Abstract

A spinal implant apparatus that is an expandable spacer including features to minimize or eliminate spacer cant or offset during and after completing the expansion process. The spacer includes a top component, a base component in engagement with the top component, and an expansion mechanism arranged to change the top component's position with respect to the base component. The mechanism for causing expansion may be a screw, a cam, a wedge or other form of distracting device. In one embodiment, the expandable spacer includes a base component with a set of towers and a top component with a set of corresponding silos, where the towers and silos are configured to minimize or eliminate tilt of the top component as it extends upwardly from the base component.

Claims (23)

1 . An expandable intervertebral spacer, comprising:

a spacer body having a longitudinal dimension extending along a longitudinal axis between a proximal end and a distal end, the spacer body including a top component having a top surface and a base component having a bottom surface, the base component being engaged with the top component so as to permit movement of the top and base components relative to one another along a height dimension to thereby vary a distance between the top and bottom surfaces along the height dimension;

at least one distal guide and at least one proximal guide, the at least one distal guide and the at least one proximal guide being configured to guide the movement of the top and base components of the spacer body along the height dimension, and the at least one distal guide and the at least one proximal guide being spaced apart from one another along the longitudinal dimension of the spacer so as to define an intermediate region therebetween; and

an actuator configured to be actuated by advancing along an actuation axis transverse to the longitudinal axis so as to drive expansion of the spacer body, the advancing of the actuator along the actuation axis including progressive movement of the actuator into the spacer body along the actuation axis as actuation progresses, and the progressive movement of the actuator into the spacer body along the actuation axis inducing the expansion of the spacer body by progressively increasing the distance between the top surface of the top component and the bottom surface of the base component, and the actuation axis being arranged such that actuation of the actuator advances the actuator into the spacer body along the actuation axis towards the intermediate region between the at least one proximal guide and the at least one distal guide.

2 . The spacer of claim 1 , wherein the base component is configured to receive an entirety of the top component therein.

3 . The spacer of claim 1 , wherein the at least one distal guide includes a distal passageway defined therein, and wherein the at least one proximal guide includes a proximal passageway defined therein.

4 . The spacer of claim 3 , wherein the distal passageway extends from a distal opening in the top surface to a distal opening in the bottom surface, and wherein the proximal passageway extends from a proximal opening in the top surface to a proximal opening in the bottom surface.

5 . The spacer of claim 1 , wherein each of the at least one distal guide and the at least one proximal guide includes a respective tower telescopically slidable within a respective silo.

The spacer of claim 5 , wherein each tower of the at least one distal and proximal guides includes an interior space that communicates with an interior space of the respective silo within which the tower telescopically slides, the interior space of each tower being aligned with the interior space of the respective silo so as to collectively define a passageway.

7 . The spacer of claim 1 , wherein the actuator is configured to directly engage the top component.

8 . The spacer of claim 7 , wherein the actuator is configured to directly engage a tapered surface of the top component.

9 . The spacer of claim 8 , wherein the tapered surface of the top component is threaded for engagement with threads of the actuator.

10 . The spacer of claim 1 , wherein the actuator is configured to advance into the spacer body along a threaded and tapered slot of the top component and a threaded and tapered slot of the bottom component.

11 . The spacer of claim 1 , wherein the actuator is a rotational actuator.

12 . The spacer of claim 11 , wherein the actuator includes a screw.

13 . The spacer of claim 1 , wherein the actuator includes a wedge.

14 . The spacer of claim 1 , further comprising a port configured to receive bone graft material.

15 . The spacer of claim 1 , further comprising a plurality of engagement interfaces for engagement with an insertion tool.

16 . The spacer of claim 15 , wherein the engagement interfaces include an off-axis positioning interface site and an on-axis positioning interface site relative to the longitudinal axis of the spacer.

17 . The spacer of claim 1 , wherein the actuation axis is oriented at an oblique angle relative to the longitudinal axis of the spacer.

18 . The spacer of claim 1 , wherein the top surface of the top component and the bottom surface of the bottom component are textured, ribbed, sawtoothed, or dovetailed.

19 . The spacer of claim 1 , wherein the top component and the bottom component include a plurality of steps configured to interlock with one another to restrict retraction of the spacer body from an expanded state.

20 . The spacer of claim 1 , wherein the top surface of the top component is parallel to the bottom surface of the bottom component.
