Abstract

A method of managing insulin includes receiving blood glucose measurements on a computing device from a glucometer. The blood glucose measurements are separated by a time interval. The method includes determining, by the computing device, an insulin dose rate based on the blood glucose measurements and determining a blood glucose drop rate based on the blood glucose measurements and the time interval. The method also includes determining a blood glucose percentage drop based on the blood glucose measurements. The method includes decreasing the time interval between blood glucose measurements by the glucometer when the blood glucose drop rate is greater than a threshold drop rate, and decreasing the time interval between blood glucose measurements by the glucometer when the blood glucose percentage drop is greater than a threshold percentage drop.

Claims (60)

1. A computer-implemented method that when executed on data processing hardware causes the data processing hardware to perform operations comprising:

receiving, from an electronic medical record (EMR) system in communication with the data processing hardware:

sequential blood glucose measurements of a patient, the sequential blood glucose measurements comprising a current blood glucose measurement measured at a current time;

a target blood glucose range for the patient comprising a range of blood glucose values between and including a lower limit blood glucose value and an upper limit blood glucose value greater than the lower limit blood glucose value; and

a specified number of consecutive blood glucose measurements of the patient within the target blood glucose range to determine a blood glucose concentration for the patient is stable in the target blood glucose range independent of a duration of time the blood glucose concentration for the patient is stable in the target blood glucose range;

determining whether the current blood glucose measurement is less than a threshold hypoglycemia blood glucose value, the threshold hypoglycemia blood glucose value less than the lower limit blood glucose value of the target blood glucose range; and

when the current blood glucose measurement is less than the threshold hypoglycemia blood glucose value:

determining a recommended dose of intravenous glucose for administration to the patient intravenously or oral glucose for ingestion by the patient;

determining a hypoglycemia time interval from the current time until a next time of a next scheduled blood glucose measurement for the patient based on the current blood glucose measurement, the hypoglycemia time interval being shorter than a long time interval that is determined from the current time until the next time of the next scheduled blood glucose measurement for the patient when the current blood glucose measurement and the one or more previous blood glucose measurements am within the target blood glucose range and equal the specified number of consecutive blood glucose measurements within the target blood glucose range;

setting a countdown timer equal to the hypoglycemia time interval, and

displaying, in a graphical user interface displayed on a screen in communication with the data processing hardware;

the recommended dose of intravenous glucose or oral glucose; and

the countdown timer to indicate a remaining time until the next time of the next scheduled blood glucose measurement for the patient.

2. The computer-implemented method of claim 1 , wherein the operations further comprise executing an intravenous treatment process configured to display the graphical user interface on the screen, wherein the intravenous treatment process is configured to:

display, in the graphical user interface, a patient information input window;

receive, in the patient information input window, the target blood glucose range for the patient; and

receive, in the patient information input window, the specified number of consecutive blood glucose measurements of the patient within the target blood glucose range to determine the blood glucose concentration for the patient is stable in the target blood glucose range.

The computer-implemented method of claim 2 , wherein the intravenous treatment process is further configured to receive, in the graphical user interface, a blood glucose input from the medical professional for a next blood glucose measurement measured at a subsequent time after the current time.

4. The computer-implemented method of claim 1 , wherein the operations further comprise displaying, in the graphic user interface, a blood glucose due message at the end of the hypoglycemia time interval indicating that the next scheduled blood glucose measurement for the patient is due.

5. The computer-implemented method of claim 1 , wherein the operations further comprise:

determining an intravenous insulin infusion rate based on the current blood glucose measurement,

displaying, in the graphical user interface, the intravenous infusion rate indicating a number of units of insulin within a configured time interval for administration to the patient intravenously.

The computer-implemented method of claim 5 , wherein the operations further comprise transmitting the intravenous insulin infusion rate to an insulin administration device in communication with the data processing hardware, the insulin administration rate when received by the insulin administration device, causing the insulin administration device to administer insulin to the patient using the intravenous insulin infusion rate.

7. The computer-implemented method of claim 5 , wherein determining the intravenous insulin infusion rate comprises calculating:

wherein IIR is the intravenous insulin infusion rate, BG is the current blood glucose measurement, K is a constant, and M is a multiplier.

8. The computer-implemented method of claim 7 , wherein the operations further comprise, in response to receiving an indication of patient solid food consumption, increasing the intravenous insulin infusion rate and maintaining the multiplier unchanged for at least two subsequent next time intervals.

9. The computer-implemented method of claim 7 , wherein K is equal to 60.

10. The computer-implemented method of claim 7 , wherein receiving the indication of patient solid food consumption comprises receiving, from the EMR system, nutritional information and a number of grams of carbohydrates consumed by the patient.

11. M system comprising:

data processing hardware; and

memory hardware in communication with the data processing hardware and storing instructions that when executed on the data processing hardware causes the data processing hardware to perform operations comprising:

receiving, from an electronic medical record (EMR) system in communication with the data processing hardware:

sequential blood glucose measurements of a patient, the sequential blood glucose measurements comprising a current blood glucose measurement measured at a current time;

a target blood glucose range for the patient comprising a range of blood glucose values between and including a lower limit blood glucose value and an upper limit blood glucose value greater than the lower limit blood glucose value, and

a specified number of consecutive blood glucose measurements of the patient within the target blood glucose range to determine a blood glucose concentration for the patient is stable in the target blood glucose range independent of a duration of time the blood glucose concentration for the patient is stable in the target blood glucose range;

determining whether the current blood glucose measurement is less than a threshold hypoglycemia blood glucose value, the threshold hypoglycemia blood glucose value less than the lower limit blood glucose value of the target blood glucose range; and

when the current blood glucose measurement is less than the threshold hypoglycemia blood glucose value:

determining a recommended dose of intravenous glucose for administration to the patient intravenously or oral glucose for ingestion by the patient;

determining a hypoglycemia time interval from the current time until a next time of a next scheduled blood glucose measurement for the patient based on the current blood glucose measurement, the hypoglycemia time interval being shorter than a long time interval that is determined from the current time until the next time of the next scheduled blood glucose measurement for the patient when the current blood glucose measurement and the one or more previous blood glucose measurements are within the target blood glucose range and equal the specified number of consecutive blood glucose measurements within the target blood glucose range,

setting a countdown timer equal to the hypoglycemia time interval; and

displaying, in a graphical user interface displayed on a screen in communication with the data processing hardware:

the recommended dose of intravenous glucose or oral glucose, and

the countdown timer to indicate a remaining time until the next time of the next scheduled blood glucose measurement for the patient.

12. The system of claim 11 , wherein the operations further comprise executing an intravenous treatment process configured to display the graphical user interface on the screen, wherein the intravenous treatment process is configured to:

display, in the graphical user interface, a patient information input window,

receive, in the patient information input window, the target blood glucose range for the patient; and

receive, in the patient information input window, the specified number of consecutive blood glucose measurements of the patient within the target blood glucose range to determine the blood glucose concentration for the patient is stable in the target blood glucose range.

13. The system of claim 12 , wherein the intravenous treatment process is further configured to receive, in the graphical user interface, a blood glucose input from the medical professional for a next blood glucose measurement measured at a subsequent time after the current time.

14. The system of claim 11 , wherein the operations further comprise displaying, in the graphic user interface, a blood glucose due message at the end of the hypoglycemia time interval indicating that the next scheduled blood glucose measurement for the patient is due.

15. The system of claim 11 , wherein the operations further comprise:

determining an intravenous insulin infusion rate based on the current blood glucose measurement,

displaying, in the graphical user interface, the intravenous infusion rate indicating a number of units of insulin within a configured time interval for administration to the patient intravenously.

The system of claim 15 , wherein the operations further comprise transmitting the intravenous insulin infusion rate to an insulin administration device in communication with the data processing hardware, the insulin administration rate when received by the insulin administration device, causing the insulin administration device to administer insulin to the patient using the intravenous insulin infusion rate.

17. The system of claim 15 , wherein determining the intravenous insulin infusion rate comprises calculating:

wherein HR is the intravenous insulin infusion rate, BG is the current blood glucose measurement, K is a constant, and M is a multiplier.

18. The system of claim 17 , wherein the operations further comprise, in response to receiving an indication of patient solid food consumption, increasing the intravenous insulin infusion rate and maintaining the multiplier unchanged for at least two subsequent next time intervals.

19. The system of claim 17 , wherein K is equal to 60.

20. The system of claim 7 , wherein receiving the indication of patient solid food consumption comprises receiving, from the EMR system, nutritional information and a number of grams of carbohydrates consumed by the patient.
