Abstract

The present disclosure describes treatment of the vasculature of a patient with an expandable implant. The implant is constrained to a reduced delivery diameter for delivery within the vasculature by at least one sleeve. The implant can be constrained to other diameters, such as an intermediate diameter. The sleeves can be expanded, allowing for expansion of the diameter of the expandable implant, by disengaging a coupling member from the sleeve or sleeves from outside of the body of the patient. The expandable implant can comprise a steering line or lines which facilitate bending and steering of the expandable implant through the vasculature of a patient.

Claims (35)

1 . A delivery system comprising:

a catheter shaft;

an expandable device arranged on the catheter shaft, the expandable device including a trunk portion defining a trunk lumen and at least two leg portions defining branch lumens, wherein the trunk lumen and the branch lumens are in fluid communication with each other;

an axially displaceable sheath extending around the expandable device and configured to releasably constrain the expandable device in a collapsed configuration during insertion of the delivery system in a patient, and

a constraint disposed around at least a portion of the expandable device and configured to constrain the expandable device in an intermediate configuration when the expandable device is released from the axially displaceable sheath such that the expandable device is retained in an intermediate configuration when the axially displaceable sheath is removed from an entirety of the constraint, and wherein the constraint is configured to release the expandable medical device to a deployed configuration; and

a steering wire configured to bend at least a portion of the expandable device in response to selective displacement of the steering wire relative to the catheter.

2 . The delivery system of claim 1 , wherein the constraint includes a coupling member that, when actuated, results in release of the constraint.

3 . The delivery system of claim 2 , wherein the steering wire is coupled to the coupling member.

4 . The delivery system of claim 3 , wherein the steering wire includes a loop which is coupled to the coupling member.

5 . The delivery system of claim 1 , further comprising a steering wire conduit within which at least portion of the steering wire is positioned.

6 . The delivery system of claim 5 , wherein the steering wire conduit is at least partially positioned within at least one of the trunk lumen and the branch lumens.

7 . The delivery system of claim 1 , wherein the steering wire is configured to bi-directionally control movement of the expandable device in response to selective displacement by a clinician.

8 . The delivery system of claim 7 , wherein the steering wire is configured to be pulled and pushed to bend and unbend the expandable device.

9 . The delivery system of claim 1 , wherein the steering wire comprises a metallic material.

10 . The delivery system of claim 1 , further comprising a pusher disposed in a lumen of the catheter shaft.

11 . The delivery system of claim 10 , wherein the pusher is axially slidable relative to the axially displaceable sheath to limit movement of the expandable device when the axially displaceable sheath is displaced.

12 . A delivery system comprising:

a catheter shaft;

an expandable device having a proximal end and a distal end and configured to diametrically expand from a delivery configuration having a first diameter along at least a portion of a length of the expandable device to an intermediate configuration having a second diameter that is greater than the first diameter along the at least the portion of the length of the expandable device and to diametrically expand from the intermediate configuration to a deployed configuration having a third diameter that is greater than the second diameter along the at least the portion of the length of the expandable device, wherein the expandable device includes a trunk portion and at least two branch portions;

a constraint disposed around a portion of the expandable device along the at least the portion of the length of the expandable device and configured to constrain the expandable device in the intermediate configuration along the at least the portion of the length of the expandable device such that the expandable device is retained in the intermediate configuration along an entirety of the expandable device, and release the expandable medical device to the deployed configuration along the at least the portion of the length of the expandable device; and

a steering wire configured to bend at least a portion of the expandable device in response to selective displacement of the steering wire relative to the catheter shaft, the steering wire being configured to bi-directionally control movement of the expandable device in response to selective displacement by a clinician.

13 . The system of claim 12 , further comprising an axially displaceable sheath, wherein the axially displaceable sheath is configured to hold one of the pair of legs in the collapsed position.

14 . The system of claim 12 , wherein the steering wire is coupled to the expandable device by a loop.

15 . The system of claim 12 , wherein the steering wire is configured to be pulled to bend the expandable device in a first direction and to be pushed to unbend the expandable device in a second direction opposite the first direction.

16 . The system of claim 12 , wherein the at least a portion of the expandable device is bendable by actuation of the steering wire while the expandable device is maintained in the intermediate configuration.

17 . The system of claim 12 , wherein the steering wire is operable to steer the trunk portion of the expandable device.

18 . A method comprising:

arranging a catheter shaft including an expandable device arranged on the catheter shaft at a treatment site;

displacing an axially displaceable sheath extending around the expandable device to allow outward expansion of the expandable device from a collapsed configuration having a first diameter toward an intermediate configuration having a second diameter that is greater than the first diameter along at least a portion of a length of the expandable device such that the expandable device is retained in an intermediate configuration when the axially displaceable sheath is removed from an entirety of the expandable device;

actuating a steering wire to bend at least a portion of the expandable device in response to selective displacement of the steering wire relative to the catheter; and

actuating a constraint to release the expandable device from the intermediate configuration toward a deployed configuration having a third diameter that is greater than the second diameter along the at least the portion of the length of the expandable device.

19 . The method of claim 18 , wherein the expandable device defines a trunk lumen and a pair of legs positioned at a leading end, the expandable device having a proximal end and a distal end.

20 . The method of claim 19 , wherein displacing the axially displaceable sheath includes holding one of the pair of legs in the collapsed position.

21 . The method of claim 18 , wherein actuating the steering wire includes bi-directionally controlling movement of the expandable device in response to selective displacement of the steering wire by a clinician.

22 . The method of claim 18 , wherein the at least a portion of the expandable device is bendable by actuation of the steering wire while the expandable device is maintained in the intermediate configuration.
