII is released into the blood stream to treat the clotting disorder.

The method of claim 12 , wherein the stabilized Factor VIII is provided as a solid tissue penetrating member coupled to an actuator disposed in a swallowable device, and wherein delivering the stabilized Factor VIII through the patient's intestinal wall comprises causing the solid tissue penetrating member to penetrate through the patient's intestinal wall by application of force on the tissue penetrating member from the actuator after oral ingestion of the device.

15 . The method of claim 13 , wherein the stabilized Factor VIII comprises PEGylated factor VIII.
16 . The method of claim 15 , wherein the PEGylated factor VIII is present in a therapeutic preparation in a range from 75 IU/kg body weight of the patient to 400 IU/kg body weight of the patient.
17 . The method of claim 15 , wherein the PEGylated factor VIII is present in a therapeutic preparation in a range from150 IU/kg body weight of the patient to 300 IU/kg body weight of the patient.
18 . The method of claims 15 , having a T max in a range from 6 to 10 hours.
19 . The method of claim 15 , having a C max in a range from about 2.4 IU/ml to 4.0 IU/ml.
20 . The method of claim 15 , having an area under the curve (AUC) of a plasma concentration-time curve in a range from 60 (IU*h)/mL to 70 (IU*h)/mL.
