Abstract

System and methods for channeling a path into bone include a trocar having a proximal end, distal end and a central channel disposed along a central axis of the trocar. The trocar includes a distal opening at or near the distal end of the trocar. The system includes a curved cannula sized to be received in the central channel, and having a curved distal end configured to be extended laterally outward from the distal opening in a curved path extending away from the trocar. The curved cannula has a central passageway having a diameter configured to allow a probe to be delivered through the central passageway to a location beyond the curved path.

Claims (57)

1. A method of ablating a nerve within a vertebral body, the method comprising:

inserting an introducer into cancellous bone of the vertebral body, the introducer comprising a channel extending through the introducer from a proximal end to a distal end;

inserting a radiofrequency energy delivery probe through the channel of the introducer and into the cancellous bone of the vertebral body,

wherein the radiofrequency energy delivery probe comprises:

a proximal handle;

an elongate member, the elongate member comprising a distal end portion that is configured to deflect toward a target treatment location within the cancellous bone upon actuation by an operator;

one or more electrodes; and

a temperature sensor; and

applying radiofrequency energy to the target treatment location via the one or more electrodes sufficient to ablate the nerve within the vertebral body.

2. The method of claim 1 , wherein:

the distal end portion of the elongate member comprises slots configured to facilitate bending of the distal end portion,

the one or more electrodes comprises a bipolar pair of electrodes, and

the nerve is a basivertebral nerve.

3. The method of claim 1 , wherein the distal end portion of the elongate member comprises slots configured to facilitate bending of the distal end portion.

4. The method of claim 1 , wherein the radiofrequency energy delivery probe comprises a sharp distal tip.

5. The method of claim 1 , wherein the nerve is a basivertebral nerve.

6. The method of claim 1 , wherein inserting the radiofrequency energy delivery probe through the channel of the introducer and into the cancellous bone of the vertebral body comprises creating a curved path to the target treatment location.

7. The method of claim 1 , wherein the vertebral body is a lumbar vertebral body, and wherein the target treatment location is in a posterior section of the lumbar vertebral body.

8. The method of claim 1 , wherein the vertebral body is a sacral vertebral body, and wherein the target treatment location is in a posterior section of the sacral vertebral body.

9. The method of claim 1 , wherein the vertebral body is a thoracic vertebral body, and wherein the target treatment location is in a posterior section of the thoracic vertebral body.

10. The method of claim 1 , wherein the proximal handle of the radiofrequency energy delivery probe comprises a thumb wheel.

11. The method of claim 1 , wherein the radiofrequency energy is configured to generate a peak temperature at the target treatment location of between 70 and 90 degrees Celsius, and wherein the radiofrequency energy is applied for between 5 and 30 minutes.

12. A method of ablating a nerve within a vertebral body, the method comprising:

advancing an introducer through cortical bone of the vertebral body and into cancellous bone of the vertebral body,

the introducer comprising a proximal handle and an elongate tube comprising a channel, the channel extending from the proximal handle to a distal end of the introducer;

inserting a radiofrequency energy delivery probe through the channel of the introducer and into the cancellous bone of the vertebral body,

wherein the radiofrequency energy delivery probe comprises:

a proximal handle;

an elongate member, the elongate member comprising a steerable distal end portion that is configured to deflect upon actuation by an operator;

one or more electrodes; and

a sensor;

positioning the steerable distal end portion at a target treatment location within the cancellous bone of the vertebral body corresponding to a portion of the vertebral body that includes at least a portion of the nerve; and

applying radiofrequency energy to the target treatment location via the one or more electrodes sufficient to ablate the nerve within the vertebral body.

13. The method of claim 12 , wherein:

the distal end portion of the elongate member comprises slots configure to facilitate bending of the distal end portion,

the nerve comprises a basivertebral nerve, and

the target treatment location comprises a location of a trunk of the basivertebral nerve.

14. The method of claim 12 , wherein the distal end portion of the elongate member comprises slots configure to facilitate bending of the distal end portion.

15. The method of claim 12 , wherein the nerve comprises a basivertebral nerve.

16. The method of claim 12 , wherein the vertebral body is a lumbar or sacral vertebral body, and wherein the target treatment location is in a posterior section of the lumbar or sacral vertebral body.

17. The method of claim 12 , wherein the radiofrequency energy is configured to generate a peak temperature at the target treatment location of between 70 and 90 degrees Celsius, and wherein the radiofrequency energy is applied for between 5 and 30 minutes.

18. A method of treating a nerve within a vertebral body, the method comprising:

advancing an introducer into cancellous bone of the vertebral body, the introducer comprising a proximal handle and an elongate tube comprising a channel, the channel extending from the proximal handle to a distal end of the introducer;

inserting a radiofrequency energy delivery probe through the channel of the introducer and into the cancellous bone of the vertebral body,

wherein the radiofrequency energy delivery probe comprises:

an elongate member, the elongate member comprising a steerable distal end portion that is configured to deflect upon actuation by an operator;

one or more electrodes; and

a temperature sensor;

positioning the steerable distal end portion of the elongate member of the radiofrequency energy delivery probe at a target treatment location within the cancellous bone of the vertebral body; and

applying radiofrequency energy to the target treatment location via the one or more electrodes sufficient to modulate the nerve within the vertebral body.

19. The method of claim 18 , wherein the radiofrequency energy is sufficient to ablate the nerve within the vertebral body.

20. The method of claim 18 , wherein:

the distal end portion of the elongate member comprises slots configure to facilitate lateral bending of the distal end portion,

the radiofrequency energy is configured to generate a peak temperature at the target treatment location of between 70 and 90 degrees Celsius,

the radiofrequency energy is applied for between 5 and 30 minutes,

the nerve comprises a basivertebral nerve, and

the one or more electrodes comprises two electrodes.
