Abstract

Adjacent tissue layers can be accessed using a catheter device with a distal tip having a conductive portion including a first cutting feature and one or more projections extending from the first cutting feature towards an outer diameter of the distal tip. Electrical energy can be supplied to the conductive portion of the device to cut tissue. A stent can be delivered to form a fluid communication between the adjacent tissue layers.

Claims (40)

1. A distal tip for a catheter, comprising:

a conically shaped tip having a proximal end having a first diameter, a distal end having a second diameter, and a lumen extending therethrough, wherein the first diameter is greater than the second diameter;

a distal tip base disposed on the proximal end of the conically shaped tip, the lumen extending through the distal tip base; and

a conductive cutting member coupled to the distal end of the conically shaped tip, the conductive cutting member comprises a forward facing cutting feature oriented perpendicular to a longitudinal axis of the distal tip and at least one outward projecting cutting feature,

wherein the forward facing cutting feature is disposed on the distal end of the conically shaped tip and comprises an arced configuration and has a diameter smaller than the second diameter and larger than a diameter of the lumen, and

wherein the outward projecting cutting feature extends from the forward facing cutting feature along at least a portion of the conically shaped tip in a proximal direction and extends away from the forward facing cutting feature as the outward projecting cutting feature extends proximally.

2. The distal tip of claim 1 , wherein the conically shaped tip comprises an insulating portion.

3. The distal tip of claim 2 , wherein the insulating portion comprises an insulating sleeve, or an insulating collar, or both.

4. The distal tip of claim 2 , wherein the forward facing cutting feature comprises a semi-circle, wherein the diameter of the semi-circle is substantially equal to an outside diameter of the lumen.

5. The distal tip of claim 4 , wherein the at least one outward projecting cutting feature comprises a first outward projecting cutting feature and a second outward projecting cutting feature.

The distal tip of claim 5 , wherein the insulating portion comprises a first insulating portion and a second insulating portion and wherein the first outward projecting cutting feature and the second outward projecting cutting feature are disposed between the first insulating portion and the second insulating portion.

The distal tip of claim 1 , wherein the conductive cutting member comprising a first conductive cutting member, wherein the distal tip comprising a second conductive cutting member, and wherein the second conductive cutting member comprising a forward facing cutting feature and at least one outward projecting cutting feature.

The distal tip of claim 7 , wherein the forward facing cutting feature of the first conductive cutting member comprises a semi-circle and the forward facing cutting feature of the second conductive cutting member comprises a semi-circle, and wherein the first conductive cutting member and the second conductive cutting member are disposed on the distal tip such that the first conductive cutting member mirrors the second conductive cutting member to form a circular cutting member.

9. A catheter assembly, comprising:

an outer sheath having a lumen extending therethrough;

an inner body having a proximal end, a distal end, and a passage extending therethrough, a portion of the inner body receivable within the lumen of the outer sheath; and

a distal tip positioned proximate to the distal end of the inner body, the distal tip comprising:

a conically shaped tip having a proximal end having a first diameter, a distal end having a second diameter, and a guidewire lumen extending therethrough, wherein the first diameter is greater than the second diameter and wherein the guidewire lumen coincides with the passage;

a distal tip base disposed on the proximal end of the conically shaped tip, the guidewire lumen extending through the distal tip base; and

a conductive cutting member coupled to the distal end of the conically shaped tip, the conductive cutting member comprises a forward facing cutting feature oriented perpendicular to a longitudinal axis of the distal tip and at least one outward projecting cutting feature,

wherein the forward facing cutting feature is disposed on the distal end of the conically shaped tip and comprises an arced configuration and has a diameter smaller than the second diameter and larger than a diameter of the guidewire lumen.

10. The catheter assembly of claim 9 , wherein the distal tip comprises an insulating portion, the insulating portion comprising an insulating sleeve, or an insulating collar, or both.

11. The catheter assembly of claim 10 , wherein the forward facing cutting feature comprises a semi-circle, wherein the diameter of the semi- circle is substantially equal to an outside diameter of the inner body.

12. The catheter assembly of claim 11 , wherein the at least one outward projecting cutting feature comprises a first outward projecting cutting feature and a second outward projecting cutting feature.

The catheter assembly of claim 12 , wherein the first outward projecting cutting feature and the second outward projecting cutting feature extend along a longitudinal axis of the conically shaped tip from the distal end of the conically shaped tip towards the proximal end of the conically shaped tip and extend along a radial axis away from the longitudinal axis of the conically shaped tip.

The catheter assembly of claim 12 , wherein the insulating portion comprises a first insulating portion and a second insulating portion and wherein the first outward projecting cutting feature and the second outward projecting cutting feature are disposed between the first insulating portion and the second insulating portion.

The catheter assembly of claim 9 , wherein the conductive cutting member comprising a first conductive cutting member, wherein the distal tip comprising a second conductive cutting member, and wherein the second conductive cutting member comprising a forward facing cutting feature and at least one outward projecting cutting feature.

The catheter assembly of claim 15 , wherein the forward facing cutting feature of the first conductive cutting member comprises a semi- circle and the forward facing cutting feature of the second conductive cutting member comprises a semi-circle, and wherein the first conductive cutting member and the second conductive cutting member are disposed on the distal tip such that the first conductive cutting member mirrors the second conductive cutting member to form a circular cutting feature.

17. A catheter assembly, comprising:

an outer sheath having a lumen extending therethrough;

an inner body having a proximal end, a distal end, and a passage extending therethrough, a portion of the inner body receivable within the lumen of the outer sheath;

a distal tip positioned proximate to the distal end of the inner body, the distal tip comprising:

a conically shaped tip having a proximal end having a first diameter, a distal end having a second diameter, and a guidewire lumen extending therethrough, wherein the first diameter is greater than the second diameter and wherein the guidewire lumen coincides with the passage;

a distal tip base disposed on the proximal end of the conically shaped tip, the guidewire lumen extending through the distal tip base; and

a conductive cutting member coupled to the distal end of the conically shaped tip, the conductive cutting member comprises a forward facing cutting feature oriented perpendicular to a longitudinal axis of the distal tip and at least one outward projecting cutting feature,

wherein the forward facing cutting feature is disposed on the distal end of the conically shaped tip and comprises an arced configuration and has a diameter smaller than the second diameter and larger than a diameter of the guidewire lumen; and

a stent constrained on the inner body.

18. The catheter assembly of claim 17 , wherein the stent is releasably constrained in an undeployed configuration between the outer sheath and the inner body.

19. The catheter assembly of claim 18 , wherein the stent is configured to self-expand from the undeployed configuration to a deployed configuration when released from between the outer sheath and the inner body.

The catheter assembly of claim 17 , wherein the at least one outward projecting cutting feature comprises a first outward projecting cutting feature and a second outward projecting cutting feature, and wherein the first outward projecting cutting feature and the second outward projecting cutting feature extend along a longitudinal axis of the conically shaped tip from the distal end of the conically shaped tip towards the proximal end of the conically shaped tip and extend along a radial axis away from the longitudinal of te conically shaped tip axis.
