Abstract

Described are methods, systems, devices for facilitation of intraluminal medical procedures within the neurovasculature. A catheter advancement device includes a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween. The flexible elongate body has a proximal segment, an intermediate segment, and a tip segment. The proximal segment includes a hypotube coated with a polymer. The intermediate segment includes an unreinforced polymer having a durometer of no more than 72D. The tip segment is formed of a polymer different from the intermediate segment and has a durometer of no more than about 35D and a length of at least 5 cm. The tip segment has a tapered portion that tapers distally from a first outer diameter to a second outer diameter over a length of between 1 and 3 cm.

Claims (79)

1. An intravascular catheter advancement device for advancing a catheter within the vasculature, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment, wherein the proximal segment comprises a hypotube;

an intermediate segment comprising an unreinforced polymer having a first durometer; and

a tip segment formed of a polymer different from the unreinforced polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment comprises at least a first radiopaque marker, and wherein the tip segment has a tapered portion that tapers distally from a first outer diameter of between 0.048″ and 0.080″ to a second outer diameter of about 0.031″ up to about 0.048″ over a length between 0.5 cm and 4.0 cm,

wherein the catheter advancement device has a length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature.

2. The device of claim 1 , wherein a taper angle of a wall of the tapered portion relative to a center line of the tapered portion is between 0.9 to 1.6 degrees.

3. The device of claim 1 , wherein the second outer diameter is about ½ of the first outer diameter, about 40% of the first outer diameter, or about 65% of the first outer diameter.

4. The device of claim 1 , wherein the intermediate segment includes a first segment having a material hardness of no more than 55D and a second segment located proximal to the first segment having a material hardness of no more than 72D.

A system including the device of claim 1 , further comprising the catheter, the catheter having a catheter lumen and a distal end, wherein an outer diameter of the flexible elongate body is sized to be positioned coaxially within the catheter lumen such that the tapered portion of the tip segment extends distally beyond the distal end of the catheter to aid in delivery of the catheter to an intracranial vessel.

6. The device of claim 1 , wherein a location of a material transition between the unreinforced polymer and the hypotube is at least about 49 cm from the distal end of the flexible elongate body.

7. The device of claim 6 , wherein the location allows for positioning the material transition proximal to the brachiocephalic take-off in the aortic arch when the distal end is positioned within a petrous portion of an internal carotid artery.

8. The device of claim 1 , wherein the hypotube has an inner diameter of about 0.021″ and an outer diameter of about 0.027″.

9. The device of claim 1 , wherein the first radiopaque marker is positioned on the first outer diameter and identifies a border between the first outer diameter and the tapered portion.

10. The device of claim 9 , wherein a second radiopaque marker is positioned on the second outer diameter.

11. The device of claim 1 , further comprising a rapid exchange opening through a wall of the catheter advancement device.

12. The device of claim 1 , wherein the hypotube is covered with a lubricious polymer.

13. The device of claim 12 , wherein the lubricious polymer is PTFE.

14. The device of claim 1 , wherein the second outer diameter is at a distal-most terminus of the flexible elongate body.

15. The device of claim 1 , wherein the single lumen of the flexible elongate body has an inner diameter of less than 0.024 inches.

16. The device of claim 1 , wherein a wall thickness of the intermediate segment and an untapered portion of the tip segment is about 0.050 inches to about 0.065 inches.

17. The device of claim 16 , wherein the wall thickness of the intermediate segment and the untapered portion of the tip segment is constant.

18. The device of claim 1 , wherein an inner diameter of the intermediate segment and the tip segment is constant.

19. The device of claim 1 , further comprising a guidewire extending within the single lumen so that a distal-most end of the guidewire is housed within the catheter advancement device proximal to a distal opening from the single lumen.

20. The device of claim 1 , wherein the length of the catheter advancement device is configured to extend to the neurovasculature.

21. The device of claim 1 , wherein a taper angle of a wall of the tapered portion relative to a center line of the tapered portion is between 2 to 10 degrees.

22. The device of claim 1 , wherein the length of the catheter advancement device is configured to extend to the neurovasculature and a flexibility of the catheter advancement device is sufficient to reach the neurovaculature.

The device of claim 22 , wherein a location of a material transition between the unreinforced polymer and the hypotube is at least about 49 cm from the distal end of the flexible elongate body and the location allows for positioning the material transition proximal to the brachiocephalic take-off in the aortic arch when the distal end is positioned within a petrous portion of an internal carotid artery.

The device of claim 22 , wherein the tip segment comprises a first radiopaque marker positioned on the first outer diameter and identifies a border between the first outer diameter and the tapered portion and wherein the tip segment comprises a second radiopaque marker positioned on the second outer diameter.

25. The device of claim 22 , wherein a wall thickness of the intermediate segment and an untapered portion of the tip segment is constant.

26. The device of claim 25 , wherein an inner diameter of the intermediate segment and the tip segment is constant.

27. The device of claim 25 , wherein the tip segment comprises at least a first radiopaque marker positioned on the first outer diameter that identifies a border between the first outer diameter and the tapered portion and a second radiopaque marker positioned on the second outer diameter.

28. An intravascular catheter advancement device for advancing a catheter within the vasculature, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment, wherein the proximal segment comprises a hypotube;

an intermediate segment comprising an unreinforced polymer having a first durometer; and

a tip segment formed of a polymer different from the unreinforced polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment has a tapered portion that tapers distally from a first outer diameter to a second outer diameter over a length so that a taper angle of a wall of the tapered portion relative to a center line of the tapered portion is between 0.9 to 1.6 degrees,

wherein the catheter advancement device has a length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature,

wherein a wall thickness of the intermediate segment and an untapered portion of the tip segment is constant.

29. The device of claim 28 , wherein the intermediate segment includes a first segment having a material hardness of no more than 55D and a second segment located proximal to the first segment having a material hardness of no more than 72D.

30. A system including the device of claim 28 , further comprising the catheter, the catheter having a catheter lumen and a distal end, wherein an outer diameter of the flexible elongate body is sized to be positioned coaxially within the catheter lumen such that the tapered portion of the tip segment extends distally beyond the distal end of the catheter to aid in delivery of the catheter to an intracranial vessel.

31. The device of claim 28 , wherein a location of a material transition between the unreinforced polymer and the hypotube is at least about 49 cm from the distal end of the flexible elongate body, and wherein the location allows for positioning the material transition proximal to the brachiocephalic take-off in the aortic arch when the distal end is positioned within a petrous portion of an internal carotid artery.

32. The device of claim 28 , wherein the tip segment comprises at least a first radiopaque marker positioned on the first outer diameter that identifies a border between the first outer diameter and the tapered portion and a second radiopaque marker positioned on the second outer diameter.

33. The device of claim 28 , wherein the hypotube is covered with a lubricious polymer.

34. The device of claim 28 , wherein the length of the catheter advancement device is configured to extend to the neurovasculature.

35. The device of claim 28 , wherein an inner diameter of the intermediate segment and the tip segment is constant.

36. The device of claim 28 , wherein the length of the catheter advancement device is configured to extend to the neurovasculature and a flexibility of the catheter advancement device is sufficient to reach the neurovaculature.

37. The device of claim 36 , wherein a location of a material transition between the unreinforced polymer and the hypotube is at least about 49 cm from the distal end of the flexible elongate body and the location allows for positioning the material transition proximal to the brachiocephalic take-off in the aortic arch when the distal end is positioned within a petrous portion of an internal carotid artery.

38. The device of claim 36 , wherein the tip segment comprises a first radiopaque marker positioned on the first outer diameter and identifies a border between the first outer diameter and the tapered portion and wherein the tip segment comprises a second radiopaque marker positioned on the second outer diameter.

39. The device of claim 36 , wherein a wall thickness of the intermediate segment and an untapered portion of the tip segment is constant.

40. The device of claim 39 , wherein an inner diameter of the intermediate segment and the tip segment is constant.

41. The device of claim 39 , wherein the tip segment comprises at least a first radiopaque marker positioned on the first outer diameter that identifies a border between the first outer diameter and the tapered portion and wherein the tip segment comprises a second radiopaque marker positioned on the second outer diameter.

42. An intravascular catheter advancement device for advancing a catheter within the vasculature, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment, wherein the proximal segment comprises a hypotube;

an intermediate segment comprising an unreinforced polymer having a first durometer;

a rapid exchange opening through a wall of the flexible elongate body; and

a tip segment formed of a polymer different from the unreinforced polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment has a tapered portion that tapers distally from a first outer diameter of between 0.048″ and 0.080″ to a second outer diameter of about 0.031″ up to about 0.048″ over a length between 0.5 cm and 4.0 cm,

wherein the catheter advancement device has a length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature.

43. An intravascular catheter advancement device for advancing a catheter within the vasculature, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment, wherein the proximal segment comprises a hypotube;

an intermediate segment comprising an unreinforced polymer having a first durometer; and

a tip segment formed of a polymer different from the unreinforced polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment has a tapered portion that tapers distally from a first outer diameter of between 0.048″ and 0.080″ to a second outer diameter at a distal-most terminus of the flexible elongate body that is about 0.031″ up to about 0.048″ over a length between 0.5 cm and 4.0 cm,

wherein the catheter advancement device has a length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature.

44. An intravascular catheter advancement device for advancing a catheter within the vasculature, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment, wherein the proximal segment comprises a hypotube;

an intermediate segment comprising an unreinforced polymer having a first durometer; and

a tip segment formed of a polymer different from the unreinforced polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment has a tapered portion that tapers distally from a first outer diameter of between 0.048″ and 0.080″ to a second outer diameter of about 0.031″ up to about 0.048″ over a length between 0.5 cm and 4.0 cm,

wherein an inner diameter of the intermediate segment and the tip segment is constant,

wherein the catheter advancement device has a length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature.

45. An intravascular catheter advancement device for advancing a catheter within the vasculature, the catheter advancement device comprising:

a flexible elongate body having a proximal end, a distal end, and a single lumen extending therebetween, the flexible elongate body comprising:

a proximal segment, wherein the proximal segment comprises a hypotube;

an intermediate segment comprising an unreinforced polymer having a first durometer; and

a tip segment formed of a polymer different from the unreinforced polymer of the intermediate segment, the polymer of the tip segment having a second durometer less than the first durometer, wherein the tip segment has a tapered portion that tapers distally from a first outer diameter to a second outer diameter over a length so that a taper angle of a wall of the tapered portion relative to a center line of the tapered portion is between 0.9 to 1.6 degrees,

wherein the catheter advancement device has a length configured to extend from outside a patient's body, through a femoral artery, and to the vasculature,

wherein an inner diameter of the intermediate segment and the tip segment is constant.
