Abstract

Dressings for treating a tissue site with negative pressure are disclosed, which may include a dressing having multiple layers and incorporating a material adapted to neutralize proteolytic enzymes. In one example embodiment, a dressing may include a first layer comprising a hydrophobic gel having a plurality of apertures. A material adapted to neutralize proteolytic enzymes may be applied to a surface of the first layer. The dressing may further include a second layer comprising a fenestrated film and coupled to the first layer. Additionally, the dressing may include a third layer comprising a manifold, which in some instances may be a polymeric foam. The dressing may further include a fourth layer comprising a polymer drape.

Claims (99)

1 . A dressing for treating a tissue site, comprising:

a first layer comprising a hydrophobic gel having at least one treatment aperture;

a second layer adjacent to the first layer, the second layer comprising a material adapted to neutralize proteolytic enzymes;

a third layer adjacent to the first layer, the third layer comprising a polymer film having a plurality of fluid restrictions that are configured to expand in response to a pressure gradient;

a fourth layer adjacent to the third layer and on the opposite side of the third layer from the first layer, the fourth layer comprising a manifold; and

a fifth layer adjacent to the fourth layer opposite the third layer, the fifth layer comprising a polymer drape.

2 . The dressing of claim 1 , wherein the material adapted to neutralize proteolytic enzymes comprises a sacrificial substrate.

3 . The dressing of claim 1 , wherein the material adapted to neutralize proteolytic enzymes comprises one or both of an enzyme sequestrator or an enzyme deactivator.

5 . The dressing of claim 1 , wherein the second layer comprises a plurality of perforations.

7 . The dressing of claim 1 , wherein the second layer comprises a biologically-derived polymer.

8 . The dressing of claim 1 , wherein the second layer comprises one or a combination of collagen, gelatin, collagen-like proteins, collagen-like peptides, cellulose, or cellulose derivative.

13 . The dressing of claim 1 , wherein the second layer comprises collagen and oxidized regenerated cellulose.

14 . The dressing of claim 1 , wherein the second layer forms a ring.

16 . The dressing of claim 1 , wherein the second layer comprises an opening aligned with the treatment aperture.

17 . The dressing of claim 1 , wherein the second layer comprises an opening having a width in a range of about 3 centimeters to about 35 centimeters.

18 . The dressing of claim 1 , wherein the second layer has a thickness of between 5 micrometers and 500 micrometers.

20 . The dressing of claim 1 , wherein the second layer comprises approximately 20-80% collagen and 80-20% ORC by weight.

21 . The dressing of claim 1 , wherein the second layer comprises approximately 55% collagen and 45% ORC by weight.

23 . The dressing of claim 1 , wherein the material adapted to neutralize proteolytic enzymes is present at a higher concentration in a perimeter portion than at a center portion of the second layer.

24 . The dressing of claim 1 , wherein the material adapted to neutralize proteolytic enzymes is present in a concentration having a circular gradient increasing from a center portion of the second layer to a perimeter of the second layer.

33 . The dressing of claim 1 , wherein the fluid restrictions of the third layer comprise a plurality of slots, each of the slots having a length of less than 4 mm and a width of less than 2 mm.

38 . A system for treating a tissue site, comprising:

the dressing of claim 1 ; and

a negative-pressure source adapted to be fluidly coupled to the dressing.

40 . A dressing for treating a tissue site with negative pressure, comprising:

a first layer comprising a perforated silicone gel coating;

a second layer coupled to the first layer and comprising an enzyme-modulating material; and

a polymer drape adjacent to the first layer opposite the second layer, the polymer drape comprising an adhesive coating.

42 . The dressing of claim 40 , further comprising:

a third layer positioned between the first layer and the polymer drape, the third layer comprising a non-porous material and a plurality of fenestrations; and

a manifold layer between the third layer and the polymer drape.

The dressing of claim 40 , wherein the enzyme-modulating material of the second layer comprises a sacrificial substrate, and wherein the sacrificial substrate comprises collagen, gelatin, chemically-modified cellulose, hyaluronic acid, chondroitin sulfate, collagen-mimic peptides, or combinations thereof.

46 . The dressing of claim 40 , wherein the enzyme-modulating material of the second layer comprises an enzyme deactivator, and wherein the enzyme deactivator comprises MMP inhibitors, small-molecule protease inhibitors, or combinations thereof.

48 . The dressing of claim 40 , wherein the enzyme-modulating material of the second layer comprises an enzyme sequestrator.

49 . The dressing of claim 48 , wherein the enzyme sequestrator comprises a chelating agent, and wherein the chelating agent comprises EDTA, EGTA, or a combination thereof.

51 . The dressing of claim 48 , wherein the enzyme sequestrator comprises a metalloprotein inhibitor having at least one zinc-binding group.

63 . A dressing for treating a tissue site with negative pressure, comprising:

a first layer comprising a hydrophobic gel having a plurality of apertures;

a second layer comprising a material adapted to neutralize proteolytic enzymes, the second layer having a plurality of fenestrations; and

a third layer adapted to be positioned adjacent to the first layer opposite the second layer, the third layer comprising a polymer drape.

64 . A dressing for treating a tissue site with negative pressure, comprising:

a first layer comprising a hydrophobic gel having a plurality of apertures;

a second layer adapted to be coupled to the first layer, the second layer comprising a polymer film having a plurality of fenestrations;

a third layer adapted to be positioned adjacent to the second layer opposite the first layer, the third layer comprising a manifold;

a fourth layer adapted to be positioned adjacent the third layer opposite the second layer, the fourth layer having a plurality of openings and comprising a material adapted to neutralize proteolytic enzymes; and

a fifth layer adapted to be coupled to the fourth layer opposite the third layer, the fifth layer comprising a polymer drape.

68 . A dressing for treating a tissue site, comprising:

a first layer comprising a hydrophobic gel adhesive and a material adapted to neutralize proteolytic enzymes, the first layer having a plurality of apertures;

a third layer adapted to be disposed between the first layer and the second layer, the third layer comprising a polymer film having a plurality of fenestrations.

71 . A dressing for treating a tissue site, comprising:

a hydrophobic gel layer;

an enzyme-modulating layer adjacent to the hydrophobic gel layer;

a fluid control layer adjacent to the hydrophobic gel layer opposite the enzyme-modulating layer; and

a manifold layer adjacent the fluid control layer opposite the hydrophobic gel layer, the manifold layer comprising a foam.
